This case describes the successful treatment of a child with a vascular injury and two ipsilateral grade IIIB open lower leg fractures using two local-advancement soft-tissue techniques. Sixteen patients were studied between 1986 and 1992. Fourteen patients were reviewed. One had died and one was lost to follow-up. The average was 35 years. There were 15 men and one woman. UNLABELLED: 126 records were studied retrospectively. The average age was 31.8 years (15-80 years) with 81% men which corresponds to other reports in the literature. In this study a group of 104 multiply injured patients (mean ISS = 34) was examined for physical status and social reintegration. Essentially it was noted that 76 per cent of all patients were able to return back to work, although this appears to be age dependent. Open fractures of the leg seem to be the dominating prognostic factor for reintegration resulting in a disability rate of between 20 and 50 per cent. This study was undertaken to document whether a basic set of a new unilateral external fixation device could provide sufficient stability in complex musculoskeletal injuries involving upper and lower extremities and in patients with unstable pelvic ring disruptions. From 1977 to 1983, 173 patients suffering from chronic posttraumatic osteomyelitis were treated by local debridement and implantation of septopal beads; 102 patients were subsequently evaluated after follow-up periods of between 4 and 10 years. The optimal choice between operative and conservative therapy in the treatment of femur shaft fractures in school age children remains controversial. Statistical data for the period from 1985 to 1990 showed rise of primary invalidity due to traumas in most regions of the Ukraine. Taking into consideration the ratio of fractures in trauma structure we have analyzed invalidity on account of this kind of pathology. Closed, reamed, antegrade nailing remains the standard of care for femoral shaft fractures. When we evoked the indications of microsurgery in Traumatology, at first, we think reimplantations performed in emergency and also secondary management of soft tissues or complex defects. In fact, we resort to microsurgery in many other circumstances. Early microsurgical reconstruction of complex trauma of the extremities yield better results than delayed free flaps. A consecutive series of 119 type III B compound fractures in 103 patients was treated at the University of Louisville from May 1983 to May 1989.
