[Cartilage repair in the knee joint].

Full thickness defects of the articular cartilage in the knee joint have lower regenerative properties compared to chondral lesions of the ankle. In order to avoid early osteoarthritis, symptomatic articular cartilage defects in younger patients should undergo biological reconstruction as early as possible. There are different surgical procedures available to achieve a biological resurfacing of the articular joint line. Numerous animal experiments and clinical studies have shown that early biological reconstruction of circumscribed cartilage defects in the knee is superior to a conservative or

Conservative treatment for a symptomatic solitary ganglion cyst of the anterior meniscus invading into the infrapatellar fat pad.

Symptomatic meniscal cysts without accompanying meniscal tears are uncommon, especially those which are formed from the anterior meniscus and invading into the infrapatellar fat pad (IPFP). The current recommended treatment for such cysts is excision through open method or arthroscopy. To our knowledge, this is the first case study about a symptomatic solitary meniscal cyst invading the IPFP that was successfully treated with needle aspiration and steroid injection after arthroscopic treatment of the associated disorders. In this paper, similar cases are also reviewed to discuss the treatment

Meniscal repair and transplantation: indications, techniques, rehabilitation, and clinical outcome.

The purpose of this paper is to provide current knowledge regarding the indications, operative techniques, rehabilitation programs, and clinical outcomes of meniscus repair and transplantation procedures. Meniscus tears that occur in the periphery may be repaired using a variety of operative procedures with high success rates. Complex multiplanar tears that extend into the central one-third avascular zone can also be successfully repaired using a meticulous vertically divergent suture technique. The outcome of these repairs justifies preservation of meniscal tissue, especially in younger athle

MRI and non-cartilaginous structures in knee osteoarthritis.

Magnetic resonance imaging (MRI) provides a sensitive tool for examining all the structures involved in the osteoarthritis (OA) process. While much of the MRI literature previously focussed on cartilage, there is increasing research on whole-organ evaluation and including features such as synovitis, bone marrow edema, and meniscal and ligamentous pathology. The aim of this session at the Outcome Measures in Rheumatology Clinical Trials (OMERACT)-Osteoarthritis Research Society International (OARSI) Workshop for Consensus in Osteoarthritis Imaging was to describe the current MRI methods for ide

Is there an association between articular cartilage changes and degenerative meniscus tears?

PURPOSE: The objective of the present study was to evaluate whether horizontal cleavage and complex meniscus tears, which are degenerative tears, are associated with an increased incidence of cartilage damage, in comparison with patients having other patterns of meniscal injury. TYPE OF STUDY: Case series study. METHODS: Data were collected prospectively from 497 consecutive knee arthroscopies carried out from 1997 to 2001. Patient data (age, sex, duration of symptoms, injuries, and possible mechanism of injury), operative details (types and number of portals, equipment used), intra-articula

[Significance and technique of autologous chondrocyte transplantation].

The bad risk for an early onset of osteoarthritis in the knee increases with the size of a cartilage defect. A collateral meniscus- or ligament-tear will enforce this hazard in addition. In order to avoid such a development, relevant full-thickness cartilage defects should be reconstructed biologically and attendant meniscus- or ligament-tears as well as varus- or valgus deformities should be treated. A number of studies, including some prospective-randomized trials, have shown that autologous chondrocyte transplantation (ACT) is the most reliable procedure for a surgical treatment of full-thi

Subchondral giant-cell tumor of the proximal tibia: arthroscopic treatment for accelerated articular cartilage and meniscal degeneration in two patients.

Giant-cell tumor most commonly occurs in the distal femur and proximal tibia and characteristically involves the subchondral bone. Incomplete resection leads to recurrence rates of up to 50%. Intralesional curettage, adjuvant treatments, and polymethyl methacralate (PMMA) reconstruction is the current mainstay of treatment and has produced recurrence rates of less than 10%. Achieving adequate curettage while preserving the articular cartilage of the tibial plateau poses a significant challenge, especially when the tumor involves the subchondral bone. We report on 2 cases, both with symptomatic

Toward tissue engineering of the knee meniscus.

This review details current efforts to tissue engineer the knee meniscus successfully. The meniscus is a fibrocartilaginous tissue found within the knee joint that is responsible for shock absorption, load transmission, and stability within the knee joint. If this tissue is damaged, either through tears or degenerative processes, then deterioration of the articular cartilage can occur. Unfortunately, there is a dearth in the amount of work done to tissue engineer the meniscus when compared to other musculoskeletal tissues, such as bone. This review gives a brief overview of meniscal anatomy, b

Animal models of osteoarthritis.

Animal models of osteoarthritis are used to study the pathogenesis of cartilage degeneration and to evaluate potential antiarthritic drugs for clinical use. Animal models of naturally occurring osteoarthritis (OA) occur in knee joints of guinea pigs, mice and other laboratory animal species. Transgenic models have been developed in mice. Commonly utilized surgical instability models include medial meniscal tear in guinea pigs and rats, medial or lateral partial meniscectomy in rabbits, medial partial or total meniscectomy or anterior cruciate transection in dogs. Additional models of cartilage

Resection of degenerate menisci - is it useful?

Forty-three symptomatic patients, with a pre-operative MRI diagnosis of an isolated degenerate tear of the meniscus, treated by Arthroscopic resection were reviewed after a minimum follow up of 6 years post-surgery. Pre-operatively there was no clinical or radiological evidence of arthroses, but at surgery 65% of patients were noted to have varying degrees of articular cartilage changes, ranging from Grade I to Grade III according to Outerbridge's classification. Six and a half years (range 6-8 years) following Arthroscopic resection of these degenerate tears 50% of the patients had moderate t

Musculoskeletal MR: knee.

Magnetic resonance imaging is the most sensitive, specific, and accurate noninvasive method for diagnosing internal derangement of the knee. During the past 15 years knowledge of pathologic conditions of the knee had evolved significantly. Beyond the basic principles of imaging knee injuries great impact was made on the understanding of indirect or collateral findings, even in rare diseases. In this article the spectrum of disorders of the knee are reviewed and an overview of the current literature is given. This includes considerations about how to achieve a high-standard MR imaging study of

[Damage to the glenoid labrum and expert assessment].

AIM: Development of criteria for the medical-legal interpretation of different patterns of labral lesions according to the labrum pathology and mechanism of injury. METHODS: A metaanalysis of the literature concerning injuries and diseases of the shoulder involving the glenoid labrum was performed. RESULTS: Lesions of the superior, anterior and posterior labrum are rare and of different severity. Arthroscopically they are divided into distinct types. They develop either spontaneously together with intraarticular diseases, due to chronic fatigue or by injury. In this field the stability of th

MR imaging before arthroscopy in knee joint disorders?

PURPOSE: To review the current literature examining the relative merits of arthroscopy and MR imaging of the knee. MATERIAL AND METHODS: All papers comparing MR imaging with arthroscopy published within the last 10 years according to Medline were collected and read. RESULTS: Technology has improved considerably during recent years allowing detailed non-invasive visualization of the knee. In particular, the development of cheaper whole-body and dedicated low-field MR units has opened up for non-invasive inspection of the knee at reasonable cost. Meniscal tears can be detected with accuracy ra

Anterior cruciate ligament tear: prospective evaluation of diagnostic accuracy of middle- and high-field-strength MR imaging at 1.5 and 0.5 T.

PURPOSE: To compare the diagnostic efficacy of current-generation middle- and high-field-strength magnetic resonance (MR) imagers in the diagnosis of anterior cruciate ligament (ACL) tears. MATERIALS AND METHODS: In 114 of 230 patients referred for knee imaging, MR imaging at 0.5 and 1.5 T was performed with identical sequences but with a slightly longer total imaging time and bandwidth optimization at 0.5 T. Radiologists were blinded to diagnosis and field strength. Sensitivity, specificity, and accuracy were determined, and ACL tear was confirmed by means of arthroscopy and pathology. RESU

Contact pressures in the patellofemoral joint during impact loading on the human flexed knee.

Currently, a bone fracture criterion is used by the automotive industry to assess the potential for a lower extremity injury from impact directed on the flexed knee. However, recent studies with animal models indicate irreversible damage to articular cartilage due to overpressures generated within the patellofemoral (P-F) joint without bone fracture, and suggest this injury may lead to a progressive, degenerative disease of the joint. The purpose of this investigation was to measure contact pressure in the human P-F joint during impact loading on the isolated, flexed knee. Loads were delivered

Magnetic resonance imaging of the knee.

Magnetic resonance imaging (MRI) is an accepted non-invasive modality for evaluation of soft tissue pathology without exposure to ionizing radiation. Current applications demonstrate excellent visualization of the anatomy and pathology of various organs. Preliminary studies in the knee reveal fine resolution of anatomy and pathology involving the meniscus. The purpose of this study is to determine a prospective correlation between MRI scans and actual meniscal pathology as documented at the time of arthroscopy. MRI scans were obtained in 155 patients, on 156 knees (one patient with bilateral s
