[Surgical treatment of extensive defects of the tissues and trophic ulcers in the area of the heel tendon using an L-shaped flap].

Operations were conducted on 22 patients with defects in soft tissues and trophic ulcers of cicatrix in the region of the calcaneal tendon. In 16 patients with extensive cicatricial lesions occupying two distal thirds or the whole region, an L-shaped flap of skin and fat was used, it was formed on the postero-lateral distal third of the leg, in the zone of the lateral malleolus and the lateral surface of the foot. The flap measured 20-22 cm in length and 6-7 cm in width. The ratio of the vertical part of the flap to the horizontal part was 3:1, 2:1, or 1:1 depending on the level of the defect

Nerve conduction studies in experimental non-freezing cold injury: II. Generalized nerve cooling by limb immersion.

After immersion of the hind limb of the rabbit, up to the lower thigh, in a waterbath, at 1 degree C for 10 to 14 hours under light anesthesia, there was evidence of persistent nerve damage to the tibial nerve, which varied in severity in different animals. Nerve conduction studies, carried out within 24 hours of removal from the bath, showed that in a proportion of the motor and/or afferent fibers, there was conduction failure between the knee and ankle. This was followed, over the next 48 hours, by distal degeneration of the affected fibers. No persistent conduction block was seen. After dis

[Detection and peripheral or central localization of sensory pathway involvement of the lower limbs by somatosensory evoked potentials].

In the presence of more or less atypical sensory or sensorimotor symptoms the questions that arise most frequently concern the authenticity of the disorders and the precise level of the lesion. In this study, somatosensory evoked potentials (SEPs) to stimulation of the tibial nerve at the ankle were recorded at different levels in 35 healthy subjects and 32 patients with sensory disorders. Recording electrodes were placed at the popliteal fossa (peripheral sensory nerve conduction velocity), at the T12-L1 level (medullary potential: N21) and at the vertex (P40 wave). The spine to cortex time i

A clinical syndrome of rostral and caudal spinal injury: neurological, neurophysiological and urodynamic evidence for occult sacral lesion.

Patients with spinal cord injury show upper motor neuron dysfunction below the level of the lesion. Some patients with cervical and high thoracic injuries show unexpected lower leg atrophy and ankle jerk abnormalities together with persistence of urinary retention. Clinical, neurophysiological and urodynamic findings in 130 patients with cervical and thoracic injuries showed that 18 patients had additional lumbosacral dysfunction. Three patients had radiological findings demonstrating a second lesion of the lower spine. The remaining 15 patients, however, did not have any obvious bony lesion t

Somatosensory evoked response evaluation of cervical spondylytic myelopathy.

There were 13 patients with cervical spondylytic myelopathy (CSM) evaluated. All had extradural defects with distortion of the cervical cord and partial or complete obstruction of myelographic dye. Posterior tibial scalp (SSEPs) were absent or delayed in all 13, whereas median SSEPs were abnormal in 9. Median SSEP abnormalties were limited to those with cord lesions at C5-C6 or above. There were eight patients with associated radiculopathies confirmed by electromyography. No meaningful differences in SSEPs were noted between those with or without root injury. Surgery confirmed the level of cor

The use of electromyography in angiology.

The author of the present study examined electric activity of the muscle in three types of chronic arterial occlusions, i.e. in obliterating atherosclerosis, obliterating thromboangiitis and diabetic angiopathy. He obtained two different electromyographic patterns. One, present in patients suffering from obliterating atherosclerosis, showed signs of a peripheral nerve-damage (characterized by broad action-potentials) and another, found in patients with obliterating thromboangiitis and in diabetics suffering from diabetic angiopathy, with a pattern corresponding to a combined injury--of impaire
