The authors report upon 773 fractures of the lower leg investigated from 1979 to 1989. They included 38 tibial fractures combined with fractures of the ankle joint. According to the mechanism of the fractures they were divided into: isolated tibial shaft fractures, transverse, segmental and communicated fractures and external rotation fractures of the tibial shaft combined with additional lesions of the ankle joint.

We describe a management strategy for upper- and lower-limb fractures with associated arterial injury and report the results in 113 cases treated over a period of 18 years. Primary amputation was performed in 23 patients and of those who underwent primary vascular repair, 27 needed secondary amputation, two-thirds of them within a week of the injury. Of those requiring secondary amputation, 51.8% had ischaemia exceeding six hours, 81.4% had severe soft-tissue injury and 85.2% had type III open fractures. Simultaneous bilateral femoral shaft fractures are unusual injuries caused by severe force and are usually associated with multiple injuries. There are few reports in the literature. From December 1986 to April 1989, there were 21 consecutive adult (greater than 16 years old) bilateral femoral shaft fracture cases treated at our hospital. The incidence was 2.0% (21/1,065). The associated multiple injuries (42 sites; 2.0 sites per case) markedly influenced the prognosis. In a retrospective study the surgically treated vascular injuries (n = 35) of the last 10 years were evaluated. One third of the patients had multiple trauma and in 57% of the cases vascular injury was accompanied by a fracture. The article summarizes 87 cases of external fixation in patients with gunshot fractures, open or closed. This method is fast and easy in application. It is indicated for treatment of concomitant and multiple injuries. The method has best results when applied just in the first days in patients with severe concomitant traumas.

We report a series of 20 children with 24 fractures that were treated using the AO tubular external fixator in a paediatric orthopaedic unit. There were no major complications related to fixator use. There were two cases of superficial pin track sepsis and one case of non-union. There were no cases of refracture after fixator removal or of leg length inequality.

We describe the results of treatment of open tibial fractures in 92 children; 22 fractures were Gustilo type I, 51 type II and 19 type III. Isolated or combined soft tissue defects of the lower leg are still a challenge as regard diagnostic and differential treatment. We distinguish acute and chronic isolated or combined soft tissue defect. Acute trauma within a multiple injury pattern is a special form of the acute soft tissue defect. For diagnostic purposes we use a preoperative and intraoperative standardized diagnostic programme. A new classification oriented towards the therapeutic procedure is presented, as is therapy algorithm which has been created.

The documentation relating to 63 surgically treated fractures sustained to the shaft of the tibia while epiphyseal growth plates were still in evidence (9 in isolation, 54 as fractures of the lower leg) between 1958 and 1983 was examined with respect to indications for the operation and the method selected. External fixation was used to treat complex femur fractures in 10 children. These injuries were associated with head trauma, cerebral palsy, epidermolysis bullosa, open wounds, and failed casting.
