An analysis of 11230 case histories of children aged to 15 who had fractures of the extremity bones has shown multiple fractures of the extremity bones in 3% of the patients. It was established that traumatic shock in children with multiple fractures of the extremities was diagnosed in 14,1% (I degree--in 51%, II--in 42,6% and III--in 6,4%). The article concerns the incidence and severity of shock in children of different age groups as well as fractures of bones of various character and localization.

The outcome of intramedullary nailing in 17 femoral fractures was studied in children aged 5 to 12 years. By this age independent vascularity of the capital femoral epiphysis has become established. Indications were: severely displaced diaphyseal fractures uncontrolled by closed reduction in patients with head injury or multiple trauma. Between Jan 1st 1978 and June 30th 1981 we treated 98 patients with multiple fractures of the lower extremities. Of a total of 260 fractures 86,2% were operated following the rule of primary definitive treatment. The postraumatic osteomyelitis rated 8,2%; the lethality was 16,3% due to other concomitant injuries of polytraumatized patients. The different patterns of fractures required individual operative-planning. Since in patients with multiple fractures of the lower limbs mobilisation is difficult, early operation to achieve training-stability proved to be valuable.

Operative fracture treatment is an important therapeutic measure in patients with cranial and other multiple injuries. If signs of reversible brain stem injury are present, the operative fixation of the long tubular bone fractures is indicated, whereas it contraindicated with irreversible damages to the brain stem. Nursing of the patients in coma is much more feasible after rigid internal fixation. Loss of consciousness alone is no contraindication of internal fixation.

Internal fixation of open fractures can be carried out according to the same indications employed in closed fractures in the presence of a type I wound. In type II and type III wounds early internal fixation may be indicated in victims of multiple trauma, in the elderly, in intra-articular fractures, in mutilated limbs, and in some fractures with associated vascular injuries. In a collective study of 22 surgical clinics 1235 closed diaphyseal tibial fractures in children from 0 to 14 years were analysed. The incidence of this type of fracture in relation to all fractures in childhood, was 6.2%. Two hundred consecutive patients admitted with 214 open fractures were reevaluated 6--10 years following injury. All patients were seen in follow-up by the authors. The average hospital stay was 36 days. Many of the patients sustained multiple injuries in automobile accidents. Twenty hip fractures in 19 children aged 2 1/2 to 18 were analyzed in a ten-year retrospective review at the University of Alabama Hospitals in Birmingham, Ala. There was one epiphyseal, one intertrochanteric, 4 subtrochanteric, and 14 intracapsular fractures. A series of 54 cases with multiple fractures of the tibia is reported. Sixty-eight per cent of the group were pedestrians hit by a car. Fifty-two per cent were open fractures. Osteosynthesis was performed in 33 cases, whereas 21 were treated conservatively.
