Pain Sensitivity, Psychological Factors, and Muscle Function in Male Athletes With Long-Standing Groin Pain and Matched Controls.

Long-standing groin pain in athletes is often managed as a localized musculoskeletal tissue injury. Treatment-resistant symptoms and disability after return to sport suggest that other factors may also contribute. This study compared pain sensitivity, psychological factors, and muscle function between male athletes with long-standing groin pain (N = 20) and sex-, age-, and sports-matched controls (N = 20). Pressure-pain thresholds were assessed with a handheld algometer at the adductor longus muscle and origin, iliopsoas tendon, pubic bone, pubic symphysis, gluteus maximus muscle, and elbow. P

The effect of hip abductor or external rotator strength on dynamic knee valgus in healthy subjects-a systematic review with partial meta-analysis.

BACKGROUND: Weak hip abductors and external rotators (ER) have long been suggested to cause increased dynamic knee valgus. Patients are often prescribed strengthening of these muscles with the expectation that it will reduce their risk of pain and injury. The validity of this claim remains unclear. METHOD: We conducted a systematic review and partial meta-analysis assessing the association between hip strength and knee valgus in healthy subjects. An online search was conducted in May 2026. Databases included Medline, EMBASE, CINAHL and Google Scholar. INCLUSION CRITERIA: English language, as

Can we rebuild what has been taken? A narrative review of biological and functional consequences of hamstring tendon harvest in ACL surgery.

This narrative review aimed to summarize the consequences of hamstring tendon (HT) harvest for anterior cruciate ligament reconstruction (ACLR), with reference to tendon regeneration, knee flexor morphology, clinical assessment of knee flexion strength, and rehabilitation strategies for optimizing recovery. The available evidence suggests that the harvested semitendinosus (ST) and gracilis (GR) tendons regenerate in ∼70-86% of cases after ACLR. Despite tendon regeneration, ACLR with HT autograft results in morphological alterations in both the ST and GR. These changes include tendon elongation

Regenerative Trifecta: A Multimodal Regenerative Approach to Soft Tissue Injury Recovery and Return to Sport.

Soft tissue injuries - affecting tendons, ligaments, and muscles - are the leading cause of missed games, lost seasons, and early career endings in competitive sports. Injury rates across major professional leagues have risen significantly over the past decade, with the knee, groin, hip, and thigh bearing the greatest burden. The financial impact is enormous at the high school and collegiate levels alone, while at the professional level, soft tissue failures have decided playoff outcomes and derailed championship-caliber teams. Standard treatments - rest, physical therapy, nonsteroidal anti-in

Complete Proximal Rectus Femoris Avulsion Repair in a Division I Collegiate Track Athlete: A Case Report.

CASE: A 20-year-old track athlete experienced a pop in her left hip with immediate pain and weakness during a race. She had a palpable defect distal to the anterior inferior iliac spine (AIIS). Magnetic resonance imaging demonstrated a complete tear of the rectus femoris direct head tendon from the AIIS, with 3 cm of retraction. Given her age and activity level, she underwent successful proximal rectus femoris avulsion repair and experienced a full return to sport. CONCLUSION: For athletes with an isolated avulsion of the direct head of the rectus femoris and significant tendon retraction, su

Knee and hip joint dynamics differ between sprinting and Nordic hamstring exercises.

Sprinting and the Nordic hamstring exercise (NHE) are common training modalities used to reduce hamstring injury risk, but the differences in the biomechanical demands of sprinting and the NHE are unclear. We conducted an exploratory analysis comparing knee and hip joint kinematics and kinetics, and biceps femoris long head (BFLH) muscle-tendon unit length and velocity during the flight phase of sprinting and the NHE. We collected motion capture and force data from fourteen athletic participants (8 males, 6 females) running (4-8 m/s) and performing the NHE. We used this experimental data and m

Augmentation of hip abductor repair with fascia of vastus lateralis.

BACKGROUND: Hip abductor tears are common and can cause severe limp and pain. Repair of gluteal tears often requires augmentation. Although current methods of gluteal augmentation have good reported outcomes, they can be costly and add an additional layer of complexity if they become infected because they rely on non-autologous materials. Based on the case series below, we report an alternative technique to augment gluteal tendon repair that uses vastus lateralis fascia. METHODS: Between 2022 and 2025, 23 hip abductor repairs augmented with a vastus lateralis fascia flap, following a primary

Direct Anterior Approach to Hip Resurfacing Arthroplasty.

BACKGROUND: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate preparation of the femoral head and acetabulum is critical for successful outcomes3. In the present video article, we describe a reproducible technique utilizing preoperative 3D computed tomography (CT)-based planning and patient-specific 3D-printed jigs to optimize femoral head p

Determining the incidence of abductor tears in total hip arthroplasty via direct lateral approach: a prospective analysis.

INTRODUCTION: Each surgical approach for primary THA has unique advantages. The direct lateral approach (DLA) offers increased visualisation of the gluteus medius and minimus tendons and, consequently, has been suggested to facilitate management of intraoperatively identified abductor tears. However, there remains a paucity of data exploring the frequency of abductor tears in patients undergoing primary THA via DLA. The purpose of this prospective study was to: (1) report the incidence of abductor tears in the DLA patients; and (2) identify risk factors for abductor tears in patients undergoin

Anatomy and Pathology of Anterior and Lateral Hip Compartments Bursae: An Ultrasound Structured Approach in Sports Medicine.

When assessing hip pain in athletes, it is important to focus on the extra-articular soft tissues that may clinically mimic joint pathology. One such extra-articular structure is the synovial bursa. Bursitis can clinically be misdiagnosed as joint-, tendon- or muscle-related pain. These pathological processes are a result of inflammation that is often secondary to acute trauma, overuse injuries, arthritis, or infection. Traumatic and overuse hip bursitis occur frequently in amateur and professional athletes and clinical presentation is often nonspecific. After clinical assessment, imaging play

Revision Total Hip Arthroplasty with Femoral Component Retention.

BACKGROUND: Revision total hip arthroplasty (THA) for isolated polyethylene exchange or acetabular revision with retention of the femoral component can present a challenge for adequate exposure. A systematic approach to a proper release can facilitate exposure and reduce the risk of iatrogenic complications. DESCRIPTION: The posterior approach is an extensile and versatile approach for revision THA. After incising the fascia and iliotibial band, the insertion of the gluteus maximus is fully released. After releasing any scar along the inferior gluteus medius, a retractor is placed to hold the

Thigh and Groin Overuse Injuries.

Overuse injuries affecting the hip, groin, and thigh represent a significant burden in both endurance and field sport athletes. This narrative review examines the anatomy, biomechanics, and imaging manifestations of the common overuse injuries in these regions, with particular emphasis on magnetic resonance imaging (MRI) findings and their clinical correlation. We systematize injuries within the Doha classification framework, discussing adductor-related pain, pubic-related pain, iliopsoas-related pain, and inguinal-related pain, along with thigh-based pathology including proximal hamstring ten

MRI assessment of muscle damage after posterolateral versus SuperPATH approach for total hip arthroplasty.

The minimally invasive SuperPATH approach for total hip arthroplasty (THA) is theorized to reduce soft tissue damage, but objective evidence comparing muscle injury to the conventional posterolateral approach (PLA) is lacking. This study aimed to objectively compare postoperative periacetabular muscle damage between the two approaches using magnetic resonance imaging (MRI). Hypothesis: The SuperPATH approach demonstrates superior efficacy in protecting posterior hip muscles compared to the PLA. In this retrospective comparative cohort study, patients undergoing primary unilateral THA for osteo

Magnetic resonance imaging features of sacrotuberous ligament partial injury with normal hamstring tendons.

The sacrotuberous ligament has an oblique course, from a broad fan-like origin in the posterior superior iliac spine, transverse tubercles of the sacrum and the upper part of the coccyx, descending laterally to attach to the medial surface of the ischial tuberosity. It provides stability to the pelvic girdle, including the sacroiliac joint, counterbalancing its rotation and limiting its movement. Magnetic resonance imaging (MRI) can identify this ligament, depicting its origin, course and continuity with the conjoined biceps femoris and semitendinosus tendon. However, there is no previous repo

Short foot exercises for flatfoot therapy: Status and prospects.

Flatfoot, a disorder defined by the flattening or loss of the medial longitudinal arch (MLA), affects approximately 15.6% of the child population. It may result from congenital or acquired dysfunction of muscles, tendons, or ligaments in the foot and ankle, as well as skeletal abnormalities. Clinically, this disorder manifests as foot pain, gait abnormalities, and compensatory injuries in lower limb joints (e.g., knee and hip), thereby affecting motor function and quality of life. Short foot exercises (SFE) as a noninvasive rehabilitation strategy enhances arch height and improves dynamic foot
