The coverage of skin defects of the ankle and of the distal fourth of the leg remains a difficult problem despite progress in reconstructive surgery. The aim of our study was to establish an arterial map of the lateral head of the soleus muscle, to compare it with the existing data in order to investigate the possibility of raising a fibular artery-based pedicle island reverse flow flap. It has the theoretical advantage over its medial counterpart of sparing the main arterial axis of the leg and foot. Wounds of the distal third of the leg with exposed bone traditionally require free flaps for coverage. Although this often provides good results, patients with multiple comorbidities cannot undergo the long operating times and multiple surgical sites required for these complex procedures. We reviewed the use of posterior tibial (PT) perforator flaps as an alternative to free flaps for distal leg wound coverage in ill patients. Six patients (mean age, 53 years) with multiple comorbidities that precluded free-flap closures were treated with PT perforator flaps to cover complex distal leg wounds.

Provision of soft-tissue coverage for defects in the distal leg and foot is a challenge, especially in patients with multiple injuries and major soft-tissue defects. Major flap reconstruction requires expertise and the results are variable, with high morbidity. We report a case in which a reverse flow sural fasciocutaneous flap was used for treatment of an open fracture-dislocation of the right ankle after repeated debridement in a 64-year-old man with a history of chronic smoking, diabetes mellitus, and hypertension.

BACKGROUND: A 10-year review of temporary intravascular shunts (TIVS) at a regional trauma center.
METHODS: Retrospective chart review of all patients treated with temporary intravascular shunts from January 1, 1997 to January 1, 2007.

Problems of wound healing are commonly observed after coronary bypass grafting (CABG) operations.
