We evaluated 245 cases of comminuted femoral fractures treated by intramedullary nailing and graded them by fracture pattern and degree of stability as segmental fractures, and grade I, grade II, grade III, and grade IV comminuted fractures. The infection rate was 0.4 per cent, and the nonunion rate was 0.8 per cent. Shortening of more than 2 cm. was seen in seven cases (3 per cent), and malrotation of more than 20 degrees was noted in four cases (1.7 per cent). The stable closure of an indolent wound below the knee may require the placement of well vascularized flap tissue. Recent advancements in axial and microvascular flap transfer have nearly obviated the use of the cumbersome traditional cross-leg flap. Thirty-two flaps in 25 below-knee defects illustrate a variety of surgical options. Closed stable wounds were achieved in 24 of the 25 defects, and bony union was accomplished in 11 of 12 unhealed fractures.

Clostridial myonecrosis developed in a compound tibial fracture wound left open after extensive surgical debridement. The patient survived after having a below-knee amputation and postoperative erythromycin. Prophylactically administered cephalothin failed to prevent the clostridial infection.

Author compared the material of supracondylar femur fractures of the National Institute of Traumatology during a period of 5 years starting from 1962 until 1966 with that of a period of 6 years starting in 1972 until 1977. A technique is described for doing arthroscopic meniscectomies using short curved instruments passed through flexible, sealable Teflon sleeves. The goal, with few exceptions, was to achieve en bloc subtotal resection of the posterior two-thirds of the meniscus. The technique of arthrodesis of the knee after removal of an infected total prostheses is described. It was used in four cases. In the first stage, the prostheses and cement were removed, the wound was left open and knee immobilized by Hoffman external fixation. Details of the fixation are given. The second stage of filling of the wound by cancellous chips was performed two weeks later. On average, the fixation was removed after seven months. Some mechanical experiments were made to establish the best type of fixation device.

Nine children who were less than fourteen years old and had open physes and ligament injuries of the knee were studied. Despite an initial thorough physical and roentgenographic evaluation, the full extent of the lesion was determined only at operation in seven of the nine patients. Open joint wounds were made in immature and mature rabbit knees by surgical arthrotomy. The wounds of the right knees were packed open for 5 days and the left knee wounds were closed primarily. The biochemical studies were selected to determine the effect of our treatment regimen on changes in the metabolism of articular cartilage and synovium. In 15 patients traction followed by cast brace was an acceptable method of treatment for ipsilateral fractures of the tibia and femur. Prolonged healing times and limited range of motion of the knee were not significant problems. This method of treatment is especially indicated in patients with severe open injuries or comminution of femoral shaft fractures which do not safely lend themselves to rigid internal fixation. Over 3,500 students from five major high schools in the Seattle metropolitan area were studied as to skiing injuries. Each had skiied an average of 13.3 days. The average age of t'he group was 13.5 years, the ages ranging from 3 to 19 years. The prime indications for tibial tubercle transfer are 2 or more complete patellar dislocations with trivial trauma or recurrent subluxation with or without pain causing "giving way." In severely displaced fractures of the tibial condyles, failure of the method of fixation may lead to disappointing results of open reduction operations. This problem may be successfully and safely treated by using methylmethacrylate (MMC) for internal fixation. In this series 6 of 7 cases achieved an acceptable result of fixation with MMC. This compares favorably to published series of fractures of this type treated by open reduction.
