Osteochondral Allograft Reconstruction for Post-traumatic Osteo-Articular Defects of the Knee: A Case Report.

CASE: A 21-year-old male sustained a high-velocity open knee injury with extensive osteochondral loss of the lateral femoral condyle, patella, and proximal tibia with an intact extensor apparatus. Following staged debridement and external stabilization, reconstruction was performed using a size-matched osteochondral allograft (OCA). The graft was contoured to restore condylar geometry and incorporated using internal fixation. At the 2-year follow-up, radiographs confirmed graft incorporation and the patient demonstrated excellent functional recovery. CONCLUSION: OCA transplantation is a valua

Comparative biomechanical analysis of hinge fracture risk between medial open-wedge high tibial osteotomy and distal tuberosity osteotomy: a porcine tibia study.

BACKGROUND: Medial open-wedge high tibial osteotomy (OWHTO) is widely performed to correct varus malalignment associated with medial compartment osteoarthritis of the knee. Despite its popularity, this technique is frequently accompanied by undesirable complications, such as lateral hinge fractures, an increase in posterior tibial slope (PTS), and a reduction in patellar height. As an alternative, the open-wedge distal tuberosity osteotomy (OWDTO) has been introduced to preserve the patellar position. However, limited biomechanical information exists regarding its influence on hinge fracture s

Bone Tunnel Microarchitecture and Early Functional Recovery After Anterior Cruciate Ligament Reconstruction: A Prospective HR-pQCT Cohort Study.

BACKGROUND: The biological healing of the bone tunnel is crucial for success in anterior cruciate ligament reconstruction (ACLR). Nonetheless, conventional imaging cannot assess the key microarchitectural properties of the healing bone, resulting in a gap in our understanding of early recovery. PURPOSE/HYPOTHESIS: This study aimed to identify the specific bone structure and density measurements most closely associated with successful recovery during the first 6 months. It was hypothesized that the microarchitectural properties of the bone tunnel, quantified by high-resolution peripheral quant

The Management of Tibial Spine Avulsion Fracture in a Skeletally Immature Patient by Using Staple Fixation: A Case Report.

Tibial spine (TS) fractures are uncommon injuries in children and typically occur after sports-related trauma or road traffic accidents. We report the case of an 11-year-old male who presented with severe pain, swelling, and an inability to move the left knee after a direct impact during a road traffic accident. Initial radiographs demonstrated a displaced TS fracture. CT confirmed an avulsion fracture at the anterior cruciate ligament (ACL) tibial insertion with intra-articular incarceration of the fragment. MRI showed bone contusions and an avulsed ACL insertion without disruption of the ACL

Two Case Reports of Tibial Avulsion Fractures of Both Cruciate Ligaments and the Medial Collateral Ligament: A Narrative Review of Current Literature.

Tibial avulsion fractures involving both the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL) are rare and pose significant diagnostic challenges, particularly when accompanied by medial collateral ligament (MCL) injury. This report presents two such rare cases and provides a focused narrative review of the current literature. Both patients underwent individualized surgical strategies based on fracture morphology, soft-tissue status, and stability requirements, using a combination of open and arthroscopic techniques. Although postoperative complications occurred, incl

Complexities in Hoffa Fracture Management: A Case Report on Diagnosis and Fixation Technique.

INTRODUCTION: Hoffa fractures, accounting for 0.65% of femoral fractures, are rare injuries to the distal femur, typically affecting the lateral condyle. These fractures are often overlooked in radiographs and pose challenges in classification, as traditional systems like Letenneur's may not adequately address certain unique fracture patterns, as seen in the present case. CASE PRESENTATION: A 25-year-old male with trauma to the left knee and pelvis was diagnosed with a pelvic open-book injury and a coronal split Hoffa fracture of the lateral femoral condyle. Surgical fixation involved externa

Updated German National S1 Guideline on Pediatric Lower Leg Shaft Fractures.

Fractures of the diaphysis of the lower leg account for approximately 6% of all fractures in children and adolescents. A distinction must be made between isolated tibial shaft fractures, which are primarily managed conservatively, and combined lower leg shaft fractures (involving both the tibia and fibula). The latter are significantly less stable and therefore more frequently treated with osteosynthesis. If the fracture is non-displaced or tolerably displaced, immobilization in a long leg cast for about 4 weeks is sufficient. This requires that no secondary displacement occurs within the cast

Safety of Rigid Intramedullary Nailing in Skeletally Immature Tibia Fractures-Risk of Growth Disturbance is Not Zero.

BACKGROUND: Rigid intramedullary nail (RIMN) fixation is the standard treatment for tibial shaft fractures in adult patients. Recently, this technique has gained favor for usage in skeletally immature patients despite the required violation of the proximal tibial physis. The goal of this paper was to determine whether growth disturbance with resultant deformity could occur after RIMN treatment in patients with open physes. METHODS: A retrospective radiographic review was performed of all patients undergoing RIMN placement for acute tibia fractures at 3 level I Pediatric Trauma Centers between

Acute or Delayed TKA for Tibial Plateau Fracture? An Observational Study From the Swedish Arthroplasty Register.

BACKGROUND: Acute TKA has been proposed as an alternative to open reduction and internal fixation for complex tibial plateau fractures in patients who are older and who have compromised bone quality. The alternative is a delayed TKA after primary fracture management with an unfavorable outcome. However, the long-term outcomes and risk for reoperation after acute TKA compared with delayed TKA for fracture sequelae remain unclear. QUESTIONS/PURPOSES: When comparing acute TKA (< 3 months after injury) for tibial plateau fracture with delayed TKA for fracture sequelae, we asked: (1) Do the risks

Evaluation of Functional Outcomes After Arthroscopic All-Inside Meniscal Repair.

Introduction Meniscal injuries are among the most common knee pathologies and play a significant role in the development of early osteoarthritis when inadequately treated. Preservation of the meniscus has therefore become a key principle in modern knee surgery. Arthroscopic all-inside meniscal repair has gained popularity due to reduced surgical morbidity, shorter operative time, and lower risk to neurovascular structures compared with traditional repair techniques. This study evaluates the functional outcomes and safety of the arthroscopic all-inside meniscal repair technique. Methodology Thi

The Anatomy of the Anterior Intermeniscal Ligament in Children: Implications for Tibial Spine Fractures, Meniscal Injuries, and Anterior Cruciate Ligament Reconstruction.

BACKGROUND: The anterior intermeniscal ligament (AIML) footprint and dimensions in skeletally immature knees have not been well documented. The AIML may play an important role in tibial spine fracture (TSF) reduction and meniscal repair/stability. The AIML is vulnerable to injuries during drill hole placement in anterior cruciate ligament (ACL) reconstruction. Therefore, it is necessary to characterize the anatomy of the AIML in skeletally immature knees to prevent iatrogenic injuries. PURPOSE: To evaluate AIML dimensions, its meniscal attachment patterns, and its relationship to the ACL. ST

Surgical Management of Horizontal Cleavage Tears of the Medial Meniscus: A Biomechanical Analysis During Simulated Gait.

BACKGROUND: Horizontal cleavage tears (HCTs) comprise one-quarter of all diagnosed meniscal pathologies. Despite several static/quasi-static cadaveric studies showing that HCTs and their repairs do not change contact mechanics while leaflet resections do, it is unclear if these conclusions hold during activities of daily living. HYPOTHESIS/PURPOSE: The purpose of this study was to quantify the comparative biomechanical effects of meniscal HCT, HCT repair, and leaflet resection on joint contact forces throughout simulated gait. It was hypothesized that HCT repair would not change contact mecha

Open Reduction and Internal Fixation Versus Suprapatellar Nailing and Condylar Bolts for the Management of Complex Bicondylar Tibial Plateau Fractures: A Clinical Study.

Purpose The surgical fixation of bicondylar tibial plateau fractures is a challenging process. Open reduction and internal fixation (ORIF) is considered the "gold standard" treatment, while recently, a minimally invasive surgical technique that combines Suprapatellar Nailing and condylar Bolts (SNB) has been proposed for the fixation of these injuries. The present study compares ORIF and SNB techniques with the aim of providing information about the effectiveness of the SNB technique. Methods This clinical study examined 48 patients with complex tibial plateau fractures who were managed at the

Prevalence of Medial Meniscus Injury After Delayed ACL Reconstruction.

BACKGROUND: The menisci serve as secondary stabilizers in the knee, and anterior cruciate ligament (ACL) tears have been associated with meniscal tears. Previous studies have indicated that a delay ≥12 months is associated with development of new meniscal pathology in an ACL-deficient knee. Some studies have indicated that the time period might be shorter in the pediatric and adolescent population, and other predictive factors remain poorly defined. PURPOSE: To examine the association between time, patient-related factors, and the extent and location of meniscal pathology for patients of all
