Treatment of anterior cruciate ligament injury in skeletally immature patients.

The incidence of ACL tears is rising in the pediatric and adolescent populations as these individuals succumb to traumatic and nontraumatic athletic injuries. Management of this condition in the skeletally immature patient poses a challenge and is controversial. Operative reconstruction carries the concern for damage to the physis with resultant limb length inequality and angular joint deformity but provides stability to the knee and allows return of function in most patients. On the other hand, nonoperative treatment has been shown to carry an increased risk of meniscal and articular cartilag

[Magnetic resonance imaging in the diagnosis of intra-articular lesions of the knee].

PURPOSE OF THE STUDY: Magnetic resonance imaging (MRI) has the highest sensitivity of all methods for the diagnosis of intra-articular knee injuries. In spite of this, its benefit for the decision-making algorithm is questionable. The aim of this study was to evaluate the real situation in our regional conditions. MATERIAL AND METHODS: The medical records of the patients who underwent knee arthroscopy in 2008 and 2009, and had pre-operative MRI examination, were retrospectively reviewed. The group included 92 patients (46 women and 46 men; average age, 41.7 years) of whom 49 had knee injury i

The effect of meniscal tears on cartilage loss of the knee: findings on serial MRIs.

BACKGROUND: The relationship between meniscal tears and progressive loss of hyaline cartilage and osteoarthritis of the knee has been reported in the literature. The current treatment protocols for meniscal tears include conservative treatment, meniscal repair, and meniscectomy. Treatment plans are based on factors such as tear pattern, patient age, and associated pathology. The mechanism, pattern, and treatment of meniscal tears vary with age and activity level. Younger, more active patients often sustain more acute tears, which are more amenable to repair due to increased propensity for heal

The health and structural consequences of acute knee injuries involving rupture of the anterior cruciate ligament.

Although there is an abundance of literature regarding the development of knee osteoarthritis after rupture of the anterior cruciate ligament (ACL), the mechanism underlying this link is not clear. Recent studies have reported that several factors may be predictive of the development of osteoarthritis, including damage to the menisci and articular cartilage during the initial trauma, altered knee biomechanics after injury, and episodic instability. This article summarizes recent developments in the understanding of the joint damage resulting from an ACL tear, and the influence that current and

The growing array of innate inflammatory ignition switches in osteoarthritis.

Osteoarthritis (OA) is the most common form of arthritis and is a major cause of chronic pain and disability. We currently lack disease-modifying OA medical therapeutics that effectively slow or halt the progression to destruction and failure of articular cartilage. Importantly, OA is a disease of the whole joint, including not only meniscal fibrocartilage and hyaline articular cartilage, but also subchondral bone, periarticular musculature, tendons and ligaments, articular adipose tissue, synovium, and synovial fluid (SF). Clinically, varying degrees of synovitis and joint effusion in OA cont

Clinically relevant injury patterns after an anterior cruciate ligament injury provide insight into injury mechanisms.

BACKGROUND: The functional disability and high costs of treating anterior cruciate ligament (ACL) injuries have generated a great deal of interest in understanding the mechanism of noncontact ACL injuries. Secondary bone bruises have been reported in over 80% of partial and complete ACL ruptures. PURPOSE: The objectives of this study were (1) to quantify ACL strain under a range of physiologically relevant loading conditions and (2) to evaluate soft tissue and bony injury patterns associated with applied loading conditions thought to be responsible for many noncontact ACL injuries. STUDY DES

Is repeated repair of the meniscus worthwhile?

Meniscal injuries are one of the most commonly encountered problems by orthopaedic surgeons. It is well established that meniscectomy will result in increased joint surface contact forces and the predictable progression of arthritis. The management of meniscal tears has evolved and current evidence would suggest that every attempt should be made to repair meniscal tears when appropriate. The reported success rate of meniscal repair is encouraging, but relatively little is known about the outcome of repeated repair of a re-torn meniscus. This review presents an illustrative case of a recurrent

An experimental study of COMP (cartilage oligomeric matrix protein) in the rabbit menisci.

INTRODUCTION: Secondary knee osteoarthritis (OA) is currently associated with meniscal injuries, but the pathogenesis is unclear. We analyzed the distribution of cells and cartilage oligomeric matrix protein (COMP) and its changes in the early stages of degeneration in meniscus. METHOD: Ten New Zealand rabbits underwent anterior cruciate ligament (ACL)-transection of the right knee-joint. Left knee-joints were used as controls. The animals were killed at 4 and 12 weeks. Gross injuries in meniscus and articular cartilage were scored. Meniscal tissues were immunostained with a specific antibody

Advances in meniscal tissue engineering.

Injuries and lesions to the meniscal cartilage of the knee joint are common. As a result of its limited regenerative capacity, early degenerative changes to the articular surface frequently occur, resulting in pain and poor function. Currently available surgical interventions include repair of tears, and partial and total meniscectomy but the results are inconsistent and often poor. Interest in the field of meniscal tissue engineering with the possibilities of better treatment outcomes has grown in recent times. Current research has focused on the use of mesenchymal stem cells, fibrochondrocyt

Delayed gadolinium-enhanced magnetic resonance imaging of hip joint cartilage: pearls and pitfalls.

With the increasing advances in hip joint preservation surgery, accurate diagnosis and assessment of femoral head and acetabular cartilage status is becoming increasingly important. Magnetic resonance imaging (MRI) of the hip does present technical difficulties. The fairly thin cartilage lining necessitates high image resolution and high contrast-to-noise ratio (CNR). With MR arthrography (MRA) using intraarticular injected gadolinium, labral tears and cartilage clefts may be better identified through the contrast medium filling into the clefts. However, the ability of MRA to detect varying gr

Approaching the ruptured anterior cruciate ligament.

Anterior cruciate ligament (ACL) disruptions are common injuries that currently hold a fearsome reputation among athletes of all abilities and disciplines. Indeed, if the diagnosis is missed at first presentation, it is difficult to attribute ongoing instability and recurrent injury to an ACL tear. Classically, patients then often improve shortly before repeatedly reinjuring their knee. At some point, the knee may lock, necessitating an arthroscopic meniscectomy. Tragically, this then hastens the progression of joint arthrosis and the decline of the joint function. While the burden of responsi

Meniscal debridement: current concepts.

The meniscus is a commonly injured structure in the knee. When intact, it serves multiple biomechanical functions that protect the underlying articular cartilage. When injured, it can cause pain and mechanical symptoms that restrict activity. Although meniscal tears can be effectively treated with arthroscopic debridement, there is concern that any amount of meniscectomy, no matter how small, is a risk factor for the development of long-term osteoarthritis. This article reviews current concepts in arthroscopic meniscal debridement, including clinical outcome studies, to understand which factor

Articular cartilage defects in the knee--basics, therapies and results.

Full-thickness defects of the articular cartilage in the knee joint have lower regenerative properties than chondral lesions of the ankle. In order to avoid early osteoarthritis, symptomatic articular cartilage defects in younger patients should undergo biological reconstruction as soon as possible. Various surgical procedures are available to biologically resurface the articular joint line. Numerous animal experiments and clinical studies have shown that early biological reconstruction of circumscribed cartilage defects in the knee is superior to conservative or delayed surgical treatment. The role of the meniscus in osteoarthritis genesis.

The menisci play a critical protective role for the knee joint through shock absorption and load distribution. Currently, the consensus in surgical treatment of meniscal tears is to preserve as much functional meniscal tissue as possible. The health professional is challenged to choose the best treatment, both in the short- and in the long-term. A degenerative lesion in the middle-aged or older patient could suggest early-stage knee osteoarthritis and should be treated accordingly. Surgical resection of nonobstructive degenerate lesions may remove only the evidence of the disorder while the os

Integrative repair of the meniscus: lessons from in vitro studies.

Current therapies for meniscal injury seek to preserve and repair damaged tissue since loss of meniscal tissue is associated with degenerative changes in the joint, ultimately leading to osteoarthritis (OA). After a meniscal tear, the difficulty of integrating juxtaposed meniscal surfaces continues to be an obstacle. In order to determine the local factors that are necessary for successful tissue repair, previous studies have developed in vitro model systems that allow both biological and quantitative biomechanical measures of meniscus repair. Many studies have shown the importance of individu

Risk of osteonecrosis of the femoral condyle after arthroscopic chondroplasty using radiofrequency: a prospective clinical series.

Radiofrequency (RF) energy can be used for treatment of intraarticular pathologies in knee joint. RF energy was found to be superior to mechanical techniques in smoothening the articular surface (chondroplasty), shortening the operation time and reducing the blood loss. As RF produces thermal energy it has been reported to be responsible for the postoperative osteonecrosis however, there is no clinical evidence in the literature supporting that RF causes osteonecrosis. The current study searches for an answer whether surgical arthroscopic modalities using RF energy causes osteonecrosis. We hyp

A novel bioabsorbable conduit augments healing of avascular meniscal tears in a dog model.

BACKGROUND: Avascular meniscal tears are a common and costly problem for which current treatment options are limited. HYPOTHESIS: A bioabsorbable conduit will allow for vascular tissue ingrowth that is associated with histologic and biomechanical evidence for avascular meniscal tear healing superior to that associated with meniscal trephining in dogs. STUDY DESIGN: Controlled laboratory study. METHODS: Twenty-five dogs underwent medial arthrotomy with creation of anterior and posterior tears in the medial menisci (N = 50 tears). The dogs were assigned treatments for their menisci: conduit (

PURPOSE: Measurement protocols which have been optimized for MRI at field strengths of 1.5 T or 3 T cannot be directly transferred to 7 T. Specific absorption rate limitations, different tissue relaxation times, as well as new image artifacts require adjustments of the sequence parameters. The goal of our study was to investigate and optimize various sequences for 7 T imaging of the knee. MATERIALS AND METHODS: Starting with sequences used on a standard 1.5 T scanner, the parameters were modified to obtain optimal image contrast, maximum coverage, and the highest spatial resolution within a r

Articular cartilage injury of the posterior lateral tibial plateau associated with acute anterior cruciate ligament injury.

Thirty-nine knees in 39 patients with recent anterior cruciate ligament (ACL) rupture (age 14-55 years; with a mean age of 22.8 years) were selected from our cases of ACL reconstruction from July 2005 to June 2006, to take part in a study on articular cartilage injury of the posterior lateral tibial plateau associated with acute ACL injury and on the correlation between bone bruises depicted on MRI and cartilage injury of the posterior lateral tibial plateau detected at arthroscopic ACL reconstruction. Using preoperative MRI, we evaluated whether there were bone bruises or not in the lateral c

Healing of a surgically created partial detachment of the posterior cruciate ligament using marrow stimulation: an experimental study in dogs.

The inability of intra-articular (cruciate) ligaments to generate a substantive reparative response may preclude the primary surgical repair of such lesions. A marrow stimulation technique that provides access to a fibrin scaffold, cytokines, and mesenchymal stem cells has been shown to be successful in generating a repair response in articular cartilage. We hypothesized that a similar approach may be effective in initiating and supporting a healing response in cruciate ligament injuries. The ability to treat cruciate ligament injuries by primary repair may have significant advantages over the
