Alignment of the extensor apparatus of the knee by alar and tuberosity transposition in recurrent subluxation of the patella and in external patellar hypertension syndrome.

In consideration of the multiple anatomical structures involved in recurrent subluxation of the patella and in external patellar hypertension syndrome, the authors propose an operation which allows for complete realignment of the extensor apparatus by transposition of the anterior tibial tuberosity, division of the external alar ligament, tightening of the internal alar ligament, and correction of the muscular insertions. In particular, removal of a capsulofascial band medially and its reimplantation into the external incision is proposed, thus avoiding the formation of retractive scar tissue.

Open joint injuries. Classification and treatment.

Experience with wounds involving the major joints of the lower extremity suggests that the extracapsular soft tissues and the intracapsular structures should be considered individually to assess accurately the magnitude of injury and to provide a prognosis. A classification scheme is proposed to characterize the areas and types of injury. Type I open joint injuries are single penetrations without extensive soft-tissue damage, permitting uncomplicated joint and wound closure. Type II injuries are single or multiple penetrations with extensive soft-tissue disruptions (flaps, avulsions, degloving

Injuries caused by pigs in Papua New Guinea.

Pigs are intelligent animals that can be formidable adversaries to humans because of their sharp tusks and their ability to attack swiftly. Domestic and feral pigs have an important role in the ecology of village life in Melanesia. A six-year review of all injuries that were caused by pigs that were referred from the villages in Milne Bay Province, Papua New Guinea, to the Provincial Hospital was completed. Some of the injuries that were seen among the 20 patients who were studied included: three penetrating abdominal injuries with prolapse and strangulation of the intestine; a "sucking" chest

Operative treatment of acute and chronic knee problems.

Attention to detail and anatomic precision is essential to accurate diagnosis. Accurate diagnosis is the keystone of care for the acute or chronic knee problem. The arthroscope is an essential diagnostic tool. Uniplanar instability can usually be treated nonoperatively with good results providing that secondary supporting structures are not anatomically disrupted, the ligaments are protected during their plastic healing phase, and the patient is committed to a complete rehabilitation program. Rotatory instabilities require surgery if a high functional level is desired by the patient, and the s

Musculoskeletal applications of three-dimensional surface reconstructions.

We have applied computer programs originally developed for craniofacial surgical planning and evaluation to complex musculoskeletal problems. These computer programs reformat ordinary CT scans into black and white images of the three-dimensional osseous surfaces found in the scanned volume. These reformatted three-dimensional CT scan images increase the utility of CT scan examinations of complex osseous structures, such as the wrist, spine, hip, knee, and shoulder. The software, which operates on an unmodified commercially available CT scanner, can produce high-quality surface reconstructions

Anatomical relationship between the medial head of the gastrocnemius and the occurrence of a dystrophic bony lacuna.

A bony lesion with identical radiological features was seen in the medial part of the distal femoral metaphysis in five patients who practised cross-country motorcycling. These findings suggested that the cause of the lesions was of muscular origin. The nature and physiopathogenesis of the lesions were studied by roentgenography, computerized tomography and analysis of anatomical specimens. The results of these procedures were compared to observations made peroperatively. The lesion was identified as a specific, dystrophic bony lacuna resulting from the action of the medial head of the gastroc

Posterior cruciate ligament insufficiency. A review of the literature.

A review of the English language literature establishes athletic mishaps as a major cause of posterior cruciate ligament injury. However, diversity of opinion exists regarding the functional significance of the lesion, its occurrence as an isolated entity, and the roles of conservative and surgical management. The posterior cruciate ligament is a composite structure, consisting of a superficial tibiofemoral and meniscofemoral portion and a deep tibiofemoral portion. The structure is intra-articular but extrasynovial, coursing from its attachment to the lateral surface of the medial femoral con

Anterior cruciate-deficient knees: a review of the literature.

Injury to the anterior cruciate ligament may lead to functional instability, meniscal injury, and premature degenerative changes of the knee or, if treatment and rehabilitation are carried out carefully, to a functionally stable knee with no premature degenerative changes. Unfortunately, the complex anatomy of the three fiber bundles, consisting of multiple collagenous strands, which constitutes the anterior cruciate ligament combined with variation of injuries to the ligament, with or without injury to other structures of the knee, makes any standardization of surgical treatment difficult. In

Congenital deformities about the knee.

Congenital deformities about the knee are the result of abnormalities of all the anatomical structures that make up this joint. In some cases the structural changes observed are due to primary disorders of formation and growth of one or more of the anatomical components of the knee. Disorders are known to occur in the integumentary, muscular, ligamentous supporting, and skeletal systems. Examples of these disorders are seen in the bone dysplasias and joint laxity syndromes. In other cases exaggerated intrauterine positions of the limb seem to bring about certain adaptive changes in the embryol
