Anatomical relationship between the distal lateral femoral cutaneous nerve and the quadriceps tendon autograft harvest site: a cadaveric study with implications for acl reconstruction surgery.

PURPOSE: The lateral femoral cutaneous nerve (LFCN) is a sensory nerve providing cutaneous sensation to the anterolateral thigh. Injury to the LFCN is a known complication of quadriceps tendon (QT) autograft harvest during anterior cruciate ligament (ACL) reconstruction. While the proximal course of the LFCN is well described, its distal anatomy relative to the QT remains poorly defined. This study evaluated the distal course of the LFCN to identify zones of reduced iatrogenic injury during QT harvest. METHODS: Twenty-nine lower extremity specimens from 17 formalin-embalmed cadavers were diss

[Surgical treatment for chronic rupture of the quadriceps tendon].

OBJECTIVE: Reconstruction of the quadriceps tendon to restore extensor function in cases of chronic rupture. INDICATIONS: Rupture of the quadriceps tendon due to delayed diagnosis, failure of primary refixation or after implantation of a knee endoprosthesis. CONTRAINDICATIONS: Infections. SURGICAL TECHNIQUE: Reopen the old incision and verify whether the quadriceps tendon can be reattached to the patella. If there is no dehiscence, refixation with bone anchors (no bone defects) or transosseously (bone defects). If the tissue quality is poor (e.g., previous surgery, knee prosthesis), augment

Uncovering the State of Current Data on Quadriceps Tendon Autograft Use Versus Bone-Patellar Tendon-Bone and Hamstring Tendon Autografts in Anterior Cruciate Ligament Reconstruction at ≥5 Years After Surgery: A Systematic Review and Meta-analysis.

BACKGROUND: The use of quadriceps tendon (QT) autograft for anterior cruciate ligament (ACL) reconstruction has been increasing since 2014. Studies have shown that QT is comparable to hamstring tendon (HT) and bone-patellar tendon-bone (BTB) autografts in terms of outcomes, although QT autograft has lower rates of donor site morbidity. Systematic reviews and meta-analyses have been previously conducted on this topic, although none have focused solely on data of patients at least 5 years out from surgery. HYPOTHESIS/PURPOSE: The purpose of this meta-analysis was to demonstrate that QT may not

Kneeling tolerance when using quadriceps tendon autograft for anterior cruciate ligament reconstruction is superior to hamstring tendon autograft.

PURPOSE: To investigate kneeling tolerance in patients undergoing hamstring (HT) versus quadriceps (QT) anterior cruciate ligament reconstruction (ACLR) and investigate correlation with patient-reported outcome measures (PROMs). METHODS: After recruitment and randomisation, 112 patients (HT = 55; QT = 57) underwent ACLR. Patients were assessed at 6, 12 and 24 months using the Kneeling Tolerance Test, which evaluates patient-reported pain in a position of both 90 (KT90) and 110 (KT110) degrees of knee flexion. PROMs collected included the International Knee Documentation Committee (IKDC) quest

Regional Patellar Tendon Strain in the Short- and Long-term After ACL Reconstruction Using Bone-Patellar Tendon-Bone Autograft.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) often involves harvesting a bone-patellar tendon-bone (BPTB) autograft. How graft harvest affects tendon strain across the 3 distinct regions (medial, lateral, and central) is not known. PURPOSE: To (1) quantify strain in the 3 regions of the patellar tendon during 60% of maximum voluntary isometric contraction (MVIC) in 90° of knee flexion and (2) assess how effort level in 2 different knee joint angles (60° and 90°) impacts strain in the medial and lateral regions of the patellar tendon, in 2 cohorts of patients after ACLR using a

Comparison of Dynamic Postural Stability in Autografts Versus Allografts Following Anterior Cruciate Ligament Reconstruction With Remnant Preservation: A Prospective Study With 1-Year Follow-up.

BACKGROUND: Graft selection is an important part of preoperative planning for anterior cruciate ligament reconstruction (ACLR). In addition, ACLR with the remnant preservation technique has recently gained attention due to potential benefit in bone-tendon healing, graft revascularization, and proprioceptive nerve remodeling. However, the ideal graft choice remains controversial, and there is limited research comparing autograft and allograft in ACLR with remnant preservation. PURPOSE: To compare knee muscle strength, neuromuscular control, and patient-reported outcomes between hamstring (HS)

Patients With a Quadriceps Tendon Autograft Demonstrate Greater Asymmetry in Landing Kinetics Than Patients With a Bone-Patellar Tendon-Bone Autograft 6 Months After Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Asymmetric landing kinetics 6 months after anterior cruciate ligament reconstruction (ACLR) are associated with higher risk of second anterior cruciate ligament injury. Little is known about landing kinetics after ACLR with an all-soft tissue quadriceps tendon (QT) autograft despite its increasingly common use in young, active patients. PURPOSE/HYPOTHESIS: The purpose of this study was to compare landing kinetics during a bilateral drop vertical jump (DVJ) 6 months after ACLR in participants who had undergone primary ACLR with a QT or bone-patellar tendon-bone (BTB) autograft. Pulmonary embolism following bilateral quadriceps tendon repair with unilateral tourniquet use: A case report.

CASE: An active healthy 68-year-old male sustained a bilateral quadriceps tendon rupture while running. He underwent a simultaneous bilateral quadriceps tendon repair in a dual-surgeon approach. The right quadriceps tendon was repaired with a tourniquet, while the left quadriceps tendon tear was repaired without one. Postoperatively, the patient developed deep vein thrombosis in the right leg, which led to a bilateral pulmonary embolism. This case illustrates a potential complication of tourniquet use in patients undergoing quadriceps tendon repairs. CONCLUSION: The use of tourniquets in orth

Torque visuomotor feedback training versus standard eccentric exercise for the management of patellar tendinopathy: protocol for a randomised controlled trial.

INTRODUCTION: Patellar tendinopathy (PT) rehabilitation programmes frequently incorporate eccentric exercise (EE), where participants have limited control over the range of motion, speed, force production and load. Newly developed training protocols that employ visual feedback with specialised devices offer controlled management of loads, speeds and forces across the full range of motion, potentially providing greater benefits. OBJECTIVE: This protocol outlines a randomised controlled trial designed to compare the effects of a visuomotor torque feedback (TF) protocol with a traditional EE pro

A comparative analysis of autograft choices of anterior cruciate ligament reconstruction and their effects on muscle strength and joint biomechanics.

INTRODUCTION: Anterior cruciate ligament reconstruction (ACLR) is crucial to restore knee stability and function after ACL injuries, especially in physically active individuals. Despite advances in surgical techniques and rehabilitation protocols, the choice of autograft has a significant impact on postoperative recovery, particularly on muscle strength and joint biomechanics. In this study, the effects of four autografts are investigated: Iliotibial band (ITB), combined ITB and hamstring tendon (ITB + HT), hamstring tendon (HT) and bone-tendon-bone (BTB) on quadriceps and hamstring peak torqu

Quadriceps and hamstring muscles strength differences in adolescent and adult recreational athletes 6 months after autograft bone-patellar-tendon-bone anterior cruciate ligament reconstruction: A retrospective study.

BACKGROUND: Knee muscle strength recovery after anterior cruciate ligament reconstruction (ACLR) is crucial for a safe return to sport (RTS) but it is poorly described in the adolescent population. Therefore, we compared the knee muscle strength at 6 months post-surgery in adults and adolescents. We hypothesized a greater muscular strength in adolescents 6 months after ACLR. METHODS: This was a retrospective analysis of 55 adolescents (13-18 years old) and 76 adults (19-39 years old) who underwent ACLR with autograft bone-patellar-tendon-bone (BPTB), subjected to isokinetic tests 6 months aft

[Left-sided quadriceps tendon rupture and right-sided Achilles tendon rupture in a statin-treated patient].

A 65-year-old man, treated with statins, experienced simultaneous ruptures of the left quadriceps and right Achilles tendon during simple gardening, without any known risk factors for tendon rupture. Statins are implicated in tendon fragility, raising concerns about their role in such rare bilateral ruptures. This case report prompts the question of whether statin therapy should be reassessed, particularly in patients presenting with tendon-related symptoms.
