Arthroscopic Transtibial Pullout Repair and Tibial Condylar Valgus Osteotomy for Medial Meniscus Posterior Root Tear With Varus Knee.

As an important structure for maintaining the hoop tension of the medial meniscus of the knee joint, the posterior root is receiving increasing attention. Medial meniscus posterior root tear is an important reason for the occurrence, development, and kinematics changes of knee osteoarthritis. It is necessary to repair the posterior root of meniscus for restoring joint kinematics and improving clinical efficacy. This Technical Note reports a medial meniscus posterior root tear repair technique using arthroscopic transtibial pullout repair (ATPR) combined with tibial condylar valgus osteotomy. T

Simplified Technique for Arthroscopic Repair of the Meniscus Root Tear of the Knee.

This report describes the arthroscopic transtibial pullout repair technique using multiple simple stitch (MSS), which was used to treat a medial meniscus posterior root tear (MMPRT) of the knee. The most commonly used technique to address this type of MMPRT is currently arthroscopic transtibial pullout repair. MSS pullout repair technique can provide excellent pullout strength and large tissue-bone contact area, which facilitates successful healing potential. This MSS pullout repair technique may be suggested as another useful option that can be used in the treatment of MMPRT.

Bucket-Handle Meniscal Tears Might Not Be an Urgency: The Time to Meniscus Repair Does Not Seem to Affect the Mid-Term Outcome-A Retrospective Study of Sixty Tears with a Mean Follow-Up of 6 Years.

Background: Bucket-handle meniscal tears are mostly treated arthroscopically. However, there is no clear evidence as to whether the time to surgery impacts the outcome and whether or not lesions should be treated urgently. Methods: Sixty patients were interviewed about the current status of their knee with a mean follow-up of 6.1 years (SD = 3.5). Forty-one patients underwent meniscus repair, and fifteen patients received partial resections. The primary outcome was the rate of reoperation after meniscus repair. Secondary outcomes were pain at rest and during exercise, return to sports, and Teg

Classification of dry eye disease subtypes.

PURPOSE: The current subclassifications of dry eye disease (DED) are aqueous deficient (ADDE) and evaporative (EDE) forms, but there lacks consistency in the clinical characteristics used to define each of these. This study used clinical data to inform cut-off values for the subclassification of ADDE and EDE, to allow more consistent study of the epidemiology of both DED subtypes. METHODS: The study enrolled 261 residents from the UK, extracted from a cohort with demographics representing the population (mean 42.4 ± 18.7 years, 56 % females). The TFOS DEWS II diagnostic criteria were used to

Horizontal Cleavage Meniscus Tear: "The Quad Tendon Augmentation Technique".

The management of medial meniscus horizontal cleavage tears can be challenging. Currently, several treatment options, including nonoperative and surgical options, have been proposed in the literature. Different repair techniques aiming to promote the healing process have been reported and have shown good outcomes. However, recurrent parameniscal cysts and decreased meniscal volume have also been reported. In this Technical Note, a novel surgical technique to repair a horizontal cleavage tear of the posterior horn of the medial meniscus is reported in young patients. The technique uses a strip

Epigenetic Dynamics in Meniscus Cell Migration and its Zonal Dependency in Response to Inflammatory Conditions: Implications for Regeneration Strategies.

Meniscus injuries pose significant challenges in clinical settings, primarily due to the intrinsic heterogeneity of the tissue and the limited efficacy of current treatments. Endogenous cell migration is crucial for the healing process, yet the regulatory mechanisms of meniscus cell migration and its zonal dependency within the meniscus are not fully understood. Thus, this study investigates the role of epigenetic mechanisms in governing meniscus cell migration under inflammatory conditions, with a focus on their implications for injury healing and regeneration. Here, we discovered that a proi

Novel Meniscus Transfer Technique: A Case Report.

The meniscus is an essential component of the knee joint, acting as a shock absorber as well as assisting in the transmission of forces. Due to the meniscus importance of the knee, much of the current literature focuses on treatment techniques that can spare and repair the meniscus when it is torn. The unique vasculature of the meniscus often makes repair difficult or, in many cases, impossible. A current focus within orthopedics has been on meniscal allograft transplantation to fill this gap. The lack of a universal surgical technique for graft fixation, along with the current failure rates,

All-inside suture meniscal repair using suture passer.

In recent years, to save the meniscus and prevent the progression of knee osteoarthritis, the indications for meniscus repair have been expanding instead of partial menisectomy. Accordingly, various repair techniques for meniscus tears have been developed. The conventional inside-out and outside-in meniscus repair techniques and all-inside repair technique with an implant/anchor can be classified as trans-capsular (TC) repair from the perspective of suture with penetrating capsule. Recently, new suture passers for all-inside meniscus repair have been developed. To distinguish from conventional

Advanced nanoparticles in osteoarthritis treatment.

Osteoarthritis (OA) is the most prevalent degenerative joint disorder, affecting hundreds of millions of people globally. Current clinical approaches are confined to providing only symptomatic relief. Research over the past two decades has established that OA is not merely a process of wear and tear of the articular cartilage but involves abnormal remodelling of all joint tissues. Although many new mechanisms of disease have been identified in the past several decades, the efficient and sustainable delivery of drugs targeting these mechanisms in joint tissues remains a major challenge. The potential of tendon autograft as meniscus substitution: Current concepts.

Meniscectomy is known to alter the mechanics, stability, and kinematics of the tibiofemoral joint, leading to early knee osteoarthritis (KOA). While several meniscal substitutions exist, such as meniscus allograft transplantation, collagen meniscus implants, and artificial substitutes, they often come with technical challenges, high costs, and risks, including allograft failure, infections, and disease transmission. Tendon autografts emerge as a promising option, offering safety, availability, biocompatibility, and a reduced risk of pathophoresis. This review delves into basic, in vivo, in vit

The management of isolated meniscal tears in skeletally immature children. An international expert consensus.

PURPOSE: The prevalence and appreciation of meniscal tears in children have increased in both number and complexity. There is currently a paucity of high-quality evidence that can guide surgeons in treating skeletally immature patients with meniscal injuries. The aim of this study was to develop comprehensive recommendations for the management of isolated meniscal tears in skeletally immature children. METHODS: An international, two-round, modified Delphi consensus was completed. Included 'experts' were identified as having an established adult knee practice, including children and either: (1

Diagnosis and Treatment Strategies of Meniscus Root Tears: A Scoping Review.

BACKGROUND: Meniscus root tears comprise 10% to 21% of all meniscal tears. These tears alter knee biomechanics, elevating contact pressure, akin to a meniscectomy. Consequently, they are linked to advanced joint degeneration and cartilage damage in the affected compartment. PURPOSE: To systematically evaluate and relate the current literature describing the diagnosis and treatment strategies for meniscus root tears. STUDY DESIGN: Scoping review; Level of evidence, 4.
