Untangling the coxa-trochanter joint appendotomy in spiders (Arachnida: Araneae).

The ability to discard body parts at a preferential locus of weakness is widespread across the Metazoans' Tree of Life and has received attention from ecological and behavioral perspectives. Still, in many cases, a clear understanding of its genetic, morphological, and anatomical pathways is lacking. Distinct terms (autotomy, autotilly, autospasy) have been used in the literature to describe the process of limb detachment, usually carrying in their meaning information regarding the mechanics involved in it, and appendotomy is the most general word to encompass all of them. Appendotomy is found

Diagnostic yield of multiple X-ray imaging requests of lower limb extremities.

OBJECTIVES: The diagnosis pathway for patients presenting in the emergency department with lower limb trauma injuries requires adequate medical imaging of the regions of interest. Due to concerns of concurrent injury or overlapping symptoms, X-rays of multiple lower extremity regions may be requested simultaneously. This article explores the diagnostic yield of imaging multiple lower limb regions following trauma. METHODS: A retrospective audit of lower limb imaging requests for patients older than 18 years from 2 hospitals was conducted. Region requested, clinical history, and radiology find

Assessment of differences in mechanical properties and thickness of temporal and occipital bones and correlation with age in Japanese forensic samples.

The mechanical properties and thickness of adult temporal and occipital bones were examined in modern Japanese forensic samples. Cranial bones were obtained from 293 Japanese corpses (179 men and 114 women). During autopsy, left temporal (LT), right temporal (RT), and occipital (O) bone samples were extracted from each skull. Sample thickness (ST) was measured using multidetector computed tomographic imaging. The fracture load (FL) of each sample was measured by a bending test, in which the flexural strength (FS) was calculated. The FL and ST values for O were significantly greater compared wi

Forensic analysis of fabricated long bone fractures for gain.

BACKGROUND: Self-inflicting injury is a recognised psychiatric disorder. Such cases are regularly seen in the emergency department, with a false history of assault. Most of these are easily detected by a forensic medicine expert as they present as described in literature but are difficult to explain if altered by professionals for nefarious reasons. METHOD: This was a prospective study carried out at a tertiary centre. The data was collected and analysed from the office records of medico-legal reports prepared at our institute between 25 February 2022 and 25 February 2023. RESULTS: 21 cases

Decapitation of an agricultural worker by a combine harvester: a case report.

BACKGROUND: Each year, there are billions of agricultural work accidents involving the operation of tractors, grain augers, harvest combines, power take-off devices, or balers and thrashers. Field accidents of this nature seem more common on afternoons, just as road accidents tend to skew toward nighttime. The lesions can vary widely and depends strictly on the operation of the machinery analyzed. AIMS: This paper aims to present a peculiar case of decapitation by a combine harvester, showing how, in cases of injury due to agricultural machinery, it is fundamental a correct execution of a sce

Changing epidemiology of lower extremity fractures in adults over a 15-year period - a National Hospital Discharge Registry study.

BACKGROUND: Demographic changes led to an increasingly ageing population in Germany and thus to possible changes in the frequency of fractures. The primary aim of this study was to report changes in fracture rates of the lower extremities in Germany in 2002 compared to 2017 and to evaluate those changes. METHODS: Inpatient data from the German National Hospital Discharge Registry (ICD10) for 2002 and 2017 were evaluated. Changes in total counts and incidence rates were analysed for fractures in the following locations: femoral neck, pertrochanteric, subtrochanteric, distal femur, femoral shaf

Partial Calcaneus Reconstruction using 'Opportunistic Grafts': A Case Report.

Although replantation of an amputated extremity has been successfully performed, occasionally the surgeon is forced to amputate in the case of an unsalvageable open fracture. These appendages can theoretically act as an autologous 'bone and tissue bank' for patients if they have reconstructable contralateral injuries. We present a case of an athlete who sustained a Gustilo and Anderson IIIC injury on 1 leg resulting in a below knee amputation. Her other lower leg had a traumatic amputation of the heel pad, partial amputation of the calcaneus, and complete Achilles tendon loss. Tissues from thi

A combination of bimalleolar fracture and fracture on talar body and neck: A rare case report.

RATIONALE: Talar fracture accompanied with malleolar fracture is rare, and its management is complex. Ankle soft tissue is much thinner than other parts of the human body, and the shape of the ankle makes wounds difficult to close immediately after surgery, which may result in poor skin condition if the wound tension is too high. However, joint congruity and osteonecrosis are the main concerns of talar fracture. PATIENT CONCERNS: A 57-year-old man presented at the emergency department following a motorcycle accident. DIAGNOSES: Physical examination revealed swelling and tenderness of the lef

Multifunctional mandibles of ants: Variation in gripping behavior facilitated by specific microstructures and kinematics.

The elongated mandibles of certain ant species are dexterous grippers that can output a wide range of forces as needed for various tasks. Our combined experimental and theoretical research reveals the multifunctionality of the mandibles of Harpegnathos venator that is facilitated by specific microstructures and characteristic kinematics. First, we found that H. venator can pull off a spider's (Heteropoda venatoria) leg by closing its long mandibles. We observed that the ant usually clamps the spider's leg using the distal or middle part of its mandibles. In contrast, the ant can grip its egg w

A case of Jack Russel Terrier scavenging: Images and three dimensional models for forensic interpretation.

Distinguishing between ante-mortem and post-mortem domestic dog injuries is fundamental in order to correctly find the cause of the death. Although fatal aggressions are chiefly perpetrated by pit bull-type dogs, small dogs are responsible for a significant share of animal bites and can cause high-degree injuries. On the other side, indoor and outdoor scavenging can hinder forensic investigation, consuming parts of the body where crucial forensic features like bruises and wounds have been inflicted. In the case we present, a man died after falling down his house stairs because of a traumatic i

Possible factors influencing the duration of hospital stay in patients with psychiatric disorders attempting suicide by jumping.

BACKGROUND: Patients with psychiatric disorders have a high rate of suicide. The present study investigated factors influencing hospital stays for Japanese patients with psychiatric disorders attempting suicide by jumping. METHODS: We diagnosed all suicide attempts (n = 113) by jumping based on the International Classification of Diseases 10th Revision (ICD-10) and investigated the mean hospital stays of patients with each diagnosis based on the ICD-10 code. We then analyzed differences in the demographic and clinical characteristics between the diagnostic groups to identify factors influenci

[Limb salvage or amputation after severe trauma to the lower extremities : Evidence from the LEAP Study].

Deciding between reconstruction and primary amputation after severe high-energy trauma to the lower extremities is difficult and consequential. The Lower Extremity Assessment Project (LEAP) prospectively included and investigated patients with severe, limb-threatening injuries below the femur, with third-grade open fractures, defined soft-tissue damage and amputation wounds. This paper aims to review the key results of the LEAP study, which were published in several parts, in due consideration of the newer relevant literature, and to deduce the consequences for clinical practice. The main resu

Assessment of Hip Fracture Risk Using Cross-Section Strain Energy Determined by QCT-Based Finite Element Modeling.

Accurate assessment of hip fracture risk is very important to prevent hip fracture and to monitor the effect of a treatment. A subject-specific QCT-based finite element model was constructed to assess hip fracture risk at the critical locations of femur during the single-leg stance and the sideways fall. The aim of this study was to improve the prediction of hip fracture risk by introducing a novel failure criterion to more accurately describe bone failure mechanism. Hip fracture risk index was defined using cross-section strain energy, which is able to integrate information of stresses, strai
