Full-Thickness Quadriceps Tendon Autograft for Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Quadriceps tendon autograft is used increasingly worldwide for primary anterior cruciate ligament (ACL) reconstruction, but remains at the third place after patellar tendon and hamstring grafts. It has shown very good results in biomechanical and clinical studies, but most of the authors describe a partial thickness tendon graft. INDICATIONS: This presentation describes the technique for a full-thickness quadriceps tendon autograft with an original technique of fixation on the femur and provides tips and tricks to avoid pitfalls. TECHNIQUE DESCRIPTION: Arthroscopic preparation of

Relationship to drill bit diameter and residual fracture resistance of the distal tibia.

BACKGROUND: The etiology of bone refractures after screw removal can be attributed to residual drill hole defects. This biomechanical study compared the torsional strength of bones containing various sized cortical drill defects in a tibia model. METHODS: Bicortical drill hole defects of 3 mm, 4 mm, and 5 mm diameters were tested in 26 composite tibias versus intact controls without a drill defect. Each tibia was secured in alignment with the rotational axis of a materials testing system and the proximal end rotated internally at a rate of 1 deg./s until mechanical failure. FINDINGS: All def

Human tibial cortical bone with high porosity in type 2 diabetes mellitus is accompanied by distinctive bone material properties.

Diabetes mellitus is a metabolic disease affecting bone tissue at different length-scales. Higher fracture risk in diabetic patients is difficult to detect with common clinical fracture risk assessment due to normal or high bone mineral density in diabetic patients. The observed higher fracture risk despite normal to high areal bone mineral density in diabetic patients points towards impaired bone material quality. Here, we analyze tibial bone from individuals with type 2 diabetes mellitus using a multiscale-approach, which includes clinical and laboratory-based bone quality measures. Tibial c

A novel murine model of combined insulin-dependent diabetes and chronic kidney disease has greater skeletal detriments than either disease individually.

Diabetes and chronic kidney disease (CKD) consistently rank among the top ten conditions in prevalence and mortality in the United States. Insulin-dependent diabetes (IDD) and CKD each increase the risk of skeletal fractures and fracture-related mortality. However, it remains unknown whether these conditions have interactive end-organ effects on the skeleton. We hypothesized that combining IDD and CKD in mice would cause structural and mechanical bone alterations that are more deleterious compared to the single disease states. Female C57BL6/J mice were divided into four groups: 1) N = 12 Contr

Transcriptome profile reveals novel candidate genes associated with bone strength in end-of-lay hens.

Bone weakness causes many problems such as osteoporosis, bone fractures, and economic loss, especially at the late stage of lay, in laying hen production. However, the genetic factors and molecular mechanism affecting the bone strength is still largely unknown. To elucidate the molecular mechanism and genetic factors affecting bone strength, a total of six cDNA libraries were constructed and used to compare genetic differences between tibia with higher(Group HBS)and lower(Group LBS)breaking strength in Hyline grey layers. A comparison between Groups HBS and LBS revealed nine differentially exp

A novel minimally invasive broken nail extractor for cannulated intramedullary nails: Trial and application in a few cases.

Successful and minimally invasive extraction of a broken distal end of an intramedullary nail is challenging. This study introduces a simple and reproducible technique for the extraction of broken cannulated intramedullary nails using a novel minimally invasive broken nail extractor. Five amputated adult lower-leg specimens were used to create models of the broken distal end of the cannulated intramedullary nails remaining in the medullary cavity of the distal tibia. Two orthopedic resident physicians with experience in tibial intramedullary nail implantation were selected to blindly extract t

[MATHEMATICAL MODELING OF OPTIONS FOR MOUNTING AN EXTERNAL FIXATION DEVICE ON THE TIBIA DURING ITS LENGTHENING].

The main problem in the treatment of axial deformities and limb shortening in pediatric orthopedics is the presence of incomplete growth of the patient, which gives an unfavorable basis for the achieved correction. The most advanced technology for the treatment of deformities and limb shortening is the use of external fixation devices (AVF). Target. To study the stress-strain state of the "tibia-AVF model with different variants of AVF mounting in conditions of tibia lengthening. Mathematical modeling of tibial osteosynthesis with external fixation devices in children with achondroplasia was c

Effect of Smoking on the Healing of Tibial Shaft Fractures in a Rural Indian Population.

Background Tibial shaft fractures account for 17% of all lower limb fractures. Nonunion and infection rates are estimated to be between 2% and 10%. Bone healing is a complex process that is influenced by biological, mechanical, and systemic factors. Adverse smoking effects on cardiovascular and respiratory systems have been well documented. An increasing interest in the effect of smoking on fracture healing following trauma has been noted in recent years. The biological consequence of smoking is relevant, especially in trauma surgery where no way of preventing presurgical smoking has been note

Outcomes after primary ankle arthrodesis in recent fractures of the distal end of the tibia in the elderly: a systematic review.

PURPOSES: Management of distal tibia fractures in the elderly is complex. The results of conservative treatments may be disappointing and primary ankle arthrodesis is now regularly offered as an alternative. In this study, we aimed to review the outcomes of primary ankle arthrodesis for distal tibia fracture in the elderly. METHODS: We conducted a systematic review of the literature, from the Cochrane, MEDLINE, and Embase databases, on studies published in English and in French between 1950 and 2020. Only studies reporting the clinical results, the function, or the complications of primary an

Relationship between Bone Quality, Egg Production and Eggshell Quality in Laying Hens at the End of an Extended Production Cycle (105 Weeks).

(1) Background: Nowadays the industry aims to improve lay persistency for extended cycles (100 weeks or longer) to make egg production more sustainable. However, intensive egg production challenges hen health, inducing severe osteoporosis and the incidence of bone fractures. In this study, the relationship between bone quality and egg production, and/or eggshell quality, was evaluated at the end of an extended laying cycle of 100 weeks, comparing groups of hens with different production and eggshell quality parameters; (2) Methods: Quality parameters of egg (as weight, egg white height), eggsh

Fracture behavior of human cortical bone: Role of advanced glycation end-products and microstructural features.

Diabetes is associated with increased fracture risk in human bone, especially in the elderly population. In the present study, we investigate how simulated advanced glycation end-products (AGEs) and materials heterogeneity affect crack growth trajectory in human cortical bone. We used a phase field fracture framework on 2D models of cortical microstructure created from human tibias to analyze crack propagation. The increased AGEs level results in a higher rate of crack formation. The simulations also indicate that the mismatch between the fracture properties (e.g., critical energy release rate

Interlocking Fixation in Fassier-Duval Rods: Performance and Success Factors.

INTRODUCTION: Pediatric patients with osteogenesis imperfecta (OI) can be treated with intramedullary Fassier-Duval rod (FDR) systems for limb deformity or recurrent fractures. Single-interlocking pins can improve epiphyseal fixation, but there is a paucity of literature examining incidence of rod migration or pin backout long-term. The purpose of this study is to quantify rates of rod migration and pin backout in OI patients treated with single-interlocking FDRs. METHODS: A retrospective chart review was performed on pediatric patients treated at a tertiary care center across a 15-year perio

Circadian rhythm disruption with high-fat diet impairs glycemic control and bone quality.

Biological functions, including glycemic control and bone metabolism, are highly influenced by the body's internal clock. Circadian rhythms are biological rhythms that run with a period close to 24 hours and receive input from environmental stimuli, such as the light/dark cycle. We investigated the effects of circadian rhythm disruption (CRD), through alteration of the light/dark schedule, on glycemic control and bone quality of mice. Ten-week-old male mice (C57/BL6, n = 48) were given a low-fat diet (LFD) or a high-fat diet (HFD) and kept on a dayshift or altered schedule (RSS3) for 22 weeks.

Limb Salvage after Lower-Leg Fracture and Popliteal Artery Transection-The Role of Vessel-First Strategy and Bone Fixation Using the Ilizarov External Fixator Device: A Case Report.

Open traumatic lesion of the popliteal artery is relatively rare. Ischemia time longer than 6 h and severity of limb ischemia have been shown to be associated with an increased risk of limb loss. Severe local infection is critical in the presence of major soft tissue trauma or open fractures. We report the case of a young female who suffered a traumatic transection of the popliteal artery associated with an open fracture of the distal tibia and fibula managed by direct vessel reconstruction with an end-to-end repair and skeletal stabilization initially with half-pin external fixation, then rep

Neglected Neck of Femur Fracture Managed by Surgical Hip Dislocation: A Case Report and Literature Review.

INTRODUCTION: Talus fracture is an uncommon fracture that can be encountered on day- to- day basis. However, it is the 2nd most common tarsal bone to get fractured after calcaneum and accounts for approximately 1% of all fractures around foot and ankle. The anastomotic ring around the talar neck is highly likely to get damaged at the time of the fracture, which, in turn, hampers the blood supply to the body of talus. As a result, the bone healing is delayed and the integrity of the healed fracture is poor which leads to poor functional outcome. Almost 39% cases are missed during the initial ev

Clinical results and complication rates of lower limb lengthening in paediatric patients using the PRECICE 2 intramedullary magnetic nail: a multicentre study.

Implantable intramedullary nail lengthening devices (e.g., PRECICE 2 system) have been proposed as alternative method to external fixation for lower limb lengthening surgery. The aim of this study was to analyse our outcomes and complications using the PRECICE 2 (P2) nail system and review them in light of the existing literature. A retrospective multicentre study was conducted on patients <18 years, who were treated for limb lengthening using the P2 system. The inclusion criteria were a limb length discrepancy ≥ 30 mm and a follow-up ≥6 months after the end of treatment. A total of 26 (15 mal
