Biomechanical evaluation of a medial-only fixation strategy for Takeuchi type 2 lateral hinge fractures in medial open wedge high tibial osteotomy.

BACKGROUND: Medial opening wedge high tibial osteotomy (MOWHTO) is a standard treatment for knee osteoarthritis. However, lateral hinge fractures occur in 19-25% of cases, with Type 2 fractures causing significant instability. While lateral plating is recommended, it requires an additional incision. This study investigated whether medial-only fixation using a 4.5 mm cortical screw inserted into the Tomofix plate's oval hole could provide sufficient stability for Type 2 hinge fractures. METHODS: Twelve fresh pig knees underwent MOWHTO with a 6 mm opening. They were divided into two groups: Gro

Dual-purpose Laparoscopy and Bipedicled Anterolateral Thigh Flap for Abdominal Wall Reconstruction After High-voltage Electrical Burn.

High-voltage electrical burns involving the abdominal wall pose complex management challenges, including delayed bowel perforation, progressive tissue necrosis, and durable reconstruction in compromised tissue. To our knowledge, no standardized approach has been described that combines visceral surveillance with abdominal wall reconstruction in this setting. We report a case of a 43-year-old man who sustained a 22,000-volt electrical injury with 30% total body surface area burns and a full-thickness lower abdominal wall defect (16 × 13 cm) exposing the intact peritoneum. Triple-contrast comput

Neglected Busch-Hoffa fracture following the patellar dislocation: a rare case report.

INTRODUCTION AND IMPORTANCE: Busch-Hoffa fractures are rare intra-articular coronal plane fractures of the femoral condyle that often remain unnoticed, particularly when complications develop. Due to their instability and risk of long-term problems, these fractures require immediate anatomical reduction and stable fixation. The occurrence of a Busch-Hoffa fracture in conjunction with patellar dislocation is extremely rare and has been rarely reported in the current literature. CASE PRESENTATION: We report a case involving a 21-year-old male who experienced a Bagaria type 2 Hoffa fracture of t

Efficacy and Safety of Intraosseous Regional Versus Intravenous Tranexamic Acid in Total Knee Arthroplasty: A Randomized Controlled Trial.

BACKGROUND: Tranexamic acid (TXA) reduces perioperative blood loss in total knee arthroplasty (TKA); however, evidence for intraosseous regional administration (IORA) remains limited. We evaluated the efficacy and safety of IORA-TXA versus intravenous TXA (IV-TXA) in TKA. METHODS: In this prospective, randomized, single-center trial, 98 patients undergoing primary TKA were randomized 1:1 to IORA-TXA or IV-TXA (49 per group) at a uniform dose of 20 mg/kg. The primary outcome was calculated perioperative blood loss. Secondary efficacy outcomes included calculated hidden blood loss, postoperativ

Is Cell-Free Collagen Matrix Effective in the Treatment of Large Cartilage Defects of Knee? Early Functional and Quantitative Magnetic Resonance Imaging Cartigram Assessment in Two Patients.

INTRODUCTION: Articular cartilage injuries, particularly large full-thickness defects (International Cartilage Repair Society Grade IV), present a significant challenge due to the tissue's limited regenerative capacity. Cell-free collagen matrix, an acellular Type I collagen matrix, has shown promise in promoting cartilage repair. However, its role in treating large chondral defects of the knee with objective radiological correlation remains underexplored. Through two case reports, we have shown that cell free collagen matrix is also effective in early healing of large cartilage defects of kne

Immediate Unrestricted Assisted Full Weight-Bearing After Bicondylar Tibial Plateau Fixation: A Case Series.

High-energy bicondylar tibial plateau fractures of Schatzker types V and VI are conventionally managed, after open reduction and internal fixation, with 6-12 weeks of restricted weight-bearing, out of concern for loss of reduction and articular collapse. Emerging evidence questions whether this restriction is necessary. Reports of immediate weight-bearing specifically after fixation of these most unstable bicondylar patterns remain scarce, as most comparative studies pool this subtype within mixed cohorts. We present three consecutive patients (two men and one woman; mean age: 54 years; all of

A rare traumatic triad: Open bicondylar non-conjoint Hoffa fracture with partial tibial tubercle and medial epicondyle avulsions - A case report and biomechanical analysis.

UNLABELLED: Coronal shear fractures of the distal femoral condyles are rare intra-articular injuries, and the simultaneous occurrence of a non-conjoint bicondylar Hoffa fracture with a partial anterior tibial tubercle avulsion and a medial epicondyle avulsion constitutes an exceptionally complex traumatic triad. We report the case of a 35-year-old male motorcyclist who sustained this "floating" articular block injury (AO/OTA 33-B3.3) following a high-velocity accident. Diagnostic assessment utilizing Computed Tomography with 3D reconstruction revealed a vertical Letenneur Type II fracture of t

The Role of Psychosocial Factors in Returning to Physical Activity and Sport Among Female Football Players After Anterior Cruciate Ligament Injury: A Scoping Review.

BACKGROUND: Anterior cruciate ligament (ACL) injuries are an increasingly common knee injury in sport requiring quick direction changes, such as the football codes. Return to physical activity and sport following an ACL injury predominantly focuses on achieving objective milestones to resolve physical impairments, with sex differences reported. However, irrespective of management approach following an ACL injury, many athletes do not return to the same level of sport, if at all, warranting the need to consider psychological and social factors. AIM: The aim of this scoping review was to invest

The Management of Orthopaedic Emergencies for the Community Surgeon.

Orthopaedic emergencies demand rapid recognition and decisive action, particularly in resource-constrained or nontertiary settings where the community surgeon often serves as the first and only line of defense. It is important to provide a practical, evidence-informed guide for the management of musculoskeletal injuries, including irreducible joint dislocations, ultra-low-energy knee dislocations, open fractures with vascular compromise, compartment syndrome, and femoral neck fractures in young patients. Emphasis is placed on early diagnosis, stabilization strategies, soft-tissue management, a

The Role of Arthroplasty in Acute and Failed Fracture Surgery Around the Knee.

Total knee arthroplasty (TKA) has become an increasingly valuable solution for managing acute periarticular fractures and failed fixation, particularly in elderly patients with severe comminution or underlying arthritis. Its key advantage lies in enabling immediate weight bearing and potentially reducing the need for reoperation compared with traditional fixation methods such as open reduction and internal fixation or intramedullary nailing. Although open reduction and internal fixation and intramedullary nailing remain standard fixation methods for many fracture types, primary TKA is gaining

Introducing the Subjective Knee Value for Daily Activity (SKV-d) and Sport (SKV-s) as Single-Item Measures to Assess Knee Function in Athletes.

BACKGROUND: Patient-reported outcome measures (PROMs) are essential for assessing knee function; however, their complexity can hinder efficiency. The subjective knee value (SKV) offers a simple alternative, but lacks distinction between daily activities and sports. PURPOSE: We propose the SKV for daily activity (SKV-d) and sport (SKV-s). This study aimed to validate the SKV-d and SKV-s as simple and accurate tools for assessing knee function. STUDY DESIGN: Cohort Study (Diagnosis); Level of evidence, 3. METHODS: A total of 79 consecutive athletic patients (mean age, 39 years; 49% women) wit

Lateral Extra-articular Procedures Reduce the Risk of Revision of Anterior Cruciate Ligament Reconstruction in Elite Athletes: A Systematic Review and Meta-analysis of Comparative Studies.

BACKGROUND: Lateral extra-articular procedures (LEAPs) have gained increasing attention as an adjunct to anterior cruciate ligament reconstruction (ACLR), particularly in individuals at high risk for reinjury. When combined with ACLR, LEAPs contribute to the restoration of normal knee kinematics and provide a significant reduction in residual anterior laxity compared with isolated ACLR. This added stability provides a protective effect on the intra-articular graft, promoting improved healing and integration while reducing mechanical stress on the reconstructed anterior cruciate ligament (ACL).
