Morphological Changes in Femoral Trochlea After Extensor Realignment Surgery in Children With Congenital Patellar Dislocation.

BACKGROUND: Femoral trochlear dysplasia (FTD) is a major factor affecting the stability of the knee joint. Congenital patellar dislocation (CPD) is usually accompanied by distal FTD. Few studies have investigated trochlear development following surgery in skeletally immature patients. PURPOSE: To observe trochlear remodeling following surgical extensor realignment and stabilization in children with CPD and to analyze the factors associated with this remodeling. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A retrospective analysis was conducted on surgical cases of CPD treated a

Suture-Based Technique for Patellar Osteochondral Fractures Fixation Using an All-Absorbable Implant.

The osteochondral fractures (OCFs) of the patella are most common injuries of adolescence, usually associated with acute dislocation of patella. Although the acute cases are managed with a variety of options ranging from either conservative or removal or refixation, the latter seems to be the best option. While fixation using screws or bioabsorbable pins has been a standard practice, complications remain a concern. The use of suture fixation for treatment of such low-profile OCFs has gained popularity in recent times. However, each author has used the sutures in a random manner without a prede

4in1 Procedure in Treating Congenital Dislocation of Patella in Children.

OBJECTIVES: Congenital dislocation of patella (CDP) is a rare condition and surgery is needed to treat CDP once a diagnosis is made. The 4in1 procedure includes lateral release, correction of quadriceps, Roux-Goldthwait procedure, and medial patello-femoral ligament (MPFL) reconstruction. This study was aimed at evaluating the efficacy and outcome of the 4in1 procedure in the operative treatment of CDP. METHODS: CDP patients were retrospectively collected from two operative centers from January 2013 to December 2024. For all patients, patellae on both sides were examined by x-ray, CT, and MR

Femoral Shortening Osteotomy in Managing Congenital Patellar Dislocation: A Case Report and Review of Literature.

INTRODUCTION: Periprosthetic humeral fractures are rare but serious complications following shoulder arthroplasty, especially in elderly patients with osteoporosis. These fractures pose a significant management challenge due to compromised bone quality and the presence of prosthetic components. Salvage procedures such as vascularized fibular grafting offer a biologically and mechanically favorable option in complex cases with failed internal fixation. CASE REPORT: We present the case of an 80-year-old female with systemic hypertension and osteoporosis, who sustained a periprosthetic fracture

Total Knee Arthroplasty for Neglected Habitual Dislocation of Patella with Knee Osteoarthritis: A Case Report.

CASE: We performed total knee arthroplasty (TKA) on a 62-year-old male patient who presented with symptomatic knee osteoarthritis resulting from habitual dislocation of the patella. The patient had a painless unassisted gait with stable patella and 5 to 95° knee range of motion, 23 months after surgery. CONCLUSION: TKA can be an effective treatment option for geriatric patients with habitual patellar instability and concomitant knee osteoarthritis, but may require surgical modifications based on individual pathoanatomy.

Hip Pain after Distal Femoral Derotational Osteotomy for a Patient with Patellar Instability: A Case Report.

BACKGROUND: Increased torsional deformity of femur is one of the most important factors in recurrent patellofemoral instability. And distal femoral derotational osteotomy is an effective treatment option in patients with increased femoral torsional deformity. However, groin pain is a rare complication after femoral derotational osteotomy. CASE PRESENTATION: The patient was a 23-year-old female suffering from anterior knee pain and patellar instability in her left leg. Continuous CT scans of the hip, knee, and ankle were performed and the femoral anteversion was 44.99°. Considering the signifi

Soft Tissue Lengthening for Flexion Dislocation of Patella.

PURPOSE OF REVIEW: Obligatory dislocation of the patella (also known as habitual dislocation) is a rare subset of patellofemoral instability in which the patella dislocates every time the knee is flexed. The problem arises due to contracture of the quadriceps muscles. Soft tissue lengthening procedures such as quadriceps tendon lengthening are the mainstay of treatment, in contrast to medial patellofemoral reconstruction (MPFL-R) for the more common recurrent lateral patellar dislocation. The current review explores the existing literature surrounding the pathophysiology and treatment strategi

BACKGROUND: Infections caused by extended-spectrum beta-lactamases producing Enterobacteriaceae (ESBL-E) are of major concern in clinical practice because of limited therapeutic options effective to treat them. Published studies showed that ESBL-E, widely spread in Europe, United States or Asia; are also frequent in Africa. However, the impact of ESBL-E infections is yet to be adequately determined in Sub-Saharan African countries, particularly in Senegal. The aim of our study was to estimate the incidence rate of ESBL-E infections and to assess their clinical and economic impact in Senegal.

4-in-1 Quadricepsplasty for Fixed and Habitual Dislocation of Patella.

Fixed and habitual dislocations of patella represent the most severe forms of patellar instability and frequently require surgical intervention. Isolated medial patellofemoral ligament (MPFL) reconstruction, which has been the cornerstone of treatment for episodic patellar dislocation, is inadequate to address such complex instability patterns. The purpose of this report is to describe the 4-in-1 quadricepsplasty technique for stabilization of fixed and habitual dislocation of patella. The 4 components of quadricepsplasty are lateral retinacular releases and lengthening, Roux-Goldthwait patell

Baksi procedure for habitual dislocation of patella in children - Revisiting the key surgical steps.

The pes anserinus transfer procedure for habitual dislocation of patella described by Baksi is a biomechanically sound technique with predictable long term results. The dynamic pes anserinus sling counteracts the lateral quadriceps contracture and keeps patella relocated till the vastus medialis activity is restored and takes over. The procedure is especially suitable for children with open physeal growth plates. Moreover, the surgery runs a low complication rate. In the present review, we recapitulated the steps of the procedure to illustrate its surgical basics. We also describe our experien

An unusual case of traumatic medial eversion-dislocation of the patella: a case report.

Medial dislocation of patella has been commonly reported as a complication of the surgical release of the lateral patella-femoral ligament. A traumatic medial eversion-dislocation of the patella has not been earlier reported. We report an unusual case of traumatic medial patellar eversion-dislocation in a young adult patient whose magnetic resonance imaging (MRI) showed a discontinuity in medial and lateral patellar retinacula. However, intraoperatively, the retinacula were found intact, and relocation of the patella was achieved by flipping the patella through a small hole in the patellar ten

Neglected Locked Patellar Dislocation in an Adult Female: Report of a Rare Case.

INTRODUCTION: Dislocation of patella is a very common injury which usually reduces spontaneously or can be reduced easily using gentle manipulation. Irreducible patellar dislocations are rare and usually result due to either rotation of patella along the horizontal or vertical axis or due to bony impaction. Neglected locked patellar dislocations are extremely rare injuries presenting additional challenges. CASE REPORT: We are reporting a case of a 24-year female who presented to us 4 months after suffering a knee injury for which she received native treatment initially. On presentation, patie

Intra-articular horizontal dislocation of the patella: a rare injury and review of the literature.

Acute traumatic intra-articular dislocation of the patella is not a common presentation in orthopaedic practice; less frequently observed than extra-articular dislocation of the patella. In some of these cases, closed reduction is not possible and an open reduction in the operating theatre must be performed. In this case report, we present an elderly patient with an intra-articular horizontal dislocation of the patella without any other bony or ligamentous lesions seen in a postreduction MRI. We conducted a literature review looking at intra-articular patella dislocations, identifying 64 publi

Intra-articular dislocation of patella with femoral impaction-A case report and review of literature.

INTRODUCTION AND CASE PRESENTATION: Acute traumatic patella dislocations are common injuries seen in emergency departments. We present a rare injury which involves an acute, irreducible intra-articular dislocation of the patella. In this particular case, we describe a horizontal type intra-articular dislocation of the patella in a 66-year-old lady, which was complicated by impaction into the distal femur. She underwent two failed closed reduction attempts, but subsequently reduced spontaneously on the orthopaedic floor as she was waiting for an open reduction in the operating room. DISCUSSION

Superior Dislocation of Patella - A RareClinicalEntity.

INTRODUCTION: Superior dislocation of patella is a rare injury. Patient usually presents with painful locking of the knee. The authors describe here a case of 54-year-male, who bumped his right knee on the edge of the bed. The patient was unable to extend his right knee. This was diagnosed as superior dislocation of patella and then managed with closed reduction with intramuscular analgesia. CASE REPORT: A 54-year-old male presented with acute painful locking of knee after his knee struck on the edge of the bed while trying to attain a kneeling position on the bed. On clinical examination, th
