The treatment of congenital dislocation of the knee with the Pavlik harness.

We have used the Pavlik harness in the treatment of congenital dislocation of the knee in six knees in five patients, with satisfactory results. We believe that the muscles in the thigh which cross both the hip and knee joints, the rectus femoris and hamstrings, play an important role in the spontaneous reduction of the joint. Full correction is not always obtained, our results are comparable with those described by other authors. We consider that the advantages of using the harness are that a spontaneous correction can be obtained, maximal correction can be obtained after three months of trea

[Treatment of femoral neck fractures using a total hip prosthesis].

From October 1. 1977 to December 1. 1982 106 patients suffering from fractures of the femoral neck (n = 75) or necroses of the femoral head and nonunions of the neck fracture respectively (n = 33) underwent total hip replacement by TEP (type Müller-Charnley). The results obtained were much better in comparison to other methods of treatment: less complications, shortening of in-hospital stay (average 21.1 days) and satisfactory till good late results in 82.8%. The mortality rate came up to 13.9% (fractures: 20.0%, necrosis of the head and nonunion: 0.0%). Concerning complications we observed wo

Traumatic rupture of iliacus muscle with femoral nerve paralysis.

Two cases of traumatic rupture of the iliacus muscle are reported. The clinical picture of this condition is characterized by pain in the groin, a tender mass in the iliac fossa, flexion contracture of the hip, and a femoral nerve palsy. Presence of a bleeding tendency should be investigated. Operative treatment is recommended for large hematomas.

Dislocation following total hip arthroplasty. Management by special brace in selected patients.

Dislocation after total hip arthroplasty (THA) occurs in 1%-3% of the cases reported in the literature. Recurrent dislocations due to malpositioned components should be treated by revision arthroplasty. Dislocated hips that have optimally positioned components but weak abductors can be treated by a special brace that prevents extremes of flexion, adduction, and rotation. Two hundred and eighty-nine total hip arthroplasties were performed from 1975 to 1981. Nine patients, all of whom had optimal position of components and the brace treatment, had dislocations. In one patient who had a spastic m

The Girdlestone procedure in spinal cord injured patients: a ten year experience.

The Girdlestone procedure which originally was developed for the treatment of tuberculosis of the hip has found a place in the management of septic hip arthritis and osteomyelitis secondary to pressure sores in spinal cord injury and other myelopathies. Eradication of the septic focus is necessary if amyloid disease is to be prevented. Early diagnosis and aggressive surgery are essential. Surgical treatment entails not only bone removal, but also a thorough joint debridement. This procedure goes hand-in-hand with the appropriate antibiotic treatment given intravenously for an adequate time (4-

Surface imaging from computerized tomographic scans.

A series of computer programs were created to display digital data obtained by the computerized tomographic (CT) scanner in a format readily comprehensible to the nonradiologist physician. The software requires no hardware beyond the CT scanner itself, is efficient in computational time and storage, and requires no operator intervention. The program manipulates standard CT axial data into images that simulate photographs of skin, muscle, or bone surfaces from a variety of perspectives. While the images themselves are planar, the surfaces they display are three dimensional. Portions of the imag

Traumatic lipomas/pseudolipomas.

The occurrence of lipomas following trauma is reported in three cases with a review of the literature including 36 additional cases. This condition is usually related to massive trauma in the hip, buttock, and thigh area that is most frequently secondary to a motor vehicle accident. It is most common in the female and may be attributed to the characteristic hip, buttock, thigh fat deposition of the female. The "tumor" usually follows heavy bruising and/or hematoma of the region and is clinically noticeable approximately 6 months thereafter. Initial treatment is preventive by drainage of the he

Psoas weakness and femoral neuropathy: neglected signs of retroperitoneal hemorrhage from ruptured aneurysm.

Femoral neuropathy may occur with aortic aneurysm more commonly than reports in the literature indicate. The combination of an aortic aneurysm and femoral neuropathy indicates rupture. The presence of abdominal pain and neuropathy should suggest ruptured aneurysm and exclude other commonly considered diagnoses. Preoperative recognition of femoral neuropathy provides the opportunity for intraoperative nerve decompression and assures the surgeon that the operation itself was not causative. The literature is reviewed, and the preoperative occurrence of femoral neuropathy in two patients with rupt

MAS trousers improperly applied causing a compartment syndrome in lower-extremity trauma.

A case is presented in which the leg segments of MAS trousers were deflated without deflating the abdominal segment, causing the trousers to act as a venous tourniquet. As a result bleeding and edema secondary to a pertrochanteric hip fracture were aggravated, causing a compartment syndrome in the thigh; fasciotomy became necessary. MAS trousers must be inflated in proper sequence, the leg segments first and then the abdominal portion. In deflation, the order is reversed. Only if this sequence is followed can MAS trousers be used safely.

Cast-bracing for fractures of the femoral shaft. A biomechanical and clinical study.

A study was made of the mechanics of blood-bearing in a series of patients treated with a cast-brace for fracture of the distal femur. Knee hinges incorporating strain-gauges, a simple force-plate on the floor and a standardised weight-bearing test were used to record axial loads through the cast-brace itself and through the fracture during the phases of healing. The cast-brace carried loads of only 10 to 20 per cent of body weight and functioned mainly as an antibuckling hinged tube. Patterns of weight-bearing recovery showed that the fracture itself limited loads to safe levels. A measure of

Treatment of proximal-third femur fractures with pins and thigh plaster.

Thirty patients with proximal third femur fractures were treated with pins-in-thigh plaster. Patients were divided into two groups: (1) acute (pins-in-plaster applied within one month after injury); and (2) late (pins-in-plaster applied later than one month ater injury). Patients in the acute group had application of pins-in-plaster inserted at an average of nine days after injury, and were treated in plaster for 9.4 weeks with union occurring at 10.7 weeks. Patients in the late group had application of pins-in-plaster an average of 13.3 weeks after injury, treatment in plaster for 7.7 weeks,

The vastus lateralis muscle flap: technique and applications.

The vastus lateralis muscle can provide a broad sheet of muscle for repair of defects of the lower abdomen, groin, perineum, hip, and ischium. The authors limit the use of the vastus lateralis muscle to situations in which more traditional techniques are not feasible. This muscle is especially suitable for defects involving irradiated tissue, infected bone, or infected prosthetic material. Although the vastus lateralis muscle has no cutaneous component, it may be used to augment an overlying tensor fascia lata flap.

Fitting of protective football equipment.

As no other guide was found to illustrate and describe protective equipment and its fitting, we have prepared this treatise. Properly fitted protective equipment for football is a must to prevent injury from occurring. Many injuries result from improperly fitted equipment. The helmet and shoulder pads are the most important pieces of equipment that require proper fit. Adequate face masks and jaw pads are essential to the helmet. Cantilever shoulder pads are necessary for most positions, with straps for those with subluxating or dislocating shoulders. Other pieces of equipment that need attenti

Hip hinge thigh brace for early mobilization of proximal femoral shaft fractures.

A new design, the 'Hip Hinge Thigh Brace' is presented for the safe early functional mobilization of patients with proximal half femoral shaft fractures. The mechanics, prescription, application technique and early clinical experience are reported. Use of the newer Crystona plastic plaster is shown to provide a more functional brace than the plaster of Paris casts.

"Snapping bottom": subluxation of the tendon of the long head of the biceps femoris muscle.

Subluxation of the tendon of origin of the long head of the biceps femoris muscle (or snapping bottom) is a rare, previously unreported degeneration of the tendinous attachments of the muscle at the ischial tuberosity. It occurs in a patient who wears out this attachment through a lifetime of hyperflexion hip movements, such as are required in bending to reach the floor. The snapping of the tendon across its point of origin causes pain and swelling in the buttock. In the older nonathlete, if conservative measures fail, the treatment of choice is simple tenotomy. The condition should not be con
