Ballistic limb trauma: lessons from a decade of french civilian and military experience.

BACKGROUND: Ballistic limb injuries are increasingly encountered in both civilian and military settings, yet robust collaborative cohorts remain limited. METHODS: We conducted a retrospective, multicenter study across six French trauma centers (three civilian, three military) from 2014 to 2022, including consecutive patients aged ≥ 16 years with ballistic limb injuries. Data collected included demographics, context (civilian vs. combat/terror), projectile type and velocity, lesion characteristics, and early management. The primary outcome was early surgical site infection (SSI, < 30 days). Sec

Molecular Lung Imaging Following Exposure to Radiation Predicts Long-Term Survival in Rats.

Delayed effects of acute radiation exposure (DEARE), including radiation pneumonitis (lung-DEARE), develop weeks to months after radiation exposure. Pathway-targeted biomarkers that capture early oxidative stress and cell death could improve risk stratification and provide objective measures of mitigator efficacy. The objective was to test whether molecular lung imaging predicts long-term survival and mitigator response after irradiation. Rats received 13.5 Gy leg-out partial-body irradiation with a subset treated with the radiation-injury mitigator lisinopril. Rats underwent lung imaging at w

Fatal head injury by crossbow and ability to act: A case report and literature review.

Historically, the crossbow was a war weapon fired from a distance. It was superseded by guns and nowadays maybe used for sport and recreation. They can cause fatal injuries though such incidents are fortunately rare. Accordingly, forensic pathologists may face challenges when they are asked to determine the manner of death when a crossbow is involved. We present the case of a young man who was found dead in a fenced field. It was summertime and he was in an advanced state of putrefaction. The examiner observed a roundish cutaneous discontinuity on the forehead, from which a crossbow bolt protr

Assessing head injury risk and neuroprotective effect of ketone monoester supplementation in military airborne training.

Warfighters face significant risk of mild traumatic brain injury (mTBI) during operations and training. We performed a double-blind, randomized, placebo-controlled trial to: (1) determine head strike (HS) incidence, during US Army Basic Airborne Course's Improved Swing Landing Trainer (ISLT) training, (2) test protective efficacy of ketone monoester (KME) supplementation, and (3) assess blood-based mTBI biomarker signatures. We enrolled 354 active-duty male and female service members randomized 1:1 to placebo (PLA) or KME; n = 318 completed ISLT and assessments (PLA = 157, 142 M/15F; KME = 161

Optimization of the Missouri Osteochondral Preservation System for the storage of peripheral nerve allografts.

Peripheral nerve reconstruction for large-gap injuries remains limited by the shortcomings of autografts and decellularized allografts, which rarely achieve full recovery and carry donor site morbidity. This study investigated whether the Missouri Osteochondral Preservation System (MOPS) could be optimized for peripheral nerve allograft preservation by supplementing it with methylcobalamin (MOPS-N-B12) or glial-derived neurotrophic factor (MOPS-N-GDNF). Fifty rat sciatic nerves were harvested, stored in one of three solutions (MOPS, MOPS-N-B12, MOPS-N-GDNF), and analyzed after 30, 60, or 90 da

Polygenic Collagen Variants and Their Association With Ligament Injury Patterns in Patients With Knee Ligament Injuries.

Knee ligament injuries, particularly anterior cruciate ligament (ACL) tears, represent a major cause of functional limitation in physically active individuals. While biomechanical and environmental factors are well-established contributors, increasing evidence suggests that genetic variability in collagen-related genes may influence individual susceptibility to ligament injury. Most previous studies, however, have focused on elite athletic populations, limiting their applicability to routine clinical settings. A prospective cross-sectional study was conducted in 94 adult patients with clinical

National rates of pediatric extremity fractures over a 20-year timespan in Denmark: a population-based descriptive cohort study.

BACKGROUND AND PURPOSE: Previous reports on incidences rates of Scandinavian pediatric extremity fractures have varied, as they are often anatomically specific and based on institution-specific findings. To gain knowledge of current and future burden on the healthcare system, a national cohort assessment is necessary. We aimed to assess the proportion and incidence within anatomical distributions of pediatric extremity fractures in relation to age, sex, and time trends. METHODS: We retrieved a 20-year population-based cohort from the Danish National Patient Registry 1999-2018. We included all

Having a Revision ACL Reconstruction Is Worse Than Tearing the Contralateral ACL and Undergoing Reconstruction: A MOON Cohort Study.

BACKGROUND: Anterior cruciate ligament (ACL) graft tears and contralateral ACL tears are both relatively common after primary ACL reconstruction (ACLR). There is little prior work comparing the outcomes of reconstruction after these injuries. HYPOTHESIS: The authors hypothesize that patient-reported outcome measures (PROMs) and activity level are lower after revision ACLR than after primary contralateral ACLR. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: From a cohort of 2333 patients who underwent primary unilateral ACLR, 267 were identified who underwent subsequent revision

Changes in Biomechanical Risk Factors for Injury Across Normalized Running Speeds in Healthy Collegiate Cross-Country Runners.

Cross-country runners' risk for injury is high in-season potentially due to changes in biomechanics during prescribed fast running speeds and competitions. The purpose of this study was to determine if biomechanics associated with overuse knee and lower leg injury during running change similarly across speeds between female and male collegiate cross-country runners. Sixteen collegiate cross-country runners (8 females) participated. A self-selected running speed was entered into regression equations to determine treadmill speeds for the conditions: recovery (∼9% slower), threshold (∼25% faster)

Surgical approaches, choice and positioning of implants in the surgical treatment of proximal tibia fractures in adults: a scoping review protocol.

INTRODUCTION: Tibial plateau fractures present a complex orthopaedic challenge and usually require surgical intervention to restore joint alignment and stability as well as to prevent complications. In practice, determining the most appropriate surgical approach, implant selection and positioning remains a challenge and depends on the surgeon's experience and patient-specific factors. Therefore, this scoping review aims to map the current evidence on surgical fixation methods for proximal tibia fractures in adults. In particular, we seek to first assess study types and their distribution acros

Clearance Criteria for Determining Eligibility for Force Plate Testing After Anterior Cruciate Ligament Reconstruction: A Scoping Review.

Background and Objectives: Throughout the return-to-play process after anterior cruciate ligament reconstruction (ACLR), clearance criteria and limb symmetry indices (LSI) play an important role in clinical decision-making by helping evaluate patient readiness and informing safe activity progressions, with the goal of reducing re-injury risk. How clearance criteria are implemented in research studies to evaluate patient readiness, specifically in force plate jumping studies, is currently unknown. This scoping review was a focused examination of clearance criteria and limb symmetry indices in s

Open surgery for isolated posterolateral corner injury with hypermobile lateral meniscus was successful at a minimum follow up of 2 years.

BACKGROUND: Isolated posterolateral corner (PLC) injuries classified as grade I/II or type B (Fanelli-Larson) may coexist with a hypermobile lateral meniscus (LM). Although lesions are often managed conservatively, residual rotational instability from LM hypermobility can persist, warranting surgery in patients. We evaluated outcomes of open repair for isolated PLC injury with hypermobile LM at a minimum 2-year follow up. METHODS: Twenty-eight consecutive patients undergoing open repair were retrospectively evaluated at a minimum 2-year follow up (median, 2.8 years; interquartile range (IQR),
