Hip disarticulation - case series analysis and literature review.

OBJECTIVE: To present a retrospective study of 16 patients submitted to hip disarticulation. METHODS: During the period of 16 years, 16 patients who underwent hip disarticulation were identified. All of them were studied based on clinical records regarding the gender, age at surgery, disarticulation cause, postoperative complications, mortality rates and functional status after hip disarticulation. RESULTS: Hip disarticulation was performed electively in most cases and urgently in only three cases. The indications had the following origins: infection (n = 6), tumor (n = 6), trauma (n = 3), a

Creative prosthetic foot selection enables successful ambulation in stiletto high heels.

BACKGROUND: Walking in high heels presents biomechanical challenges, yet they remain part of many women's attire. However, women with a lower limb amputation are limited in available footwear options. Case description and methods: This case study is in response to one patient's assertion that she walked better and more symmetrically in heels than flat shoes with her below-knee prosthesis. She underwent gait analysis in athletic shoes and 10-cm stiletto high heels worn with a pediatric running foot to determine if these claims could be substantiated through biomechanical measures. Global gait a

Neck of Femur Fracture in a Patient with a Chronic Osteomyelitis of the Ipsilateral Foot.

This case report describes a successful two-stage treatment in a 75-year-old male with a displaced neck of femur fracture, also suffering from an active chronic osteomyelitis of the ipsilateral calcaneus. In our case, a below-knee amputation was performed first, followed by total hip arthroplasty two weeks later. At 15-month follow-up, full recovery of the prefracture level of activities of daily living without significant impairment was obtained. Only a few cases of total hip arthroplasty in amputees have been published, but the indication for surgery was mainly traumatic or advanced osteoart

Epidemiology of Total Ankle Arthroplasty: Trends in New York State.

The rate of total ankle arthroplasty (TAA) is increasing in the United States as its popularity and indications expand. There currently is no national joint registry available to monitor outcomes, and few studies have addressed the challenges faced with TAA. The purpose of this study was to evaluate the incidence, complications, and survival rates associated with TAA using a large statewide administrative discharge database. Individuals who underwent primary TAA from 1997 to 2010 were identified in the Statewide Planning and Research Cooperative System database from the New York State Departme

Does intact limb loading differ in servicemembers with traumatic lower limb loss?

BACKGROUND: The initiation and progression of knee and hip arthritis have been related to limb loading during ambulation. Although altered gait mechanics with unilateral lower limb loss often result in larger and more prolonged forces through the intact limb, how these forces differ with traumatic limb loss and duration of ambulation have not been well described. QUESTIONS/PURPOSES: The purpose of this study was to determine whether biomechanical variables of joint and limb loading (external adduction moments, vertical ground reaction force loading rates, and impulses) are larger in the intac

Total hip arthroplasty following an ipsilateral above knee amputation.

Total hip arthroplasty in patients with previous above knee amputations is rare. We present a unique case where the patient had only 130 mm of proximal femur remaining following a previous traumatic above knee amputation. The short segment of femur meaning a conventional femoral stem could not be used. We describe the technique of total hip arthoplasty for this patient using a mini hip prosthesis and report a successful clinical and radiological outcome at 2 years post-op.

Kinematic comparison of walking on uneven ground using powered and unpowered prostheses.

BACKGROUND: Recent research has focused on the design of intelligent prosthetic ankle devices with the goal of adapting behavior of the device to accommodate all walking surfaces that an individual encounters in daily life. To date, no studies have looked at how such devices perform on uneven terrain. METHODS: 11 young adults with unilateral transtibial amputation participated in two data collection sessions spaced approximately 3 weeks apart. In each session they walked across a loose rock surface at three controlled speeds. In the first session, they wore a passive, energy storage and retur

Changes to level ground transtibial amputee gait with a weighted backpack.

BACKGROUND: Many occupations and hobbies require the use of a weighted pack. To date there has been limited backpack gait studies performed on the amputee population. It is important that we address this knowledge gap in order to further improve individual's quality of living through changes in rehabilitation, and prosthesis development. METHODS: The study population was ten male, unilateral, traumatic, K4-level (ability for prosthetic ambulation with high impact, stress, or energy levels), transtibial amputees. Ten walking trials were collected on level ground; five with a 24.5 kg backpack a

Gait characteristics of individuals with transtibial amputations walking on a destabilizing rock surface.

Individuals with transtibial amputation (TTA) have a high incidence of falls during walking. Environmental factors, such as uneven ground, often play a contributing role in these falls. The purpose of this study was to quantify the adaptations TTA made when walking on a destabilizing loose rock surface. In this study, 13 young TTA walked over a rock surface and level ground at four controlled speeds. Subjects successfully traversed the rock surface by adopting a conservative gait characterized by shorter and wider steps. They also took shorter steps with their prosthetic limbs and exhibited gr

Gait characteristics of a soldier with a traumatic hip disarticulation.

BACKGROUND AND PURPOSE: No reports have analyzed the temporal-spatial, kinematic, or kinetic components of gait coupled with a metabolic analysis of patients with hip disarticulations. Most of the research on this population is based on older adults. As a result, guidelines for reasonable functional outcomes for patients with hip disarticulations who are young, premorbidly fit, and goal oriented are lacking. This report describes quantitative measures of energy cost and gait characteristics of a young soldier with a unilateral traumatic hip disarticulation. CASE DESCRIPTION: One patient, a so

Traumatic bilateral knee dislocations, unilateral hip dislocation, and contralateral humeral amputation: a case report.

Bilateral traumatic knee dislocations are a rarity. We report a case of bilateral traumatic knee dislocations with concomitant right hip dislocation and complete traumatic amputation of the left, nondominant upper extremity at the level of the proximal one-third of the humerus. Angiograms revealed no evidence of popliteal artery injury. Orthopedic treatment consisted of immediate reduction of the dislocations and urgent revision amputation of the upper extremity. Staged, bilateral knee ligamentous reconstructions were performed on hospital days 24 and 29, respectively. Despite this constellati

Lower extremity amputation: a contemporary series.

We sought to identify the results achieved with lower extremity amputations performed by both community and university-based surgeons as well as from multiple disciplines (orthopedic/general/vascular surgeons) serving a predominantly nonurban population. A review of 411 consecutive patients undergoing 508 non-traumatic lower extremity amputations at Charleston Area Medical Center from January 1999 to December 2003 was conducted. Amputations were performed most frequently at the below knee level (50.9%). Perioperative mortality (30-day) for the cohort was 11%. Mortality increased with more prox

Rehabilitation outcomes in the older adult.

Numerous studies support the use of rehabilitative interventions in the older adult. Given the many fiscal challenges in health care today, it is of utmost importance that funding for rehabilitation result in fruitful outcomes. Specific rehabilitative interventions have been found to be very effective in the elderly and can be demonstrated through numerous studies. Outcomes for conditions including stroke, traumatic brain injury, spinal cord injury, amputation, hip fracture, and joint replacement are discussed. There is great need for ongoing research to determine optimal rehabilitative interv

Validity of an alternative anthropometric trait as cardiovascular diseases risk factor: example from individuals with traumatic lower extremity amputation.

BACKGROUND: Published studies reveal that individuals with lower extremity amputation are vulnerable to cardiovascular diseases (CVD) because of poor physical activity level. Many cardiovascular risk assessment studies have utilized anthropometric traits (primarily body mass index and waist circumference) as cardiovascular risk factor. However, some studies emphasized the technical limitations of measuring waist circumference for studying cardiovascular risk, and so it is difficult to obtain correct measurement from the individuals with lower extremity amputation. OBJECTIVES: The objectives o

Amputation through the hip joint during the pre-anaesthetic era.

Even to the present day, hip disarticulation is one of the most radical orthopaedic operations. First carried out on the Continent in 1774 and in Britain in 1779, this procedure was only deemed necessary in circumstances where death was otherwise inevitable. George James Guthrie claimed to have undertaken the first amputation through the hip joint in which the patient survived. We describe some of these cases from the pre-anaesthetic surgical literature in which amputation through the hip joint was undertaken, and we comment on the surgical techniques used. This information is complemented by

Lower limb landmine injuries.

The medical records of 186 patients seeking treatment for landmine injuries in the authors' region between 1993 and 2001 were evaluated. Of these patients 13 died of accompanying complications. Ten (10) patients with general body trauma and upper limb trauma were excluded from the study. Of 163 patients with lower-limb injuries included in the study, 21 with traumatic amputation underwent surgical amputation at different levels. Patients without traumatic amputation were divided into 2 groups. There were 41 patients (29%) in Group I who were treated by limb salvage procedures. Treatments used
