Impact of virtual monoenergetic images on the assessability of lower extremity arteries in (Poly-) trauma photon-counting detector CT.

BACKGROUND: Computed tomography (CT) is state of the art imaging modality in patients after severe trauma. A double-bolus contrast media protocol can be used to combine both arterial and parenchymal contrast in one scan. In case of lower extremity trauma, the CT scans can be extended to include the legs in order to rule out vascular pathologies. Virtual monoenergetic (VME) images derived from a Photon-Counting detector CT (PCDCT) proved to be helpful in vascular diagnostics and could therefore also add information in these extended trauma scans. Aim of this study was to determine the VME recon

Micro-CT-based virtual reconstruction of a kerf-like defect in a partially severed human clavicle: a case report.

We report a forensic case in which micro-computed tomography (micro-CT)-based three-dimensional (3D) registration of partially severed human clavicle fragments provided additional morphological context for interpreting skeletal toolmarks. Conventional stereomicroscopic examination of the cleaned cut surfaces revealed crossing and overlapping striations that superficially resembled reciprocating saw marks and were difficult to interpret in isolation. The registered micro-CT models exhibited an acute, V-shaped, kerf-like profile with a maximum apparent penetration depth of 7.3 mm. A Python-based

Posterior Hamstring Harvest and Preparation for Quadruple Autograft All-Inside Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Hamstring tendon autografts are the most commonly used grafts for anterior cruciate ligament (ACL) reconstruction and are usually harvested through an anterior approach. This harvest is not without risks of complications, and the tendons can alternatively be harvested through a posterior approach. INDICATIONS: Patients undergoing autograft hamstring ACL reconstruction without previous ipsilateral hamstring harvest or distal hamstring injury. TECHNIQUE DESCRIPTION: The patient is positioned supine, and the leg is elevated. With the knee in flexion, the semitendinosus can be palpat

Endoscopic Repair of a Partial Undersurface Proximal Hamstring Tendon Tear.

BACKGROUND: Proximal hamstring tendons are commonly injured among athletes, and when tendon tears are indicated for surgery, the gold standard has been open repair with suture anchors. However, endoscopic repair techniques like the technique described in this study for hamstring undersurface concealed crescentic injuries (HUCCIs) have been shown to have high patient satisfaction and limit perioperative complications. INDICATIONS: General indications for surgery for hamstring tears are 2-tendon, partial tears with >2 cm of retraction or 3-tendon complete tears that have been recalcitrant to no

Electromyographic and co-activation patterns during landings: A sex-based analysis in athletes.

Knee injuries remain prevalent in sports involving landing tasks, particularly among non-professional athletes. Sex-specific neuromuscular strategies during landing may contribute to different joint stabilization patterns. The aim of this study was to examine sex-related differences in thigh muscle activation and hamstrings-to-quadriceps (H/Q) co-activation during standardized single-leg landings in recreational athletes. A total of 162 physically active participants (25% women) performed a single-leg drop landing from 30 cm. Surface electromyography was recorded from the rectus femoris and bi

A Cadaveric Case Report of an Anomalous Sural Nerve Course Through the Gastrocnemius.

The sural nerve is a superficial sensory nerve of the posterior leg that innervates the lateral foot. Anatomical variations in its course are clinically relevant, as they may affect surgical procedures, nerve blocks, and the risk of iatrogenic injury. This case report describes a rare variant in which the sural nerve pierces between the two heads of the gastrocnemius muscle, a course infrequently reported in the literature and important for surgical awareness. During routine lower-extremity dissection by first-year medical students at William Carey College of Osteopathic Medicine, Hattiesburg,

The effects of ultrasound stimulation on passive muscle stiffness of triceps surae.

Passive muscle stiffness is a critical mechanical property that influences muscle function and the risk of strain injury. This study aimed to investigate the acute effects of 1  MHz ultrasound stimulation on the passive muscle stiffness of the triceps surae. One leg of 20 healthy adults was assigned to the ultrasound-stimulated leg, while the other served as a control leg. The ultrasound-stimulated leg received continuous ultrasound (1  MHz, 2.0  W/cm2) for 10  min, while the control leg did not. Shear wave velocity (SWV), an index of passive muscle stiffness, was measured using shear wave ela

Diagnostic pitfalls in forensic pathology: Postmortem facial lesions mimicking trauma caused by pharaoh ants (Monomorium pharaonis).

Misinterpretation of insect-related postmortem artifacts as traumatic injuries remains a critical diagnostic challenge in forensic pathology, particularly in enclosed indoor environments. We report a case involving a deceased adult male found indoors with multiple superficial perioral and facial lesions initially suspected to be perimortem trauma. However, based on photographic evaluation by a forensic entomologist, the lesions were identified as likely caused by postmortem activity of Monomorium pharaonis (pharaoh ants), a synanthropic species commonly found in human dwellings. Numerous ant e

Direct Anterior Approach to Hip Resurfacing Arthroplasty.

BACKGROUND: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate preparation of the femoral head and acetabulum is critical for successful outcomes3. In the present video article, we describe a reproducible technique utilizing preoperative 3D computed tomography (CT)-based planning and patient-specific 3D-printed jigs to optimize femoral head p

A rare case of planned complex suicide involving multiple methods: forensic reconstruction and interpretive challenges.

Planned complex suicides that combine multiple mechanisms pose substantial challenges for medicolegal reconstruction. We report a rare case of a 27-year-old man who simultaneously employed four methods: plastic-bag asphyxia with a continuous natural-gas supply (stove hose taped to bag tubing), environmental sealing of door seams with adhesive tape, rodenticide ingestion, and superficial incised injuries to the neck and wrist. Scene reconstruction, full autopsy, and comprehensive toxicology established asphyxia from natural-gas inhalation via the plastic-bag apparatus as the cause of death; ant

Fatal ileal perforation following umbilical granuloma excision: A medico-legal case analysis.

Umbilical granuloma (UG) is a benign and common umbilical anomaly in neonates and infants, typically managed conservatively with salt application or silver nitrate. While rarely requiring surgical intervention, complications arising from invasive treatments are significantly underreported. We present a rare, but fatal case, of bowel perforation following electrocautery excision of a UG in an 11-month-old male infant. The child had a persistent pinkish umbilical lesion unresponsive to conservative treatment. Ultrasonography ruled out underlying vitellointestinal anomalies, and surgical excision

High-Resolution Ultrasonographic Morphometry of the Superficial and Deep Peroneal Nerves in Healthy Young Adults: Reference Values From an Indian Cohort.

Introduction Peripheral nerve ultrasonography is an important diagnostic modality for evaluating nerve pathologies. Cross-sectional area (CSA) and perimeter measurements are key parameters for diagnosis and clinical management. The deep peroneal nerve (DPN) and superficial peroneal nerve (SPN) are prone to entrapment and traumatic injuries, yet normative morphometric data for these nerves in healthy individuals are limited. Understanding their relationship with anthropometric parameters such as body mass index (BMI) may help refine diagnostic criteria and guide procedures, including nerve bloc

Two autopsy cases of superficial siderosis: ultrastructural similarity across traumatic and non-traumatic contexts and forensic uncertainties.

Superficial siderosis of the central nervous system (SSCNS) is increasingly recognized radiologically but remains rarely documented in medicolegal autopsies, where its role in the cause and manner of death is often uncertain. We aimed to characterize the neuropathological features and medicolegal implications of SSCNS in two autopsy cases. We examined two men with SSCNS, one with a remote traumatic subarachnoid hemorrhage and one diagnosed with idiopathic SSCNS. In both cases, the brain and upper cervical cord showed symmetrical brown discoloration of the cerebellum and brainstem, with widespr

Self-inflicted Thoraco-abdominal stabbing followed by bathtub drowning: Reconstruction of the terminal mechanism in a complex suicide.

Complex suicide involving sharp-force injury and drowning may closely mimic homicide and requires careful reconstruction of the terminal mechanism. We report an anonymized case of a 27-year-old man found prone in a bathtub containing water mixed with blood after emergency entry into his apartment. Blood traces were documented in several rooms. Autopsy identified 18 stab-incised wounds of the precordial and epigastric regions, hesitation-type superficial injuries, old linear forearm scars, and no defensive injuries. Seven wounds penetrated the thoracic cavity; however, the limited right ventric

Postoperative Infections Following Open Tibia Fractures: A Retrospective Study Evaluating Incidence and Prognostic Factors at a Major Trauma Centre.

Background Open fractures are common, high-risk injuries in which the fracture site communicates with the external environment, predisposing patients to postoperative fracture-related infection (FRI). These infections can lead to prolonged hospital stay, additional surgery and long-term morbidity.  Objectives To determine the incidence of FRI following open tibia fractures at a single major trauma centre and explore potential prognostic factors, including timing of antibiotic administration, timing of initial debridement, and definitive skeletal and soft-tissue management.  Methodology This wa

Hybrid Total Achilles Tendon Reconstruction Combining Acellular Dermal Matrix Placement and Free Latissimus Dorsi Flap Coverage: A Report of Two Cases.

Reconstruction of complex Achilles tendon defects involving both the tendon and the overlying soft tissues represents a challenging scenario for plastic surgeons. In this report, we present an innovative technique of hybrid total Achilles tendon reconstruction combining acellular dermal matrix placement to restore the full-thickness continuity of the tendon and free latissimus dorsi (LD) flap coverage to allow graft integration and simultaneously resurface the soft tissue defect, ultimately leading to lower limb salvage. Between August 2023 and February 2024 two patients received microsurgical

Repair and reconstruction of lower extremity defects with combined pedicled superficial circumflex iliac artery perforator flap.

BACKGROUND: Extensive lower extremity tissue defects pose difficulties for surgeons. Considering the natural contour, aesthetics, biomechanics and restoration of motor function, matching and customizing a flexible and large flap is the ideal solution. Restoring circulation early can reduce the risk of infection and flap necrosis. As superficial circumflex iliac artery perforator flaps (SCIP flaps) have matured to cover multiple defects, we present a novel combined pedicled SCIP flap for repair and reconstruction of tissue defects in lower extremity. METHODS: From September 2015 to June 2023,
