Dislocation analysis on bio-inspired open hip joint with active-and-passive stabilizers.

This study presents a simulator framework to replicate joint dislocation that can be used to evaluate the contributions of various joint stabilizers. Compared to conventional axial joint configurations, biologically inspired open joints allow for a large range of motion with fewer rotational components and no singularities. Despite these advantages, open joints have few practical applications due to joint dislocation. To facilitate the development of bio-inspired open joints, our study evaluates the performance of biomimetic joint stabilizers involving (1) the muscle actuators as the active st

Load Management After Gluteal Tendon Repair: A Controlled Laboratory Study.

Tendon lesions of the gluteus medius muscle require surgical repair if conservative treatment fails. Postoperatively, the success of the therapy is influenced by the loading conditions during follow-up treatment. Excessive loading of the hip joint during walking after surgery - and consequently, potential overload of the repaired gluteal tendon - could jeopardize the success of the therapy. In this controlled laboratory study, six subjects performed an instrumented 3D gait analysis using optoelectronic motion capture under three different loading conditions: full weight-bearing, partial weight

Dermal Allograft Augmentation for Open Gluteus Medius Repair: A Case Series at 2-Year Follow-up.

BACKGROUND: Gluteus medius (GM) tears are a common cause of peritrochanteric regional pain and disability among middle-aged and older populations. While largely successful, when primary repair fails, it is often attributed to poor tendon quality. Dermal allograft augmentation offers a solution for this patient population by enhancing the biological and mechanical properties of the repair construct. PURPOSE: To evaluate the outcomes of open isolated GM repair with dermal allograft augmentation at 2-year follow-up, with a sex-based subanalysis comparing clinical outcomes in male versus female p

Case Report: PET-avid compensatory tensor fascia lata hypertrophy mimicking metastatic disease in a patient with a chronic Achilles tendon rupture.

In this case report, we describe an unusual case of PET-avid muscle enlargement in the right tensor fascia lata (TFL) muscle mimicking metastatic disease during lung cancer staging. The patient, a 67-year-old woman, had a history of left Achilles tendon rupture, which had been managed non-operatively. A PET/CT scan revealed focal fluorodeoxyglucose (FDG) uptake in a markedly expanded right TFL muscle. The presence of concurrent ipsilateral pelvic lymphadenopathy on the scan initially raised concern for muscle metastasis in this context. However, subsequent MRI of the hip showed diffuse muscle

Comparison between moderate-load and high-load exercises in the rehabilitation of runners with Achilles tendinopathy: Protocol for a blind randomized controlled trial.

BACKGROUND: Achilles tendinopathy is one of the most frequent conditions affecting runners. Exercise-based interventions are the main treatment options for Achilles tendinopathy; however, the ideal exercise dosage remains unknown. OBJECTIVE: The objective of this study is to investigate the effects of a moderate-load exercise intervention compared to a high-load exercise intervention in runners with Achilles tendinopathy in terms of pain, symptom severity, quality of life, muscle strength, and function. METHODS: Sixty amateur runners will be randomly allocated to one of two groups: high-load

A Biomechanical Comparison of Gluteus Medius Repair With and Without Iliotibial Band Autograft Augmentation.

BACKGROUND: Gluteus medius tears are an increasingly recognized cause of lateral hip pain in active individuals, yet no gold standard repair technique has been established, and failure and retear rates remain high. Iliotibial band (ITB) graft augmentation is a validated option in rotator cuff and hip labral repairs for stronger repair construct, but its biomechanical utility in gluteus medius repairs has not been investigated. HYPOTHESIS: It was hypothesized that ITB-augmented gluteus medius repairs would demonstrate superior ultimate load to failure and increased repair stiffness as compared

Can a Clinician Accurately Diagnose Iliopsoas Tendinitis on a Physical Examination?

BACKGROUND: Iliopsoas (IP) tendinitis is an underrecognized but important cause of anterior hip pain in active patients. Some studies have reported that a physical examination (PE) has limited reliability in making a diagnosis of IP tendinitis. Differentiating anterior hip pain due to IP tendinitis from intra-articular sources is critical to appropriately and expediently treat patients. HYPOTHESIS/PURPOSE: PE techniques can detect IP tendinitis, and we sought to quantify the diagnostic utility of these techniques using responses to an ultrasound-guided anesthetic injection as the reference st

The Assessment of the Tendon of Insertion of the Iliopsoas Muscle in Dogs Using Diagnostic Ultrasound Has Good Intraobserver Consistency but Lacks Interobserver Consistency.

The iliopsoas muscle (IPM) is a flexor of the hip joint in dogs that is vulnerable to injury. Intraobserver and interobserver reliability of musculoskeletal ultrasound when evaluating the tendon of insertion of the IPM was studied. The IPM tendons of insertion of a randomly selected cohort were screened by one investigator and recorded. Musculoskeletal ultrasound recordings were separated into two groups of 20 dogs with presumptively normal IPM tendons of insertion and 32 dogs with one or two presumptively abnormal IPM tendons. Recordings were anonymized. The 104 tendons from these 52 dogs wer

Abductor Tendon Tears in the Native and Prosthetic Hip.

Gluteal tendon tears are an increasingly recognized source of laterally based hip pain. These tendon tears are typically degenerative in nature and require careful history, physical examination, and imaging interpretation for accurate diagnosis. After a trial of nonoperative management, open or endoscopic repair can predictably improve pain, though expected functional improvement is correlated to the degree of preoperative muscle atrophy. The authors advocate for an open double-row suture technique to reliably visualize the full extent of the tear, restore the anatomic footprint of the avulsed

Increased contralateral dynamic valgus in female athletes following ACL reconstruction.

PURPOSE: Female athletes have a higher risk of anterior cruciate ligament (ACL) injury, which has been associated with neuromuscular deficits and dynamic valgus, while contralateral risk profiles after ACL reconstruction remain insufficiently characterized. The purpose of this study was to investigate whether female patients after ACL reconstruction exhibit greater contralateral valgus angles compared with male patients, indicating a higher risk profile for secondary ACL injury. It was hypothesized that dynamic valgus would be more pronounced in women than in men. METHODS: In this predefined

Clinical, Radiological, and Kinematic Impact of Gluteus Maximus Tendon Release During Total Hip Arthroplasty: A Prospective Randomized Controlled Trial.

BACKGROUND: Release of the gluteus maximus tendon during a total hip arthroplasty (THA) can lead to gluteal dysfunction. Sciatic nerve injury, while rare, remains a serious complication of THA. During deep hip flexion, release of the tendon increases the distance between the femoral neck and the sciatic nerve, potentially reducing the risk of nerve injury. The functional consequences of such a release have yet to be investigated. The aim of this study was to evaluate the effects of gluteus maximus tendon release on the development of gluteal dysfunction using clinical, radiological, and kinema

Correlation analysis between PFNA insertion point and neck-shaft angle in intertrochanteric fractures, and selection of PFNA (240 mm) under varying radius of femoral curvature.

BACKGROUND: The Intramedullary fixation of unstable intertrochanteric fractures is usually performed using the proximal femoral nail anti-rotation (PFNA), but the problems of optimal insertion point of intramedullary nail and anterior cortical impingement have not been solved.Meanwhile, the variation of the femoral neck-shaft angle (NSA) may also result in the deviation of the PFNA's insertion point. The purpose of this study to determine the ideal intramedullary nail placement position in the treatment of intertrochanteric fractures through the three-dimensional (3D) models, and to investigat

Editorial Commentary: Preoperative Glucagon-Like Peptide-1 Receptor Agonist Use May Improve Outcomes After Arthroscopic Rotator Cuff Repair but Beneficial Effects May Vary Across Patient Subgroups.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have rapidly emerged as a cornerstone therapy for weight loss and metabolic optimization in obese patients. In orthopaedic surgery, these agents have been associated with improved perioperative risk profiles in patients undergoing hip and knee arthroplasty surgery; however, their impact on outcomes after arthroscopic rotator cuff repair remains incompletely understood. Recent evidence suggests that preoperative GLP-1 RA use in obese patients undergoing arthroscopic rotator cuff repair is generally safe, may reduce health care utilization, a

Pain Sensitivity, Psychological Factors, and Muscle Function in Male Athletes With Long-Standing Groin Pain and Matched Controls.

Long-standing groin pain in athletes is often managed as a localized musculoskeletal tissue injury. Treatment-resistant symptoms and disability after return to sport suggest that other factors may also contribute. This study compared pain sensitivity, psychological factors, and muscle function between male athletes with long-standing groin pain (N = 20) and sex-, age-, and sports-matched controls (N = 20). Pressure-pain thresholds were assessed with a handheld algometer at the adductor longus muscle and origin, iliopsoas tendon, pubic bone, pubic symphysis, gluteus maximus muscle, and elbow. P
