Coordinated mosquito biting dynamics at the human skin interface.

The bite of a female mosquito is the critical point of pathogen transmission, yet the fine-scale biting dynamics on human skin remain unknown. Here, we combine high-speed videography, deep-learning-based tracking, and electrophysiology to analyze biting dynamics of the malaria vector Anopheles coluzzii in real time at the human skin interface. We uncover a previously uncharacterized synchronization of mouthpart and leg movements during probing and identify five distinct below skin sub-behaviors: piercing, vessel searching, blood ingestion, retraction, and withdrawal. We show that disruption of

Forensic reconstruction of cause of death in a dismembered and partially burnt body: A case report.

BACKGROUND: Dismemberment and postmortem burning are uncommon forensic challenges that may obscure identification, injury interpretation and determination of cause of death. Systematic anatomical reconstruction and careful examination of dismemberment wounds, vital reaction of tissues and thermal alteration are essential for forensic reconstruction. Toxicological findings must be interpreted according to the substances and analytical categories actually examined. CASE PRESENTATION: Dismembered remains of an adult male were recovered from multiple locations and anatomically reassembled for med

Clinical outcomes and patient satisfaction following implant removal after open wedge high tibial osteotomy.

PURPOSE: Symptoms of implant-associated irritation have been reported as a complication of open wedge high tibial osteotomy (OWHTO). While some centers perform implant removal only when such complications occur, others routinely perform implant removal. However, the benefits of implant removal after OWHTO have not yet been clarified. The purpose of this study was to evaluate patient satisfaction with implant removal after OWHTO and to investigate the benefits of implant removal. METHODS: We prospectively enrolled 40 consecutive patients who underwent implant removal after OWHTO between August

Medial Hemiplateau High-Tibial Osteotomy Technique for Varus Tibial Plateau Malunion.

Tibial plateau fractures can be devastating injuries and lead to progressive pain, post-traumatic arthritis, and joint instability. Although these are typically treated with open reduction internal fixation at the time of injury, occasionally persistent deformity and/or malunion can occur. In the older patient, knee arthroplasty can be an effective treatment, but in the young patient, joint preservation approaches should be attempted to allow return to desired levels of activities. The purpose of this technical note is to describe the preoperative planning and surgical technique in the treatme

Occult Incarcerated Osteochondral Fracture of the Medial Patellar Facet Following Acute Lateral Patellar Dislocation in an Adolescent: A Case Report.

Acute lateral patellar dislocation is a common injury in adolescents and is most frequently associated with rupture of the medial patellofemoral ligament (MPFL). Concomitant osteochondral fractures may remain occult or be misinterpreted on initial imaging, resulting in delayed diagnosis and potentially compromising long-term joint function. We present a rare case of an incarcerated osteochondral fracture of the medial patellar facet that was initially misinterpreted as arising from the medial femoral condyle following acute lateral patellar dislocation. A previously healthy 15-year-old boy pre

Management of Advanced Tricompartmental Knee Osteoarthritis With Degenerative Meniscal Pathology Using Extracorporeal Magnetotransduction Therapy, Extracorporeal Shockwave Therapy, Microfragmented Adipose Tissue, and Platelet-Rich Plasma.

Advanced tricompartmental knee osteoarthritis (OA) with degenerative meniscal tears and ligamentous insufficiency represents a considerably difficult presentation in non-surgical musculoskeletal medicine. Although total knee arthroplasty is the current standard for surgical intervention, many patients may prefer to first explore non-surgical options or may require interim symptom management prior to surgery. This report describes management of a complex case of advanced knee OA via a staged non-surgical protocol utilizing extracorporeal shockwave therapy (ESWT), extracorporeal magnetotransduct

Biomechanical and finite element analysis of a novel annular multi-axial locking plate for patellar fracture fixation.

BACKGROUND: Traditional tension band structures for patellar fractures have multiple complications such as wires lossening and soft tissue irritation. In response to the above problems, a new type of annular multi-axis locking plate has been designed for the treatment of patellar fractures. This study aims to compare the biomechanical performance of two methods for stabilizing patellar fractures. METHODS: A three-dimensional finite element model was established to evaluate the biomechanical performance of annular multi-axis locking steel plate fixation for patellar fractures at knee flexion a

Diffuse stimulant-associated vasculopathy presenting with acute limb ischemia, compartment syndrome, and multifocal visceral infarction.

Stimulant-induced vasculopathy typically presents as localized ischemia secondary to vasospasm and thrombosis. Diffuse involvement of multiple vascular territories is uncommon and may result in severe, multisystem complications and increased morbidity. We report a 40-year-old female with polysubstance exposure, including cocaine and amphetamines, who presented with acute right lower extremity ischemia and compartment syndrome. Computed tomographic angiography demonstrated distal tibial, peroneal, and posterior tibial arterial occlusion. The patient underwent emergent four-compartment fasciotom

Successful Use of Peripheral Nerve Blocks for Emergent Trauma Surgery in a Patient With Perioperative Myocardial Infarction: A Case Report.

Patients with concurrent trauma-related surgical emergencies and ST-segment elevation myocardial infarction (MI) present a unique anesthetic challenge, as damage control surgery and rapid resuscitation must take priority over timely coronary revascularization. During these surgeries, the physiologic insult from general anesthesia can be detrimental to a failing heart, so regional anesthesia techniques and their implications in trauma should be considered. We present a 62-year-old male motorcyclist with hypertension, diabetes, and coronary artery disease and a history of percutaneous coronary i

History and Evolution of Lower Extremity Salvage.

There has been a paradigm shift in the management of lower extremity injury from amputations to more complex limb salvage techniques. Despite advancements in orthopedic surgery, trauma surgery, plastic surgery, and vascular surgery, not every patient is a candidate for complex lower extremity limb salvage. For this reason, novel advancements in plastic and reconstructive surgery have targeted not only limb salvage but also functional amputations. Here we review the history and advancements in microsurgical limb salvage and lower extremity amputation to discern how today's surgeon can better ai

Use of Biodegradable Temporizing Matrix for Donor and Recipient Site Wound Healing in Delayed Reverse Sural Flap: A Case Report.

INTRODUCTION: Reconstruction of distal third leg soft tissue defects is challenging, particularly in patients with multiple co-morbidities. Whilst the reverse sural artery (RSA) flap remains a viable reconstructive option, flap failure due to torsion, congestion, and necrosis persists. CASE PRESENTATION: This case report presents a patient who underwent a staged RSA flap surgery, with initial application of a biodegradable temporising matrix (BTM) over both the donor site and the heel defect. This was performed with three aims: to create a physical interface as an adjunct to surgical delay; t

Calf Injuries: Anatomy, Pathology, and Treatment.

Calf injuries are common in athletes and typically result in missed time from sport and activities of daily living. The three individual triceps surae muscles can be injured, with differing described mechanisms. Gastrocnemius or plantaris strains tend to occur during sudden explosive movement of the ankle from a dorsiflexed to a plantarflexed position with the knee in extension. Soleus strains typically occur from repetitive dorsiflexion of the ankle with the knee flexed. Imaging modalities such as magnetic resonance imaging and ultrasonography can be helpful in determining the location and gr

Characterization of lower-extremity biomechanics when walking with a hands-free crutch: an integrated experimental and computational analysis.

BACKGROUND: Lower-leg wounds and fractures are some of the most common types of traumatic injuries sustained by U.S. Service members during combat operations. Existing guidelines for on-field management of lower-leg injuries recommend stabilizing the injury and preventing weight-bearing on the affected leg. However, adhering to these recommendations could be challenging because the casualty may need to walk independently to obtain medical care. In this exploratory experimental and computational study, we assessed the biomechanical characteristics of a hands-free crutch device to aid in the mob
