A novel meniscal root refixation pull-in technique with an all-suture anchor shows biomechanical properties comparable to standard suture anchor and transtibial pull-out techniques.

PURPOSE: Repair techniques for posterior meniscal root (PMMR) tears include repair with transtibial pull-out and anchors. An alternative approach uses all-suture anchors pulled in, avoiding a posterior medial portal. While clinical feasibility has been assessed, biomechanical properties of this technique remain unknown. We hypothesised that the biomechanical properties using the pull-in technique would be comparable to those achieved with conventional repair techniques. METHODS: Fifty fresh-frozen porcine tibiae were fixed in a steel pot. Whereas in group (1) the native meniscal root was kept

Management of Isolated Tibial Plateau Defect in the Setting of Anterior Cruciate Ligament Reconstruction and Meniscus Repair: A Case Report and Review of the Literature.

Purpose: Our work describes the use of matrix-induced autologous chondrocyte implantation (MACI) for a lateral tibial plateau osteochondral defect, in the setting of a concomitant meniscus repair and anterior cruciate ligament (ACL) reconstruction. To our knowledge, there is minimal research describing the application of MACI for cartilage defects of the tibial plateau, which brings about its own challenges. Methods: A 26-year-old male presented with lateral right knee pain and instability following a soccer injury. A bucket-handle lateral meniscus tear, 2 cm by 2 cm osteochondral defect of th

Partial Meniscectomy or Physical Therapy in Degenerative Meniscus Tears: A Retrospective Cohort Study with 2-Year Follow-Up.

Degenerative meniscal injuries are a common occurrence in orthopedic practice. However, there is currently no consensus regarding the optimal treatment algorithm and the efficacy of different treatment modalities. Therefore, this study aimed to analyze the clinical reflections of arthroscopic partial meniscectomy (APM) and physical therapy (PT) methods, as well as the potential development of osteoarthritis (OA) following treatment. The study group comprised patients diagnosed with degenerative meniscal tears who were treated with either conservative or APM methods at a center between March 20

Treatment of Failed Lateral Meniscal Oblique Radial Tear Repair With Segmental Allograft Meniscus Transplantation.

Surgical techniques that preserve the native meniscus have been investigated in the setting of irreparable meniscal tears, including meniscal allograft transplantation (MAT) and meniscal synthetic scaffolds. Complete MAT removes the entirety of the native meniscal tissue for transplantation with suboptimal long-term survivorship. Similarly, synthetic meniscal scaffolds have shown variable success rates with an inability to regenerate functional tissue or prevent early osteoarthritis. Therefore, segmental MAT techniques have been explored to preserve the healthy, native meniscal biology and dec

Key insights and implications of cartilage degradation in osteoarthritis.

Progressive degradation of articular cartilage is characteristic of osteoarthritis (OA), but OA is more than a wear-and-tear disease of the cartilage. It is a complex, multifactorial disease affecting all joint tissues, amplified by local and systemic inflammation. Chondrocytes play a crucial role in cartilage homeostasis and various molecular pathways that leading to their catabolic state have been identified. Cartilage degradation fragments and direct exposure of chondrocytes to extracellular matrix molecules provide feedback loops that further stimulate the catabolic profile. Synovial infla

Needle-Assisted All-Inside Medial Meniscal Ramp Lesion Repair.

Multiple techniques have been described to address ramp lesions. Suture hook repair from the posteromedial portal is currently considered the gold standard but can be technically demanding, especially for larger lesions extending into the medial portion of the meniscus. In this technical note, an anatomic repair technique for both the meniscocapsular and meniscotibial ligaments is presented. It requires a bent spinal needle introduced through the posteromedial portal for tear reduction and standard all-inside suture devices introduced from the anteromedial portal.

Multi-loop Balanced Continuous Suture for the Treatment of Radial Tears in the Body of the Meniscus.

Radial tears in the body of the meniscus occur quite often in clinical practice. Because of the concentration of stress, lack of resistance to external forces, and poor blood supply in this area, healing is more difficult compared with other types of tears. As a result, some physicians may perform meniscectomy, which can lead to rapid joint degeneration and osteoarthritis. Currently, there are numerous methods for suturing this type of tear. According to the direction of suturing, they are divided into outside-in, all-inside, and inside-out techniques. In terms of suturing methods, there are h

Decellularisation of human meniscus tissue using sodium dodecyl sulphate (SDS): Preserving biomechanical integrity for scaffold-based meniscal repair.

PURPOSE: Meniscal tears are common knee injuries and a major risk factor for secondary osteoarthritis. Recently, there has been a paradigm shift toward meniscal preservation, reflecting the meniscus's vital role. In this context, tissue engineering approaches such as the development of meniscal scaffolds have gained attention. However, to reduce the immune response and improve biocompatibility, decellularization of allografts, while preserving the histoarchitectural and meniscal properties, is essential. The current study aimed to evaluate the effectiveness of decellularization and its impact

MV2SwimNet: A lightweight transformer-based hybrid model for knee meniscus tears detection.

Knee Ailments, such as meniscus injuries, bother millions globally, with research showing that more than 14% of the population above 40 years lives with meniscus-related conditions. Conventional diagnosis techniques, like manual MRI interpretation, are labour-intensive, error-prone, and dependent on skilled radiologists, making an automatic and more accurate alternative indispensable. Current deep-learning solutions heavily depend on CNNs, which perform poorly in long-range dependencies and global contextual info. We proposed MV2SwimNet, a hybrid of MobileNetV2 and Swin Transformer, integratin

Anatomical Tibial Tunnel Vector in Pull-Out Repair for a Medial Meniscal Posterior Root Tear (MMPRT) and Its Clinical Potential.

Introduction This study focuses on the tibial tunnel in the pull-out repair of medial meniscus posterior root tears (MMPRTs), with attention to the distal hole as the anatomical vector. Little research has explored this aspect, as previous studies have mainly emphasized the proximal hole as the attachment point. Methods The anatomical vector was identified by analyzing magnetic resonance images of 84 healthy knees. Clinical nine cases that underwent pull-out repair using the anatomical tibial tunnel (AT) with open wedge high tibial osteotomy (OWHTO) were evaluated to investigate the interferen

Evaluating the diagnostic value of ultrasound in meniscal injury detection: current evidence and future directions.

PURPOSE: Meniscal injuries are a common cause of knee dysfunction and healthcare utilization, with magnetic resonance imaging (MRI) being the diagnostic gold standard. However, MRI's cost, limited accessibility, and contraindications of MRI have prompted interest in ultrasound (US) as a more affordable, portable, and radiation-free alternative. This review aimed to synthesize the current evidence on the diagnostic accuracy of US for meniscal tears and to define its role alongside MRI and arthroscopy in clinical practice. METHODS: We conducted a systematic review following the PRISMA guideline

The management of lateral meniscus pathology does not affect anterior cruciate ligament revision rates in football players, but affect rates in other athletes: Long-term data from the Swedish Knee Ligament Registry.

PURPOSE: To investigate whether the treatment method of a concomitant lateral meniscus (LM) injury during primary anterior cruciate ligament (ACL) reconstruction (ACLR) affects ACL revision rates in football (soccer) players, nonfootball pivoting athletes and nonpivoting patients differentially, when compared to controls. METHODS: Data from the Swedish Knee Ligament Registry (SKLR) from 2005 to 2022 were used in univariate and multivariate logistic regression. The study compared two groups: patients with isolated ACLR and patients with ACLR and concomitant LM injury. These groups were further

Meniscal vascular contact loss ratio (MVCLR): A novel MRI index for predicting meniscal irreparability.

BACKGROUND: Accurate preoperative identification of meniscal tears with limited healing potential remains challenging. Conventional magnetic resonance imaging (MRI) descriptors - tear pattern, extrusion, displacement, and signal alterations - offer limited insight into the biological viability of the vascular peripheral rim. No MRI-based parameter currently quantifies the loss of meniscocapsular contact, the anatomical region most critical for vascular-mediated healing. This gap limits the ability to anticipate reparability and contributes to intraoperative decision variability. PURPOSE: To v

Lateral Meniscus Hypermobility: Current Concepts in Pathoanatomy, Diagnosis, and Evolving Treatment Strategies.

Hypermobility of the lateral meniscus (HLM) is an under-recognised yet clinically important source of knee pain and mechanical symptoms. It differs from conventional meniscal tears in that the meniscus itself remains structurally intact, while insufficiency of stabilising structures such as the popliteomeniscal fascicles, posterior capsule, and meniscotibial ligament permits abnormal translation. The distinctive anatomy of the lateral meniscus explains its greater vulnerability to instability compared with the medial side, particularly in young and athletic individuals. Patients commonly repor

Progression of medial meniscal extrusion following anterior cruciate ligament reconstruction correlates with graft diameter: a retrospective longitudinal study.

BACKGROUND: Posttraumatic osteoarthritis (PTOA), a subtype of knee osteoarthritis (OA), develops following intra-articular injuries such as fractures, ligament tears, or meniscal damage, accounting for approximately 12% of symptomatic knee OA. Notably, 87% of patients with anterior cruciate ligament (ACL) injuries are expected to develop PTOA, and there is currently no established method to prevent its progression. While ACL reconstruction (ACL-R) restores joint stability, it does not necessarily halt PTOA development. Previous MRI-based studies have identified medial meniscal extrusion (MME)

State of the Art for Meniscal Scaffolds.

Meniscal damage increases contact stress to the underlying chondral surface, leading to cartilage degeneration. However, meniscal repair is not always feasible, and partial meniscectomy is still the current standard of care for irreparable symptomatic lesions, including failure of primary repair. This approach can lead to the development of early osteoarthritis and irreversible knee damage in the long term. The main goals for treating the post-meniscectomy deficient knee are to allow pain-free daily activities, prevent swelling, and avoid further joint degeneration. The concept of meniscal reg

Incidence and Treatment Strategy of Lateral Meniscus Posterior Root Tears and Ramp Lesions Identified During Isolated ACL Reconstructions: Report From a Nationwide Knee Ligament Register.

BACKGROUND: Meniscus repair is increasingly common during primary anterior cruciate ligament reconstruction (ACLR), particularly because of an increased focus on the severity of untreated lateral meniscus posterior root tears (LMPRTs) and ramp lesions. However, the incidence and current treatment strategies for these injuries across a larger population remain unclear as previous reports come from selective cohort studies. PURPOSE: To report on the incidence of LMPRTs and ramp lesions in primary ACLR in Norway, compare patient- and activity-related characteristics in those with and without the
