Fear of movement/(re)injury and activity avoidance in persons with neurogenic versus vascular claudication.

BACKGROUND CONTEXT: Activity avoidance and fear of movement/(re)injury are increasingly being recognized as important factors in the rehabilitation of persons suffering from chronic low back pain, yet these factors have not been thoroughly explored in persons suffering from neurogenic claudication resulting from lumbar spinal stenosis. PURPOSE: To determine, compare, and explain differences in the degree of fear of movement/(re)injury and activity avoidance in persons with neurogenic claudication, vascular claudication, and asymptomatic volunteers. STUDY DESIGN: Prospective controlled cohort

Superficial peroneal and sural nerve transfer to tibial nerve for restoration of plantar sensation after complex injuries of the tibial nerve: cadaver feasibility study.

BACKGROUND: Nerve reconstruction following lower-extremity nerve injuries usually leads to worse outcomes in comparison with upper-extremity injuries due to the long distances of nerve regeneration. This study was performed to consider the clinical application of distal nerve transfer for the treatment of long gaps of the tibial nerve (TN) and in established compartment syndrome. It aimed to determine the anatomic suitability of transferring the sural nerve (SN) in combination with the superficial peroneal nerve (SPN) to the TN at the level of the tarsal tunnel for restoration of plantar sensa

The course of the distal saphenous nerve: a cadaveric investigation and clinical implications.

INTRODUCTION: Injury to the saphenous nerve at the ankle has been described as a complication resulting from incision and dissection over the distal tibia and medial malleolus. However, the exact course and location of the distal saphenous nerve is not well described in the literature. The purpose of this study was to determine the distal limit of the saphenous nerve and its anatomic relationship to commonly identified orthopaedic landmarks and surgical incisions. METHODS: Sixteen cadaveric ankles were examined at the level of the distal tibia medial malleolus. An incision was made along the

Influence of spinal reflexes on the locomotor pattern after spinal cord injury.

In complete spinal cord injured (cSCI) subjects a shift from dominant early (60-120ms latency) to dominant late (120-450ms latency) spinal reflex (SR) components occurs over time after injury. This shift is assumed to reflect a spinal neuronal dysfunction below the level of a spinal lesion. The neuronal pathways of SR are suggested to be closely connected with spinal locomotor circuits. The aim of this study was to explore the influence of the two SR components on the electromyographic (EMG) pattern induced by assisted locomotion in cSCI subjects. Leg muscle EMG activity was analysed during as

Effectiveness of manual physical therapy in the treatment of cervical radiculopathy: a systematic review.

BACKGROUND: In sub-Saharan Africa poly-parasite infections are frequently observed in children, and with poly-parasitism modulating immune mechanisms, mediated by cytokines and chemokines, are required to prevent overwhelming inflammation and host tissue damage. We analyzed in children co-infected with helminthes and protozoan parasites their cellular production of regulatory and pro-inflammatory cytokines and chemokines in response to parasite antigens and allergens. METHODS: Intestinal and intravascular parasite infections were detected in stool and urines samples. The in vitro cellular cyt

Topographical distribution of motor fascicles in the sciatic-tibial nerve of the rat.

Knowledge of the intraneural topography of peripheral nerves may help to improve nerve repair after injuries and the selectivity of neural interfaces. We studied the fascicular pattern of motor fibers of the rat sciatic-tibial nerve. We carried out an anatomical dissection of the muscular tributaries of the tibial nerve in the leg. Immunohistochemistry against choline acetyltransferase was used to identify motor axons. Retrograde tracing allowed localization of the muscular fascicles at proximal levels of the sciatic trunk. The distribution of motor fibers in transverse section of the tibial n

Femoral shortening in total hip arthroplasty for high developmental dysplasia of the hip.

BACKGROUND: When reconstructing a hip with developmental dysplasia with a high dislocation, placing the acetabular component in the anatomic position can result in a prosthetic hip that is difficult to reduce. Subtrochanteric femoral osteotomy and shortening makes reduction easier but can be associated with complications (eg, limp, sciatic nerve injury, nonunion of the osteotomy) or compromise long-term stem survival. QUESTIONS/PURPOSES: We therefore evaluated (1) the short-term complication rate, (2) functional scores, and (3) survivorship of prostheses in patients with high developmental dy

Sportsman hernia: what can we do?

INTRODUCTION: Sportsman (sports) hernia is a medially located bulge in the posterior wall of the inguinal canal that is common in football players. About 90% of cases occur in males. The injury is also found in the general population. CLINICAL PRESENTATION: The presenting symptom is chronic groin pain which develops during exercise, aggravated by sudden movements, accompanied by subtle physical examination findings and a medial inguinal bulge on ultrasound. Pain persists after a game, abates during a period of lay-off, but returns on the resumption of sport. Frequently, sports hernia is one c

The proximally designed sural flap based on the accompanying artery of the lesser saphenous vein.

Most of the literature about the sural flap deals with soft tissue defects at the lower leg and foot based on the classical description of Masquelet et al. Nevertheless, some authors have shown that the lesser saphenous vein and its accompanying artery play an important role in the vascularization of the posterior skin of the leg. This finding allowed a more proximal design of the flap, increasing its arc of rotation. Thirteen cases of distally based sural flaps were performed and divided in two groups according to the level where the flaps were harvested. In group A, the entire flap was outli

Protecting the genitofemoral nerve during direct/extreme lateral interbody fusion (DLIF/XLIF) procedures.

A 77-year-old male presented with a history of severe lower back pain for 10 years with radiculopathy, positive claudication type symptoms in his calf with walking, and severe "burning" in his legs bilaterally with walking. Magnetic resonance imaging (MRI) revealed lumbar stenosis at the L3-L4 and L4-L5 levels. During the direct or extreme lateral interbody fusion (DLIF/XLIF) procedure, bilateral posterior tibial, femoral, and ulnar nerve somatosensory evoked potentials (SSEPs) were recorded with good morphology of waveforms observed. Spontaneous electromyography (S-EMG) and triggered electrom

Changes in spinal reflex and locomotor activity after a complete spinal cord injury: a common mechanism?

Locomotor activity and spinal reflexes (SRs) show common features in different mammals, including humans. Here we report the time-course of the development of locomotor activity and SRs after a complete spinal cord injury in humans. SRs evoked by tibial nerve stimulation were studied, as was the leg muscle electromyography activity evoked by mechanically assisted locomotion (Lokomat) in biceps femoris, rectus femoris, tibialis anterior and gastrocenmius medialis. Around 8 weeks after the injury, an early SR component (latency 60-120 ms) appeared, as in healthy subjects, and a well-organized le

Proximal motor branches from the tibial nerve as direct donors to restore function of the deep fibular nerve for treatment of high sciatic nerve injuries: a cadaveric feasibility study.

OBJECTIVE: The results of surgical repair of the fibular division of the sciatic nerve have been considered unsatisfactory, especially if grafts are necessary to reconstruct the nerve. To consider the clinical application of the concept of distal nerve transfer for the treatment of high sciatic nerve injuries, this study aimed to determine detailed anatomic data about the possible donor branches from the tibial nerve that are available for reinnervation of the deep fibular nerve at the level of the popliteal fossa. METHODS: An anatomic study was performed that included the dissection of the p

Peripheral nerve grafts in a spinal cord prosthesis result in regeneration and motor evoked potentials following spinal cord resection.

PURPOSE: To evaluate whether a standardized nerve graft bridging of a spinal cord injury gap in a device with white to grey matter projections allows regeneration and electrophysiological contact from supraspinal centers to the leg, since previous studies using nerve grafts in combination with acidic fibroblast growth factor have been difficult to microsurgically reproduce. METHODS: A moulded prosthesis containing twelve peripheral nerve grafts with estimated pre-set projections guiding white tracts to grey matter replaced a spinal cord resection gap at level T11 in adult rats. The animals we

A posttraumatic, joint-connected sural intraneural ganglion cyst-with a new mechanism of intraneural recurrence: a case report.

UNLABELLED: Intraneural ganglion cysts are rare in occurrence and most commonly involve the peroneal nerve at the fibular neck. We present a case of a traumatically induced intraneural ganglion cyst of the sural nerve that developed after a nondisplaced posterior malleolus ankle fracture. The intraneural ganglion cyst was connected to the subtalar joint by its articular branch and ascended several centimeters into the distal fourth of the leg. It was resected from the sural nerve proper and the posterior branch of the lateral calcaneal nerve, and the articular trunk was ligated. The patient de

The Dixie Cup technique for salvaging limb length and preserving nerve continuity in the mangled lower extremity.

The mangled lower extremity posses a difficult problem for the reconstructive surgeon. We describe a novel technique for preserving limb length and maintaining nerve continuity in a severely injured lower extremity. This specific example of fixating the viable, sensate distal foot tissues onto the proximal tibia was vital in maintaining optimal limb length. This allowed for a more desirable level of amputation, preserved knee function, and provided well-vascularized, sensate soft tissue coverage.

Albert Sidney Johnston's sciatic dueling injury did not contribute to his death at the Battle of Shiloh.

OBJECTIVE: To determine whether General Albert Sidney Johnston's dueling wound and nerve injury (1837) contributed to his death at the Battle of Shiloh (1862). General A.S. Johnston was commander of the Confederate Army at Shiloh and was killed by a bullet that severed his right popliteal artery. The location of this wound in the popliteal fossa region was largely unnoticed and, consequently, was not treated expeditiously. It has been widely assumed that the sciatic nerve was injured in a duel 3 decades before and that this injury resulted in a loss of sensation in the right posterior thigh an
