It has been postulated that the adaptations of lower extremity function exhibited by anterior cruciate ligament (ACL) deficient and post-ACL surgical patients represent early accommodations to the loss of ACL function after injury so that excessive anterior displacement of the tibia is prevented. Prior studies have suggested that compensation patterns in ACL deficient and post-ACL surgical subjects may affect joint moments of the knee as well as the hip. However, the variance in knee and hip forces between ACL deficient, post-surgical ACL and uninjured groups has not been clearly elucidated. T

OBJECTIVES: (1) To identify adaptations caused by intra-articular knee joint effusion during walking and (2) to determine if knee joint effusion may be a causative factor in promoting quadriceps avoidance gait patterns.
DESIGN: Gait testing of 14 healthy individuals who underwent incremental saline injections of the knee joint capsule.Background. Gait adaptations have been reported in the literature for knee injured and rehabilitating individuals. Knee joint capsular afferent activity can influence knee joint function.
METHODS: Gait analysis was employed in a pre- and post-test, repeated mea

The adult hip is a new and challenging site for evaluation with ultrasound (US). Clinically, diseases involving the hip region may be difficult to diagnose without the help of imaging. The hip region is a crossroad for numerous vascular, nervous, and muscular structures that pass between the trunk and the lower extremity. Thus, inflammatory processes and neoplasms may spread to and from the hip, buttock, thigh, pelvis, and retroperitoneum, and trauma may also effect the adjacent areas of the trunk and thigh. Because it is a crossroad, many conditions that are not specific to the hip occur in t

The first goal of this review is to describe a local plasmid gene transfer technology known as the gene activated matrix (GAM). GAM was the first gene therapy designed specifically for tissue engineering applications, and the mechanism of action of plasmid gene transfer is closely tied to the normal sequence of events associated with wound healing. The normal sequence of wound healing events is stereotyped for most tissues, and one assumption has been that GAM could serve as a platform technology for local gene delivery in various tissues and organs. This hypothesis essentially has been proved

A concern for individuals with hemiparesis affecting their gait, which heretofore has never been studied, is the possibility that various compensations occurring in the unaffected limb may strain or fatigue the muscles or ligaments and/or predispose to joint injury in that limb. We studied the biomechanics of the unaffected limb during walking in 20 subjects with hemiparesis who had stiff-legged gait as a result of stroke. An optoelectronic motion analysis and force platform system was used to estimate torques in all three planes about the hip, knee, and ankle. Sagittal plane joint motion and

When pain and effusion have been minimized and patients assume appropriate knee posture at rest and during exercise, the extent of postinjury or postoperative quadriceps femoris neuromuscular inhibition and avoidance during locomotion is reduced. Restoring normal lower-extremity ROM and musculotendinous extensibility (with consideration for biarticular muscles) is foundational to the implementation of an exercise program that integrates the trunk, hip, and ankle muscles into dynamic knee-stabilization challenges while addressing isolated quadriceps femoris deficiencies. Cardiovascular conditio

Three-month-old male rats were subjected 3 times weekly for 1 h to eccentric exercise of one triceps surae muscle (30 stimulations/min) under general anesthesia in order to induce Achilles tendon disorder corresponding to paratenonitis and tendinosis in man. Net muscle work during the sessions ranged between 0.67 and 4.37 mJ (mean 1.72, SD 0.77). After 9 and 13 sessions, respectively, 2 rats started to show gait alterations during the functional test which was performed 2-3 times weekly. These rats were killed after additional sessions which showed a worsening of the limp. The other trained ra

Sonography is currently being applied to many areas of the musculoskeletal system. Although some areas are in the experimental stage, there exist several indications in which sonography can produce results at least equal to what is possible with MR imaging. Examples include evaluation for shoulder and ankle tendon abnormalities; soft tissue infection and joint effusion; soft tissue foreign bodies; soft tissue masses, such as Baker's cyst and wrist ganglion cyst; carpal tunnel syndrome; developmental dysplasia of the hip; and other indications where MR imaging is contraindicated. The role of so

Heterotopic ossification is a well-recognized complication of spinal cord injury, closed head injury, total hip arthroplasty, burns, and other trauma and has been observed in various tissues such as muscles, tendons, ligaments, and menisci. Complications of arthroscopic acromioplasty are relatively uncommon and include hematoma, traction neuropathy, infection, acromial fracture, reflex sympathetic dystrophy, and instrument breakage. However, little has been reported on heterotopic ossification of the shoulder, particularly after arthroscopic surgery. Recurrent rotator cuff impingement symptoms

While MR imaging studies of the hip have been directed routinely toward the diagnosis of atraumatic osteonecrosis (ON), interest has increased in the use of MR imaging to diagnose other disorders of the hip and surrounding soft tissues, especially in athletic injuries. In addition to reviewing MR imaging technical considerations and image appearance, this article discusses applications of MR imaging to diagnosing pelvic and hip fractures, muscle contusion and strain, tendon injuries, acetabular labral tears, bursitis, and osteitis pubis and chronic symphyseal injury.

BACKGROUND: Posttraumatic piriformis syndrome is a rare disorder that is not clearly defined in the orthopaedic literature. We report on the specific diagnosis, operative treatment, and outcome of treatment of fifteen cases of piriformis syndrome (in fourteen patients), treated by one surgeon, in which the common etiology was blunt trauma to the buttock. We are unaware of any previously published report of this kind.
METHODS: Fourteen patients (fifteen cases of piriformis syndrome), with an average age of thirty-eight years (range, twenty-four to fifty-six years), were managed with an operati

The objective of this study was to compile knowledge of athletic injuries and complaint patterns related specifically to equestrian acrobatics (e.a.). A 20-page standardized questionnaire was sent to 114 e.a. participants. The mean age of the participants in e.a. groups was 15 +/- 3 years, that of independent participants 21 +/- 3 years. A total of 489 injuries was reported, mainly to muscles and tendons (35%), skin (33%) and joints and ligaments (25%). Bone injuries (6%) and head injuries (2%) were infrequent. Analysis of the localization showed that the head-face-neck region was involved in

Stiff-legged gait, characterized by limited knee flexion during the swing period, is a common consequence of upper motor neuron injury. The purpose of this investigation was to determine whether the rectus femoris and hamstrings muscles (which act at both the hip and knee) contribute to stiff-legged gait if active during the swing period of the gait cycle. Ten subjects with unilateral stiff-legged gait due to stroke were evaluated. Swing period free gait data were obtained. A biomechanical model of the affected limb was developed for each subject. Muscle and tendon lengths were scaled to indiv

Arthrogryposis multiplex congenita (AMC) is a rare disease with multiple joint contractures. It is widely believed that bilaterally dislocated hips should not be reduced since movement is satisfactory and open reduction has had poor results. Since 1977 we have performed a new method of open reduction using an extensive anterolateral approach on ten hips in five children with AMC. The mean age at surgery was 31.5 months (17 to 64) and the mean follow-up was 11.8 years (3.8 to 19.5). At the final follow-up all children walked without crutches or canes. Two managed independently, one required a l

This review examines the diagnosis and management of iliopsoas bursitis and/or tendinitis. It is a relatively uncommon and unrecognised cause of anterior hip pain and anterior snapping hip. In view of its pathology, iliopsoas bursitis might be better referred to as iliopsoas syndrome. It can usually be diagnosed by history and physical examination, though real time ultrasound may be useful in confirming the diagnosis. Magnetic resonance and computerised tomography imaging have limited roles in its diagnosis, but may identify other pathology or surgical lesions. Nonoperative management has not

The aim of this study is to analyse the changes in select gait parameters following anterior cruciate ligament (ACL) reconstruction. The study was performed on 15 subjects who underwent ACL reconstruction by the bone-patellar tendon-bone technique. Gait analysis was performed using the Elite three-dimensional (3D) optoelectronic system (BTS), a Kistler force platform and the Telemg telemetric electromyograph (BTS). Kinematic data were recorded for the principal lower limb joints (hip, knee and ankle). The examined muscles include vastus lateralis, rectus femoris, biceps femoris and semitendino
