The conclusion from the above clinical and experimental presentation is that stabilization by using external fixation in problematic cases is the method of choice because the risk of infection is better than by using the standard methods of plating and nailing. Using external fixation based on the tubular system of ASIF we can achieve rigid stability. Correct application allows early mobilization ensuring alignment even in bone defects. In order to prevent permanent disability, a proper diagnosis of the fixed posterior fracture dislocation of the fibula with fracture dislocation of the ankle joint must be made. Full-length (knee to ankle) radiographs are necessary to make this diagnosis. Although a closed reduction has been described as successful, we feel that an open reduction is usually necessary. Freeing of the trapped fibula from behind the tibia results in a satisfactory reduction of this fracture dislocation. The reduction and fixation of the malleolar fragment is then simple, and an excellent result can be expected.

Residual valgus deformity and overgrowth was found in 4 children out of 6 with proximal tibial metaphyseal fractures. Primary reduction of this minimally displaced fracture under general anaesthesia is advised. In case of an irreducable fracture open revision is suggested. A new method of reduction and fixation of a uniformly depressed fracture of the tibial condyle is described. A curved steel probe, introduced through a cortical window in the uninjured condyle, is used to reduce the fracture fragments as well as to compact the fracture region (TV-monitored x-ray projectioning is desired). The instruments needed for the operative procedure are as follows: a probe for reduction and compaction, a tube and a plunger for deposition of bone transplants, and a forked plate. Five cases in which the curved probe reduction method has been used are reported.

For the conservation of crushed extremities all achievements of up-to-date traumatology are to be used. The impairement of the seriously crushed extremity displays a certain similarity to the polytraumatism involving the whole organism. This is to be taken into consideration in the surgical treatment. Conserving the vitality of the extremity deferred treatment and reconstructive operations at a later date become possible. As absolute rule must be in view that all cases are to be examined with the greatest care and individually, as regards the surgical treatment.
