Foot drop: where, why and what to do?

Foot drop is a common and distressing problem that can lead to falls and injury. Although the most frequent cause is a (common) peroneal neuropathy at the neck of the fibula, other causes include anterior horn cell disease, lumbar plexopathies, L5 radiculopathy and partial sciatic neuropathy. And even when the nerve lesion is clearly at the fibular neck there are a variety of causes that may not be immediately obvious; habitual leg crossing may well be the most frequent cause and most patients improve when they stop this habit. A meticulous neurological evaluation goes a long way to ascertain

Treatment of combined osteoporotic compression fractures and spinal stenosis: use of vertebral augumentation and interspinous process spacer.

STUDY DESIGN: Treatment of osteoporotic compression fractures has rapidly evolved with the use of kyphoplasty and vertebroplasty. Likewise, lumbar spinal stenosis is being treated with less invasive procedures such as interspinous process spacers. The combination of such minimally invasive procedures offers new advantages to selected patients. OBJECTIVE: Evaluation of vertebral augmentation combined with interspinous decompression procedure in the same sitting. SUMMARY OF BACKGROUND DATA: We present 2 cases of elderly females who had lumbar compression fractures with back pain, radicular pai

Traumatic abdominal wall hernia diagnosed 14 years after a bad fall with lumbar spine fracture.

UNLABELLED: Traumatic abdominal wall hernias have significant implications for patients and insurance companies, especially when not been discovered at the time of trauma. We present the case of a gardener who sustained a bad fall during work in 1994 with immediate admission to the hospital for treatment. A fracture of the second lumbar spine body has been diagnosed and stabilized operatively. Postoperative computer tomography and magnetic resonance examinations demonstrated correct healing of the fracture. Neither the pain in the sacral spine, the left leg and left lower abdominal wall nor th

Risk to neurovascular structures using posterolateral percutaneous ankle screw placement: a cadaver study.

BACKGROUND: Over the past 20 years, ankle arthrodesis with use of screw augmentation has become a popular technique to gain fusion of the arthritic ankle. The objective of this cadaver study was to identify the risks to local neurovascular structures using standard operative practices for percutaneous guide pin placement. METHODS: Nine fresh frozen cadaver limbs were used. A guide pin from the Synthes (Paoli, PA) 7.3-mm cannulated set was placed percutaneously into the distal posterolateral leg with the ankle held in neutral position. A layered dissection was then performed from the skin to t

Effect of progesterone on recovery from nerve injury during leg lengthening in rats.

We investigated the effect of progesterone on the nerve during lengthening of the limb in rats. The sciatic nerves of rats were elongated by leg lengthening for ten days at 3 mm per day. On alternate days between the day after the operation and nerve dissection, the progesterone-treated group received subcutaneous injections of 1 mg progesterone in sesame oil and the control group received oil only. On the fifth, tenth and 17th day, the sciatic nerves were excised at the midpoint of the femur and the mRNA expression level of myelin protein P0 was analysed by quantitative real time polymerase c

Promotion of regenerative processes in injured peripheral nerve induced by low-level laser therapy.

OBJECTIVE: This study aimed to assess in vitro the influence of low-level laser therapy (LLLT) on the regenerative processes of a peripheral nerve after trauma. BACKGROUND DATA: In peripheral nerve injury initiated after severing due to accident or by a surgeon during operation, photomodulation by light in the red to near-infrared range (530-1000 nm) using low-energy lasers has been shown to accelerate nerve regeneration. METHOD: Twenty-four New Zealand adult male rabbits were randomly assigned to two equal groups (control and laser-treated). General anesthesia was administered intramuscular

[Exertion-induced compartment syndrome of the peroneal space after bowling : correction by transfer of the posterior tibialis muscle].

A 30-year-old man presented several times with pain in the calf after twisting his foot and injuring it during bowling. It was some time before the diagnosis of acute exertion-induced compartment syndrome was made, and an immediate fasciotomy could no longer prevent necrosis of the peroneal muscles. Because of the patient's drop-foot, we performed a tibialis posterior tendon transfer, fixing it to the cuneiform bone. The sort of compartment syndrome observed after engagement in various sports is a recognised, albeit uncommon, variant of acute compartment syndrome. A high level of suspicion and

[Possible nerve injuries in ankle dislocations. Trimalleolar fracture including the proximal fibula].

We present a patient with an exceptional fracture type--a Maisonneuve fracture associated with a lateral ankle fracture--and the therapy of this injury. The Maisonneuve fracture consists of a proximal fibular fracture which is associated with failure of the deltoid ligament or medial malleolus and diastasis of the distal tibiofibular syndesmosis. Diverse nerves of the lower leg can be affected by severe trauma as well as by fracture-associated complications or an operation. Fracture fragments, an excessive distension caused by the trauma, or a fracture-associated hematoma can cause injury to t

Parallel facilitatory reflex pathways from the foot and hip to flexors and extensors in the injured human spinal cord.

Spinal integration of sensory signals associated with hip position, muscle loading, and cutaneous sensation of the foot contributes to movement regulation. The exact interactive effects of these sensory signals under controlled dynamic conditions are unknown. The purpose of the present study was to establish the effects of combined plantar cutaneous afferent excitation and hip movement on the Hoffmann (H) and flexion reflexes in people with a spinal cord injury (SCI). The flexion and H-reflexes were elicited through stimulation of the right sural (at non-nociceptive levels) and posterior tibia

Microsurgical treatment of injury to peripheral nerves in upper and lower limbs: a critical review of the last 8 years.

Nerve injuries to the upper and inferior limbs represent a common event due both to home and working accidents. Minor traumas can lead to severe disabilities if a wrong treatment is carried out. From 1997 to 2005, 920 patients were observed with a total of 1,200 major or minor nerves injuries operated. In 852 cases acute lesions treated. In 68 patients we reviewed old injuries. In 707 patients direct nerve suture and in 145 patients nerve grafts were carried out. One to 8 years follow up observed. Different protocols were adopted according to the injury: simple cut, complex lesion or surgical

Surgical anatomy and accuracy of percutaneous achilles tendon lengthening.

BACKGROUND: Percutaneous Achilles tendon lengthening is frequently done to treat gastrocsoleus equinus contracture. To our knowledge, no study has documented the proximity of tendinous or neurovascular structures to the nearest edges of each hemisection in a percutaneous Achilles tendon lengthening, the complication rates related to injury of such structures, or the Achilles tendon rupture rates from inaccurate cuts. Thus, our goal was to document these distances and determine the accuracy of this procedure. METHODS: We performed triple-hemisection percutaneous Achilles tendon lengthening (Ho

Below-knee amputation using a medial saphenous artery-based skin flap.

BACKGROUND: A modified below-knee (BK) amputation with the medial saphenous artery-based skin flap coverage was designed to preserve a functional BK stump for those who were unable to receive the conventional long posterior flap or skew-type amputation. METHODS: In designing, the medial skin flap was outlined with the margins beginning 1 to 2 cm medial to the tibial crest to close to the middle of the posterior calf, with the length of the flap being equal to the transverse diameter of the leg at the anticipated level of bone section. The posterior margin of the flap was placed close to the m

Management of subtrochanteric fractures in skeletally immature adolescents.

BACKGROUND: There have been no studies that have specifically looked at the management of subtrochanteric femoral fractures in skeletally immature adolescents. It was the purpose of this study to investigate the treatment of this injury in this unique patient population. METHODS: A retrospective review of all subtrochanteric femoral fractures treated at a major pediatric trauma center since 1990 was performed. RESULTS: There were 13 adolescents with an average age of 13 years and 6 months. The average length of follow-up was 2 years and 3 months. Eight of the 10 operatively treated patients

Quality of life assessment in patients undergoing nucleoplasty-based percutaneous discectomy.

OBJECT: Nucleoplasty is a minimally invasive surgical procedure for disc decompression developed to treat patients with symptomatic contained herniated discs. Nucleoplasty uses nonheat-driven radiofrequency energy to ablate and coagulate the disc nucleus via a percutaneous "discography" trajectory under fluoroscopic guidance. In this study the authors evaluated pain, functioning, and quality of life (QOL) in patients with radicular leg and back pain who underwent nucleoplasty-based percutaneous disc decompression. METHODS: The study was designed as a prospective nonrandomized longitudinal coh

[Traumatic knee dislocation with popliteal vascular disruption: retrospective study of 14 cases].

PURPOSE OF THE STUDY: Complex femorotibial dislocation of the knee joint generally results from high-energy trauma caused by a traffic or a contact sport accident. Besides disruption of the cruciate ligaments, in 10-25% of patients present concomitant palsy of the common peroneal nerve and more rarely disruption of the popliteal artery. The purpose of this work was to assess outcome in a monocentric consecutive series of knee dislocations with ischemia due to disruption of the popliteal artery and to focus on specific aspects of management. MATERIAL AND METHODS: This retrospective series incl

Neurovascular compression following isolated popliteus muscle rupture: a case report.

This case report concerns an unusual complication of neurovascular compression following an isolated popliteus muscle rupture. A 59-year-old man, after a fall from a horse, gradually developed symptoms of a swollen leg, dysesthesias in the sole of his foot, and muscle weakness of his toe flexors. At presentation, he was found to have a complete tibial nerve injury at the level of the popliteal fossa and significant neuropathic pain. MRI demonstrated a rupture in the muscular portion of the popliteus muscle with extensive edema and hemorrhage compressing the tibial nerve in the popliteal fossa.

Microsurgical treatment of lumbosacral plexus injuries.

Surgical treatment of lumbar and sacral plexus lesions is very rarely reported in the literature. The incidence of the involvement of these nervous structures in traumatic lesions of different etiology is probably much higher than believed, and surgical treatment should be taken into consideration more often. In this paper the experience derived from the surgical treatment of 15 cases is reported. Different surgical approaches have been employed according to ethiology, to level of nerve lesion and concomitant lesions of other organs. Patients who suffered a lesion in the lumbar or sacral plexu

Postmortem serum protein S100B levels with regard to the cause of death involving brain damage in medicolegal autopsy cases.

The protein S100 is an acidic calcium-binding protein, and the subunit S100B is the most abundantly found in the brain. The aim of the present study was a comprehensive analysis of serum S100B levels in medicolegal autopsy cases (within 48 h postmortem, n = 283) including victims with head and non-head injuries and also non-injury fatalities with regard to the cause of death involving brain damage. The serum level was usually higher in the subclavian vein than in the right heart and external iliac vein, and the lowest in the left heart blood, showing no significant postmortem influence. The se

[Gunshot injuries of peripheral nervous system: the questions of classification and diagnostics].

Two hundreds and sixty-seven patients with gunshot injuries of 394 nerves and plexus underwent clinical and electromyographic investigation and H-reflex evaluation as well. Based on the results of the data obtained, 3 types of nerves trunk injuries: neuroapraxia, axonothmesis and neurothmesis, were identified. The reduction of biopotential amplitude and synergic muscular activity, decrease of impulse conduction velocities (ICVeff--up to 30%), M-response amplitude (up to 50% and more) and motor units functioning were characteristic of neuroapraxia of nerve trunks. Axonothmesis of nerve trunks f
