Are new implants for patellar fractures reasonable? A biomechanical comparison of intramedullary locking nails versus tension-band osteosynthesis.

BACKGROUND: Patellar fractures are predominantly treated surgically by open reduction and internal fixation using classic tension band osteosynthesis. Biomechanical testing was conducted to examine transverse patellar fracture stabilization using locked intramedullary nail systems in comparison to classic tension band osteosynthesis. METHODS: Twenty-four Sawbones® with transverse patellar fractures type AO 34-C1.1 were divided equally into three test groups. Three principles for fracture stabilization were used: classical tension band osteosynthesis and newly developed locked intramedullary do

Time-Dependent Alterations in Walking Biomechanics After Anterior Cruciate Ligament Reconstruction Compared with Healthy Individuals: A Systematic Review with Meta-analysis.

BACKGROUND: Knee injuries involving the anterior cruciate ligament (ACL) are often treated through surgical care to restore knee joint stability. Despite surgical treatment, there is evidence of altered biomechanical gait characteristics for short-, mid-, and even long-term post ACL reconstruction (ACLR). OBJECTIVE: In this systematic review with meta-analysis, we aimed to analyze lower extremity joint kinematics and kinetics recorded during walking in individuals with ACLR and determine the time course of recovery of gait biomechanics following ACLR. METHODS: Five electronic databases (Scop

A Modified Lag Screw and Cerclage Wire Technique for the Management of Sagittal Patellar Fractures: A Case Report.

Sagittal patellar fractures account for a minority of patellar injuries and remain insufficiently represented in the orthopedic literature, particularly with respect to standardized operative fixation techniques. These fractures pose unique biomechanical challenges due to patellofemoral joint forces and the need to address both compressive and tensile stresses across the fracture site. We report the case of a 27-year-old man who sustained a displaced medial sagittal patellar fracture (AO/OTA 34B2.1) following a high-energy motor vehicle collision. Given the degree of articular displacement and

Translation, Validation, and Cultural Adaptation of the Greek Versions of the Lysholm Knee Score and Tegner Activity Scale in Patients With Anterior Cruciate Ligament Injury.

BACKGROUND: The Lysholm score and the Tegner Activity Scale (TAS) are two knee-specific patient-reported outcome measures with widespread clinical and research use in patients after anterior cruciate ligament (ACL) injury. However, there is a lack of previously validated Greek-language patient-reported outcome measures for ACL-injured patients. The aim was to translate, cross-culturally adapt, and evaluate the psychometric properties of the Lysholm score and TAS in Greek for patients with ACL injury. METHODS: Translation and cross-cultural adaptation of Lysholm and TAS questionnaires into Gre

Management of posterior cruciate ligament tibial avulsion fractures using a mini-open screw fixation technique.

BACKGROUND: Posterior cruciate ligament (PCL) tibial avulsion fractures are relatively infrequent injuries but may result in persistent posterior instability and compromised knee function if not managed appropriately. METHODS: This prospective case series included 12 patients with isolated PCL tibial avulsion fractures who underwent fixation using a mini-open posterior approach between January 2020 and December 2023. Fractures were classified according to the McKeever classification. Patients were followed clinically and radiologically for a mean duration of 11.25 ± 1.60 months (range, 9-14 m

Surgical Timing and Rehabilitation (STaR) for Multiple Ligament Knee Injuries: Study Protocol for a Multicenter Integrated Clinical Trial.

Multiple ligament knee injuries (MLKIs) often result in persistent pain, residual disability including post-traumatic osteoarthritis, and high healthcare costs. Although nonrandomized and retrospective research supports operative management, evidence informing the optimal timing of MLKI surgery and subsequent rehabilitation is lacking. In addition, return to preinjury military duty, work, and sports after MLKI is poorly understood. The Surgical Timing and Rehabilitation (STaR) Trial for MLKIs was designed to address these gaps in evidence for surgery and postoperative rehabilitation for milita

Establishment of a Rat Pull-Out Repair Model Demonstrating Fibrous Meniscus-Bone Healing and Suppression of Meniscal Extrusion After Medial Meniscus Posterior Root Tear.

BACKGROUND: Medial meniscus posterior root tear (MMPRT) disrupts meniscal hoop tension and is associated with osteoarthritis progression. Although pull-out repair is a standard surgical technique for MMPRT, its precise healing process remains unclear. PURPOSE/HYPOTHESIS: This study aimed to establish a rat model for MMPRT pull-out repair, investigating its biomechanical and histological healing and its effect on medial meniscus extrusion (MME). It was hypothesized that pull-out repair would show reduced MME and restore biomechanical property toward normal. STUDY DESIGN: Controlled laboratory

Bone Marrow Aspirate Concentrate Improves the Early Osseous Integration of Fresh Osteochondral Allografts in the Knee: A Randomized Controlled Trial.

BACKGROUND: Fresh osteochondral allograft (FOCA) transplantation is an increasingly used technique for treating symptomatic cartilage defects in young and active patients. However, insufficient osseous integration of the graft remains a primary cause of failure. Bone marrow aspirate concentrate (BMAC), rich in mesenchymal stem cells, may enhance graft integration. PURPOSE: To determine whether the use of autologous BMAC in FOCA transplantation of the knee improves osseous integration on computed tomography (CT) during the first postoperative year and yields superior clinical outcomes compared

Nodular fasciitis with intra-articular presentation within the knee.

A female in her early 30s presented with worsening right knee pain with atraumatic onset. Imaging revealed a heterogeneous, contrast-enhancing, T1 hypointense mass in the retropatellar fat pad. Initial differential diagnosis favoured localised tenosynovial giant cell tumour. Malignancy was also considered. These alternative pathologies require more aggressive treatment modalities and demonstrate significant recurrence rates. The patient ultimately underwent open excision and histological analysis which revealed intra-articular nodular fasciitis (NF). Detection of ubiquitin-specific protease 6

Top 10 Technical Tips and Tricks for Safe Sciatic Nerve Management During Posterior Hip Endoscopy.

BACKGROUND: A thorough understanding of posterior hip anatomy is essential for safe and effective posterior hip endoscopy. The sciatic nerve runs laterally to the ischium and hamstring origin. While sciatic nerve injury during proximal hamstring repair is uncommon, the increasing use of endoscopic techniques has led to a rise in iatrogenic injuries. This highlights the need for a structured approach to identifying and protecting the sciatic nerve. INDICATIONS: Surgical intervention is indicated for complete proximal avulsions involving all 3 hamstring tendons, partial avulsions involving ≥2 t
