Meniscal repair using the outside-to-inside technique.

Research during the past decade has elucidated the structure and function of the knee joint meniscus, and both clinical and experimental studies have demonstrated the importance of this structure. Recent advances, such as the use of an exogenous fibrin clot, have allowed preservation of an increasingly greater proportion of injured menisci. The success of these methods will be established only by long-term follow-up studies demonstrating a lower incidence of progression to degenerative joint disease. It is hoped that an increasing understanding meniscal fibrochondrocytes biology and response t

Technical considerations in the management of complex meniscus tears.

Meniscal repair continues to be an important procedure in patients with meniscal tears. The orthopedic literature has recommended excision of complex tears or tears that are located in the central avascular region. For the past 13 years, the authors have repaired all meniscal tears that can be reduced anatomically and that have healthy tissue that will support the multiple sutures required for a stable repair, regardless of tear pattern or location. Correct patient selection criteria continue to be important. In all cases, the risks of complications and retear must be considered carefully and

MR imaging before arthroscopy in knee joint disorders?

PURPOSE: To review the current literature examining the relative merits of arthroscopy and MR imaging of the knee. MATERIAL AND METHODS: All papers comparing MR imaging with arthroscopy published within the last 10 years according to Medline were collected and read. RESULTS: Technology has improved considerably during recent years allowing detailed non-invasive visualization of the knee. In particular, the development of cheaper whole-body and dedicated low-field MR units has opened up for non-invasive inspection of the knee at reasonable cost. Meniscal tears can be detected with accuracy ra

Meniscal repair. An experimental study in the goat.

To evaluate the healing potential of the meniscus, we created a longitudinal medial meniscal tear in both knees of 24 adult goats (48 knees). In all goats, the meniscus was repaired in one knee and left unrepaired in the other. The goats were placed into one of three groups based on the location of the tear and the status of the anterior cruciate ligament. At 6 months' followup, an arthrogram was obtained and the menisci were examined. In knees with an intact anterior cruciate ligament, tears in the peripheral 20% to 25% of the meniscus healed in six of seven repaired menisci, but in only one

Bilateral discoid medial menisci with extensive intrasubstance cleavage tears: MRI and arthroscopic correlation.

Although rare, the discoid meniscus can become problematic in active individuals. The arthroscopist is then presented with a challenging treatment problem. Through the current techniques of arthroscopy and MRI, the diagnostic and treatment difficulties are lessened; however, our understanding of the discoid meniscus is incomplete. Currently, the need exists for correlation between arthroscopy and MRI. We present a case of bilateral complete discoid medial menisci with symptomatic extensive intrasubstance horizontal cleavage tears. Both arthroscopy and MRI views of the lesions, pre- and postope

Principles and decision making in meniscal surgery.

This article provides a review of the basic science and clinical information available to the orthopedist on which a systematic approach to meniscal surgery can be based. Attitudes toward the meniscus have changed dramatically in the last 50 years. Laboratory investigations show that the menisci participate in many important functions, including tibiofemoral load transmission, shock absorption, lubrication, and passive stabilization of the knee joint. Histologic/structural analyses reveal the menisci to be annular structures, with the ability to transmit and properly distribute load over the t

[Rupture of the anterior cruciate ligament. Current status of treatment].

This article summarizes the present knowledge on the diagnosis of and treatment rationales for ruptures of the anterior cruciate ligament (ACL) of the knee. There is an increasing incidence of this injury due to the high number of persons involved in dynamic sports. The most significant diagnostic criterion is a positive pivot shift associated with a pathological anterior translation of the tibia in slight flexion of the knee. Instrumented testing of the knee is becoming increasing important and is standard in follow-up studies. A survey of the literature at present delineates very clearly the

[Current developments in ultrasonography of the meniscus].

Sonography of the meniscus is a new diagnostic method, the instructive value and clinical relevance of which are subject to controversy. Sonographic assessment of the meniscus requires the use of focusing that can cope with the different anatomical conditions of the individual parts of the meniscus and can largely eliminate artifacts by means of wide sound-wave propagation. The dynamic examination technique allows the identification of both longitudinal and flap tears, which are relatively easy to diagnose, and of tear formations that are harder to visualize, such as horizontal and transversal

[Partial lesions of the posterior cruciate ligament of the knee: CT findings].

Posterior cruciate ligament (PCL) tears are more common than usually reported, especially in athletes. In the acute phase, PCL tears are particularly difficult to diagnose at clinical examination and may go unnoticed even to a skilled clinician because of concomitant lesions to other structures, of hematrhos or of strong muscular defense. The PCL is responsible for 95% of biomechanical knee stability and therefore plays a major role in the maintenance of joint congruence. In the long run, PCL tears cause posterolateral instability and an untimely development of degenerative changes, particular

Current status of meniscus salvage.

The direct repair of meniscus tears with rasp preparation of all tear surfaces, stable suture fixation, and exogenous clot injection is effective for single longitudinal tears with peripheral white rims of 4 mm and less. Radial split and flap tears at the posterior horn of the lateral meniscus can be directly repaired as well. Single longitudinal tears typically in chronic knees with peripheral white rims of 5 mm and greater may have better reliability with use of the fascia sheath. The sheath is indicated in complex tears including flaps and radial splits. A structured rehabilitation program

Double-blind assessment of the value of magnetic resonance imaging in the diagnosis of anterior cruciate and meniscal lesions.

A prospective double-blind study was undertaken to evaluate the usefulness of magnetic resonance imaging in the accurate interpretation of pathological intra-articular changes in the knee. Forty-seven patients who were scheduled to have arthroscopy and three patients who wer to have arthrotomy volunteered for magnetic resonance imaging preoperatively. The radiologists had no clinical or roentgenographic information about the patients before the evaluation of the magnetic resonance images, and the radiologists' interpretations were unknown to the surgeon before the arthroscopy or arthrotomy was

A simple method of meniscus repair.

Current understanding of knee meniscus pathology and the known consequences of meniscal loss dictate preservation of tissues wherever feasible. Surgical approaches to meniscal repair include open and combined arthroscopic-open or arthroscopic-percutaneous procedures. A variety of needles, guides, sutures, and retractors have been marketed. Over a 2 year period, we used these techniques in the repair of 19 medial and 5 lateral meniscus tears, with no known retears and no complications. We used an 18-gauge, 6-inch spinal needle and a strand of number one nonabsorbable suture.

[Arthroscopic surgery--current status and perspectives].

The morbidity after arthroscopic surgery is low; a hospitalization is not necessary in most patients. The possible operations of the knee joint include removal of loose bodies, resections on meniscus, plicae and synovium as well as more complicated procedures as suturing of a meniscus, total synovectomy and operations in patients with osteoarthritis. The advantage of the arthroscopic operation compared with arthrotomy is well documented in meniscal resection (shorter treatment, stay in the hospital and sick leave, reduced costs, and nevertheless excellent results) whereas the indication and cl

Arthroscopic meniscal repair with two-year follow-up: a clinical review.

The potential for healing of meniscal tissue has been historically underappreciated, but is currently more widely acknowledged. We have reviewed our experience with arthroscopic meniscal repair in 29 patients who had had a minimum of 2 years' follow-up. Between September 1983 and November 1986, 31 patients who had undergone arthroscopic meniscal repair with a minimum of 2-years' follow-up were identified. Of the 31 patients, 29 were available for additional follow-up. The patient population averaged 31 years of age, with 15 men and 14 women. Utilizing a closed arthroscopic cannulated technique

Magnetic resonance imaging of the knee.

Magnetic resonance imaging (MRI) is an accepted non-invasive modality for evaluation of soft tissue pathology without exposure to ionizing radiation. Current applications demonstrate excellent visualization of the anatomy and pathology of various organs. Preliminary studies in the knee reveal fine resolution of anatomy and pathology involving the meniscus. The purpose of this study is to determine a prospective correlation between MRI scans and actual meniscal pathology as documented at the time of arthroscopy. MRI scans were obtained in 155 patients, on 156 knees (one patient with bilateral s

Arthroscopic meniscal repair.

Arthroscopic meniscal repair is a promising new technique for treating peripheral meniscal tears. With proper attention to surgical technique, the repair can be safely performed by any surgeon with moderate arthroscopic skills. We recommend that in all cases the capsule be exposed through a small incision to identify and protect the neurovascular structures. When an associated ACL tear is present, ligamentous reconstruction is also recommended. Although we currently limit our repairs to peripheral tears, newer techniques may expand indications for this procedure.

Meniscal tears: MR demonstration of experimentally produced injuries.

Magnetic resonance (MR) imaging was performed using a 1.5-T magnet on eight immature swine knees with surgically produced vertical and horizontal meniscal tears. Three radiologists, interpreting the images independently, detected all tears. Tears in the middle portion of the meniscus were best seen on sagittal views, while lesions of the anterior and posterior horns of the meniscus were best seen on coronal views. MR has several advantages over arthrography, the current diagnostic method for knee injuries: it is noninvasive, uses no ionizing radiation, and provides excellent soft-tissue contra

Current concepts of meniscal function and pathology.

The advantages of preserving the menisci are reduced degenerative changes and improved stability. At long-term follow-up, even arthroscopic partial meniscectomy in the anterior cruciate ligament-deficient knee results in more than twice the incidence of Fairbank's changes compared with a normal meniscus, stable meniscus tears left alone, or meniscus repair.

Function, post-traumatic sequelae and current concepts of management of knee meniscus injuries: a review article.

Long thought to be expendable or insignificant for function, the menisci may well be very important structures in the knee joint. Menisci are of critical importance from the standpoint of stability, shock absorption, load transmission, and lubrication, and also play a role in preventing synovial impingement, and in defining the endpoints of flexion and extension. In the wake of meniscectomy, certain predictable time-related events afflict the joint, i.e., instability and degenerative arthrosis. These eventualities are less pronounced following partial resection, and lend credence and support t
