[Unbalanced pelvic girdle in motor disability of cerebral origin. Success and limitations of adductor muscle tenotomy].

One hundred and fifty eight children with an unbalanced pelvis were reviewed. All suffered from cerebral palsy and were treated by tenotomy of the adductor muscles. The aims were: 1 - To improve the position of the femoral head. For this purpose, the best effects were obtained before the age of 4 so that acetabular dysplasia was prevented and the need for pelvic osteotomy, which is often hazardous in this type of patient, avoided. 2 - To achieve better balance of the pelvic girdle. Adductor tenotomy was often associated with tenotomy of the ilio-tibial tract or with division of the costo-iliac

Radiographic comparison of adductor procedures in cerebral palsied hips.

Hip muscle imbalance in cerebral palsy may cause subluxation or dislocation. Pelvic radiographs were used to assess hip stability in 40 affected hips before and after open adductor surgery. Preoperative rates of hip migration were determined from radiographs using the center-edge angle and migration percentage. Either an open adductor tenotomy with partial obturator neurectomy or a posterior adductor transfer was performed. Follow-up averaged 5 years, with a reversal of preoperative lateral hip migration in 36 of 40 hips. There was no statistically significant difference in the radiographic ou

Histochemical correlates of hamstring injuries.

This study reports the histochemical fiber type composition of the human hamstring muscles. Muscle specimens from necropsy specimens were obtained from seven locations in the hamstring, four locations in the quadriceps, and one location in the adductor magnus. The hamstring muscles are shown to have a relatively high proportion of Type II fibers. Type II fibers are more involved with exercise of higher intensity and force production and it is postulated that the hamstrings are capable of high intrinsic force production. The hamstrings are two-joint muscles and are, therefore, subject to increa

The dancer's hip.

Conditions that occur in the dancer's hip fall into the following categories: poor training; conditions that occur as the result of normal use; overuse syndromes, including tendinitis and myositis; and conditions referring pain to the hip. Dancers are highly motivated and goal oriented and often suppress symptoms for long periods, making diagnosis and treatment difficult. Observing the dancer at work and understanding his art are emphasized, and a practical guide to therapy is presented. Development of proper dance technique and a proper flexibility program can decrease the incidence of injuri

Pathogenesis and treatment of certain fatigue fractures of the neck of the femur.

During the unipedal phase of walking, it has been shown (Fischer, Fick & Pauwels) that the forces acting on the hip obey the fundamental laws of mechanics and can be calculated and graphically recorded fairly precisely. In these circumstances, the bones, muscles and tendons act reciprocally in such a way as to achieve the best possible motor unit, a complex machine designed to achieve the maximum work with the minimum material and the minimum expenditure of energy. This explains why, if the bones undergo changes in form or structure, complicated pathogenetic mechanisms are invoked which are in

Ballet injuries: the Australian experience.

There is a distinct difference between ballet injuries and sports injuries in general, and the sports medicine physician needs to study the technique of dance and the specific injuries that it may produce in order to treat dancers effectively. In Australia, which is typical of other countries where ballet is performed, ballet injuries include strained lumbar muscles, sprained ankle, Achilles tendinitis, clicking hip, jumper's knee, chondromalacia, stress fractures, patellar subluxation, and other knee and tendon problems.

"Snapping bottom": subluxation of the tendon of the long head of the biceps femoris muscle.

Subluxation of the tendon of origin of the long head of the biceps femoris muscle (or snapping bottom) is a rare, previously unreported degeneration of the tendinous attachments of the muscle at the ischial tuberosity. It occurs in a patient who wears out this attachment through a lifetime of hyperflexion hip movements, such as are required in bending to reach the floor. The snapping of the tendon across its point of origin causes pain and swelling in the buttock. In the older nonathlete, if conservative measures fail, the treatment of choice is simple tenotomy. The condition should not be con

Progress in the management of the paralytic hip in myelomeningocele.

1. The primary aim of management at the hip joint in the patient with myelomeningocele is the prevention and correction of deformity, usually a flexion deformity. 2. It is necessary to treat both flexion deformity and the causative muscle imbalance; correction of this imbalance will also prevent subsequent dislocation of the lip. 3. The psoas tendon is generally excised; transfer is reserved for a few children with excellent walking potential and a good prognosis from all viewpoints. 4. Flexion deformity is treated by a soft tissue release procedure (for children under the age of eight years a

[Spasm of the adductor muscles, pre-dislocation and dislocations of the hip joints in children and adolescents with cerebral palsy. Clinical observations on aetiology, pathogenesis, therapy and rehabilitation. Part I: The effect of open myotenotomy of the gracilis muscle and of the long and short adductor muscles in connection with total extrapelvine resection of the obturator nerve, on the hip joints and static function (author's transl)].

Spasm and contraction of the adductor muscles involve, on the one hand, danger in respect of the development of a dislocation of the hip, and are a serious impediment to a walking ability on the other. Hence, surgery is often necessary. The article reports on the results of consequent weakening of the adductor muscles as a result of open myotenotomy in association with complete extrapelvine resection of the obturator nerve. 27 patients were subjected to surgery--in most cases bilaterally--at an age between 2 years and 5 months and 18 years, with a follow-up period of up to 15 years. The study

[Spasm of the adductor muscles, pre-dislocations and dislocations of the hip joints in children and adolescents with cerebral palsy. Clinical observations on aetiology, pathogenesis, therapy and rehabilitation. Part II. The importance of the iliopsoas tendon, its tenotomy, of the coxa valga antetorta, and correction through osteotomy turning the hip into varus (author's transl)].

The following factors besides spasm and contraction of the adductor muscles contribute to the occurrence of dislocations of the hip in spastic paralysis: Spasm and contraction of the iliopsoas muscle and enhanced valgus position and antetorsion. The author holds the opinion that in case of malformation of the proximal end of the femur, it is not only the indirect action of the spastic musculature via the proximal femur-epiphyseal cartilage which is responsible for this phenomen in accordance with the law on functional adaption through longitudinal growth (Pauwels), but also the direct traction

Tendon transfers in the paralytic hip.

We analyzed the results of transfers of the iliopsoas or external oblique muscles performed to augment the abductor power of the hips in 149 patients with flaccid paralysis of the hips. More than 800 supplemental procedures were performed on the lower extremities of these patients. Twenty-three of the thirty-eight hips that had been subluxated or dislocated became stable in the reduced position after the transfer operation. Every hip that had been stable before the transfer remained stable, and thirty-seven patients who had been brace-dependent became brace-free. We concluded that transfers of

The adductor transfer in the high-risk hip in myelodysplasia: a preliminary report.

Hip instability occurs in meningomyelocele patients with lumbar level paralysis because of muscle imbalance. Transferring the hip adductor origins to the ischium can be used as the primary procedure to restore muscle balance and prevent hip instability in myelodysplasia patients if performed by the age of one year. Twelve lumbar meningomyelocele patients underwent bilateral adductor transfers. Secondary procedures, such as iliopsoas release or transfer or varus derotation osteotomy, were done in 14 hips. Ten hips needed no further surgery. Twenty of 24 hips were clinically and roentgenographic

Calcific tendinitis of the neck.

When a calcific deposit in the shoulder, hip or elbow ruptures, it results in severe pain and tenderness. Few clinicians are aware that the longus colli muscle of the neck is also subject to acute tendinitis. When it occurs, the pain and spasm mimic infectious spondylitis or meningitis. Four patients are presented with acute tendinitis of the longus colli muscle and the classic radiographic findings of soft-tissue swelling and amorphous calcium deposits in the tendon.

Paralytic dislocation of the hip in myelodysplasia. The role of the adductor transfer.

In thirty-nine patients with myelodysplasia at the third, fourth, or fifth lumbar level and subluxation or dislocation of the hip (fifty-nine hips involved), the following surgical approaches were utilized to balance the muscles about the hip: In one group (twenty hips in eleven patients) the origins of the adductors longus, brevis, and gracilis were transferred to the ischial tuberosity, and in fifteen hips in that group the iliopsoas also was transferred to the greater trochanter when the abductors were severely weakened. In the second group (thirty-nine hips in twenty-eight patients), only
