Superficial posterior compartment syndrome of the leg with deep venous compromise. A case report.

A 15-year-old boy was struck twice in the calf, developing what appeared initially to be phlebitis and deep venous compromise. Careful clinical examination and compartment pressure measurement revealed the presence of an isolated superficial posterior compartment syndrome. Fasciotomy provided prompt relief. Findings at surgery showed ischemic changes but no necrosis. Venous patency was demonstrated on a subsequent venogram. Recovery was complete. This is the first reported case of isolated traumatic superficial posterior compartment syndrome. It adds to numerous other reports and provides furt

[Diagnosis of disorders of lymph flow in restricted edema of the lower limbs].

The direct "inferior" lymphography of the medial superficial collector was used for the examination of 45 patients with posttraumatic edemas of lower extremities. The cause of them was shown to be not only a trauma of major collectors but also multiple injuries of smaller lymph vessels, the stabilization of the process and reduced edema being developed at the expense of organization of the lateral lymph flow by branching the lymph vessels of the collector inferior to the blockade level, and in its certain portions by anastomoses between the sources of smaller vessels falling into different seg

Accidents in the surgical treatment of varicose veins.

Serious accidents in the surgical treatment of varices are quite exceptional and it is impossible to say with any accuracy how often they occur. Generally they result from major errors of surgical technique, and arise mainly from: haemorrhaging after the tearing of the femoral vein or because of the loosening of the stump of the saphenofemoral junction, traumatism of the femoral artery, damage of the nervus peronaeus in crossectomy of the saphena parva. Minor accidents are more frequent but here too it is impossible to state a precise percentage. There are four fairly common types of accident:

Adductor canal outlet syndrome.

On rare occasions, jogging, skiing, and other athletic activities can cause thrombosis of the superficial femoral artery at the outlet of Hunter's canal. We report three cases in which mechanical trauma of the femoral artery in Hunter's canal resulted in intimal tear and a segmentally localized thrombosis. It was confirmed by anatomic dissection that the intrinsic mechanical trauma occurred at the point where the femoral artery crosses the adductor magnus tendon. This tendon has been previously implicated as a cause of atherosclerotic stenosis and thrombosis. An awareness of its possible relat

Femoral anastomotic aneurysms.

Both the literature and this experience support host vessel degeneration as the primary etiologic factor in femoral anastomotic aneurysms. Associated factors that produce increased "intra-anastomotic tension," such as hypertension, superficial femoral artery occlusion, and flow turbulence, appear to contribute to vessel deterioration. Other factors, much less prevalent in present-day vascular surgery, such as rigid grafts, deficient suture material, inappropriate angle of incidence, and excessive tension on the graft can contribute to anstomotic disruption. Certain guidelines may be helpful in

[Free omental autografts revascularized by microsurgery (author's transl)].

The greater omentum is good plastic material and supplies richly vascularized connective tissue for transplants. Some superficial and deep lesions of the soft tissues require extensive excision, and in favorable cases it is possible to use the greater omentum pedicled as described by Kiricuta. Two such cases are reported. In other cases a free transplant of omentum revascularized by microsurgical methods has to be used. This procedure was employed in ten patients and allowed repair of lesions with wide or deep loss of tissue under excellent conditions. An intermediate saphenous graft was neces

Hypotrophy of the soleus muscle in man after achilles tendon rupture. Discussion of findings obtained by computed tomography and morphologic studies.

Seven athletes (age range, 35 to 43 years), who sustained total subcutaneous ruptures of the Achilles tendon 2 to 5 cm above its distal insertion, were treated surgically with suturing of the tendon, immobilization of the leg and foot for 6 weeks, and cast changes so as to increase the dorsiflexion of the foot. Needle biopsies were obtained several times from the soleus muscles of both the injured and uninjured legs at a depth of about 5 cm. The cross-sectional area was measured by computed tomography at the same level the tissue was obtained by biopsy. Results of morphologic studies revealed

Volkmann's contracture in children: aetiology and prevention.

A review was conducted of the records of fifty-five children who were admitted to the Hospital for Sick Children in Toronto between 1955 and 1975 with a diagnosis of Volkmann's contracture in fifty-eight limbs. Ten patients had been transferred to this hospital with established ischaemia after Bryant's traction for a fractured femur; all had a very poor outcome. Thirteen other cases of Volkmann's contracture affecting the superficial posterior compartment had been treated with a fixed Thomas' splint and a Bradford frame after fractures of the femoral shaft. Supracondylar fractures of the elbow

[Percutaneous transluminal recanalization in a patient with arterial occlusive disease and multiple injuries (author's transl)].

A case of a 66 year old patient with multiple injuries is reported. A combination of conventional trauma treatment and percutaneous transluminal angioplasty (PTR) of a segmented superficial femoral artery occlusion prevented leg amputation. When the patient died 8 weeks later from trauma sequelae the successful recanalization was demonstrated in histopathology investigation. It is concluded that PTR is an ideal accessory to the present therapeutic spectrum in vascular surgery.

Arterial injuries of the extremities.

Fifty-eight patients with arterial injuries of the extremities were treated during the past 8 years. Fifty-one had acute injuries and seven had nonacute injuries. Blunt trauma or shotgun wounds caused 74% of the injuries, and 55% were associated with skeletal trauma. All the acute injuries endangered the limb; the average ischemic time was 8.5 hours. Brachial, popliteal, and superficial femoral arterial injuries were seen most frequently. Repair was accomplished with autogenous saphenous vein grafts in 47% and end-to-end anastomosis in 41%. Six patients died, four due to injuries of other orga
