A Case of Severe Necrotizing Soft-tissue Infection With Reconstruction of the Mid-lower Leg Using a Distally Based Pedicled Gracilis Muscle Flap.

Here, we report the management of a severe necrotizing soft-tissue infection involving the entire lower limb of a man in his 70s. The infection originated from a lower leg hematoma that developed during anticoagulant therapy for deep vein thrombosis. Necrosis extended from the deep thigh to the lower leg, involving areas surrounding critical structures such as the femoral artery, sciatic nerve, and tibia. Limb salvage was achieved through multiple debridement procedures and skin grafts performed in collaboration with orthopedic and cardiovascular surgery teams. The exposed cortical bone on the

The role of venoactive drugs in a modern treatment paradigm: evidence review.

Chronic venous disease (CVD) causes substantial symptom burden and healthcare cost, driven not only by venous hypertension but by a persistent molecular cascade of endothelial activation, glycocalyx degradation, and inflammation within the vein wall. Venoactive drugs (VADs) - a heterogeneous class of flavonoid, synthetic, animal-derived, and plant-derived compounds - target this cascade and are recommended by international guidelines as adjuncts to compression therapy, endovenous intervention, and wound care in venous leg ulcer (VLU) management. This review synthesizes the pharmacological rati

Bilateral Lower Extremity Chronic Exertional Compartment Syndrome With Recurrent Symptoms After Prior Fasciotomy: A Case Report.

Chronic exertional compartment syndrome (CECS) is an exercise-related cause of lower extremity pain characterized by reproducible pain, tightness, cramping, and neurologic symptoms that improve with rest. Diagnosis may be delayed because chronic lower leg pain has a broad differential diagnosis, and currently used diagnostic strategies remain imperfect. We report the case of a young woman who began experiencing bilateral lower extremity pain during high school sports that progressed to pain with prolonged standing. Treadmill-provoked bilateral four-compartment pressure testing reproduced sever

Disease Burden and Long-term Psychosocial Outcomes in Children and Young Adults with Graves' Disease.

OBJECTIVE: We aimed to compare disease burden and psychosocial outcomes between patients with GD in childhood/young adulthood and an unexposed population cohort. METHODS: This cohort study used prospectively collected data from medical records and multiple Swedish registers. Patients diagnosed with GD aged <21 years (n=87, of whom 55 were <18 years) in 2003-2005 were included. Each patient was matched with 10 individuals without GD. RESULTS: Over the 17-year follow-up (median: 15.5 years), patients with GD had significantly higher recorded rates of 'endocrine, nutritional, and metabolic dise

Comparing 10-Year Follow-up Outcomes Among Patients Without Unstable Chondral Lesions Versus Those With Unstable Chondral Lesions (Left In Situ) at the Time of Arthroscopic Partial Meniscectomy.

BACKGROUND: The Chondral Lesions and Meniscal Procedures (ChAMP) trial observed the effect of debridement on unstable chondral lesions (CLs) encountered during arthroscopic partial meniscectomy (APM) while following a cohort with no CL to compare outcomes. At 5 years after APM, patients in the ChAMP Trial without unstable CLs had better outcomes than those with unstable CLs left in situ. HYPOTHESIS: Patients without unstable CLs would have better outcomes than those with unstable CLs 9 years after APM. STUDY DESIGN: Cohort study: Level of evidence, 2. METHODS: The authors compared outcomes

A Comparative Study of Topical Treatments for the Management of Chronic Leg Ulcers in a Multi-Center Cohort.

Background Chronic leg ulcers represent a significant clinical and economic burden due to prolonged healing times, frequent outpatient visits, and high recurrence rates. Topical wound care is widely used; however, comparative evidence supporting specific topical agents remains limited, and inappropriate or prolonged use may delay healing or increase complications. This multi-center study aimed to compare the effectiveness of three commonly used topical treatment strategies in the management of chronic leg ulcers. Methods A total of 125 patients with chronic leg ulcers of more than four weeks'

Penetrating Injury to the Lower Limb by a Needlefish: A Case Report.

Needlefish are elongated marine fish known for their high-speed leaps toward light sources, occasionally resulting in penetrating injuries in humans. We report a case of a 42-year-old fisherman who presented with a penetrating injury to the left leg caused by a needlefish beak. Clinical examination revealed a small entry wound with a protruding foreign body, and imaging confirmed multiple retained fragments within the soft tissue. The patient underwent surgical exploration and removal of the foreign bodies under spinal anesthesia, followed by wound debridement and irrigation. Due to the risk o

Free vascularized tibial bone and nerve transfer: A spare-part reconstruction strategy for multiple extremity injuries.

We describe a novel reconstructive approach using a free vascularized tibial bone transfer with skin and tibial nerve components to salvage a complex upper extremity injury in the setting of a non-salvageable lower limb. This technique was applied in a patient who presented with severe blast injuries to the left upper arm and to the right upper and lower leg. Injuries of the right lower leg were so severe that reconstruction was deemed surgically not feasible. The harvested composite free flap included tibia, posterior tibial artery, tibial nerve and a skin paddle based on the posterior tibial

Reverse Sural Flap for Reconstruction of a Distal Leg Soft-Tissue Defect Following Dog Bite Injury: A Case Report.

Dog bites are a frequent cause of extremity trauma and pose a substantial risk of infection and complications, particularly when bone exposure is present. We present the case of a 58-year-old male with no comorbidities who arrived at the emergency department after being attacked by a dog while cycling, sustaining multiple bites to the left lower extremity and right forearm. On physical examination, the patient exhibited complex wounds measuring up to 10 × 5 cm, with muscular and tibial bone exposure in the left leg but no associated fracture, and additional forearm lesions with muscle exposure

Tennis Serve Biomechanics, Joint Load Mechanics and Overuse Injuries: A Narrative Review.

BACKGROUND: The tennis serve is a fundamental stroke in a player's performance during matches. But it may be also eminently traumatic as it causes very high joint mechanical loading which can lead to injury regardless of the player's experience, sex or age. This narrative review aims to synthesize current scientific knowledge on the key factors contributing to changes in joint mechanical loadings during the tennis serve and its potential association with overuse shoulder, elbow, wrist and lower back injuries. MAIN BODY: The databases PubMed, Google Scholar, ScienceDirect and SPORTDiscus were

Borrowing from the cancer playbook: a master protocol basket trial for hard-to-heal wound therapeutics targeting shared pathobiology.

OBJECTIVE: Hard-to-heal wounds, such as diabetic foot ulcers (DFUs) and venous leg ulcers (VLUs), remain associated with poor healing outcomes despite optimised standard care. Although classified by aetiology, these wounds share common pathophysiological features that may be amenable to mechanism-targeted therapies. This manuscript aims to describe the design, scientific rationale and statistical framework of a mechanism-driven basket trial evaluating wound therapies across multiple wound aetiologies. METHOD: This multicentre, randomised, sham-controlled clinical trial was designed under a ma

Healing outcomes after placental membrane-based skin substitute allografts in multiple wound types in routine care: a retrospective case series from a prospective observational wound registry.

BACKGROUND: Real-world data on cellular, acellular, and matrix-like products (CAMPs) remain limited, particularly in mixed-etiology cohorts. OBJECTIVE: To describe routine practice hard-to-heal wound healing outcomes among adults treated with placental membrane-based allografts. METHODS: This retrospective case series drew on a prospective observational wound registry. Eligible adults had at least 1 application of a placental-derived CAMP and 1 follow-up visit. One index wound per patient was selected for primary analyses; secondary summaries covered all wounds. The primary endpoint was comp

A Double Diagnosis Behind Chronic Ulcers: Livedoid Vasculopathy With Acroangiodermatitis in an Elderly Woman.

Chronic leg ulcers are a significant source of morbidity in elderly patients, particularly those with multiple comorbidities. The most common etiologies are vascular, including chronic venous insufficiency, peripheral arterial disease, and diabetic neuropathy. However, ulcers that are non-healing and refractory to standard treatment may reflect rarer underlying pathologies, warranting comprehensive reassessment. We report the case of an 87-year-old woman presenting with recurrent, painful, bilateral lower-limb ulcers, persisting for 18 months on the right leg and 10 months on the left. Her med

Delayed Presentation of a Retained Knife Fragment Following Penetrating Abdominal Trauma and Exploratory Laparotomy.

This case report describes a rare delayed presentation of a retained knife fragment following penetrating abdominal trauma and prior exploratory laparotomy. A 23-year-old male patient presented seven months after sustaining a stab wound with progressive left lower abdominal, buttock, and radicular leg pain, along with new-onset testicular discomfort. His initial injury required emergent laparotomy for multiple small bowel and mesenteric injuries. Despite an initially uncomplicated postoperative course, persistent and worsening symptoms prompted reevaluation. Computed tomography (CT) revealed a

Multifocal Necrotizing Fasciitis of the Bilateral Chest Wall and Contralateral Lower Limb in a Patient With Diabetes: A Case Report.

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening infection involving the fascia and subcutaneous tissues. Chest wall NF is rare and typically occurs following thoracic surgery or penetrating trauma. We report a 54-year-old man with type 2 diabetes mellitus who developed bilateral chest wall NF after sustaining a pectoralis major muscle injury while lifting a heavy engine part. Imaging demonstrated bilateral chest wall abscesses with mediastinal extension. During hospitalization, he also developed cellulitis of the contralateral lower limb, which subsequently progressed to

Flexor Digitorum Longus Strain in a Spinal Cord Injury Patient: A Case Report of a Rare Diagnosis.

Calf injuries are common, and they are often described in the gastrocnemius, soleus, or plantaris muscle. They are also documented in the flexor hallucis longus and the tibialis posterior, although less frequently. We present a case of an acute flexor digitorum longus muscle strain, a rare injury, often underreported and not well-documented in the literature. We describe the case of a 34-year-old active female undergoing rehabilitation for an incomplete tetraplegia, with suspected multiple sclerosis and left limb monoparesis. She sustained acute pain in her right leg during eccentric plantar f

Outcomes of Quadriceps and Patellar Tendon Repairs Augmented With Polyester Tapes.

INTRODUCTION: Rupture of the quadriceps tendon and patellar tendon (QT/PT) is an infrequent injury that displays a bimodal distribution, typically in low-energy injuries in elderly patients and sporting injuries in younger patients. Independent risk factors for QT/PT rupture include renal failure, obesity, steroid or antibiotic use, old age, and diabetes. Prompt repair restores the mobility of the extensor mechanism of the leg and failure can lead to significant morbidity. Augmenting QT/PT repair with polyester tapes could assist in improving the strength of the repair. The purpose of this stu

A Cognitive Dual Task Alters Dynamic Tibiofemoral Movements During Jump-Landing Assessments in Healthy Participants.

BACKGROUND: Many sports situations impose high cognitive demands on athletes, requiring them to divide their attention across multiple tasks and leading to landing mechanics associated with an increased anterior cruciate ligament (ACL) load and injury risk. PURPOSE: To investigate the influence of a cognitive dual task on dynamic tibiofemoral movements (ie, dynamic anterior tibial translation [ATTd] and dynamic internal tibial rotation [ITRd]) during jump-landing assessments in healthy participants. Atypical Presentation of Acute Compartment Syndrome in the Lower Limb: A Case Report of When Pain Does Not Guide the Diagnosis.

Acute compartment syndrome (ACS) is a surgical emergency that, if not promptly treated, can lead to muscle necrosis, limb loss, or death. Although disproportionate pain is traditionally considered the hallmark symptom, atypical presentations without pain have been described. We report the case of a 42-year-old male who sustained multiple traumatic injuries in a motorcycle-vehicle collision and subsequently developed silent compartment syndrome in the left thigh and leg. Despite being conscious and neurologically intact, the patient did not report significant pain at any point. The diagnosis wa
