Secondary Limb-Salvage Fifty Years After Vascular Injury: A Case Report.

A 73-year-old veteran presented to vascular surgery with a nonhealing left lower leg wound. Fifty years before the presentation, he sustained a gunshot wound to the left leg during combat. Emergent superficial femoral artery to popliteal artery bypass using vein graft and complex bone and soft-tissue repair resulted in successful primary limb salvage. He was entered into the Vietnam Vascular Registry at the time of his initial injury. On current presentation, he was diagnosed with infected myositis ossificans and bypass occlusion. He was originally offered wound care alone with possible amputa

Peroneal Artery Danger Zone With Syndesmotic Screw Fixation: A Computed Tomography Angiography Study.

BACKGROUND: Additional trans-syndesmotic screws (TSS) are used in "fibula pro-tibia" approach for unstable ankle fractures applied to high-risk patients (diabetes, smoking, osteoporosis, obesity). These patients are at higher risk of complications. The peroneal artery (PA) may be at risk with additional TSS fixation. Iatrogenic vascular compromise may explain these postoperative complications. Therefore, this study investigates the range in which the PA and its deep perforating branch (dPA) are at risk with TSS fixation and identifies safe zones to avoid iatrogenic injury in this vulnerable po

Integration of arterial and angiosome injury into computed tomography (CT) soft-tissue zone of injury models for open OTA 42A-C tibia fractures.

PURPOSE: To evaluate whether incorporation of arterial injury into CT-based zone-of-injury (ZOI) models improves complication prediction after open tibial shaft fractures. METHODS: A retrospective cohort study was conducted at an urban multicenter academic hospital (2012-2024). Patients ≥ 18 years with open OTA 42A-C tibia fractures, preoperative CT, and ≥ 6 months follow-up were included. Arterial injury and wound location within the anterior tibial, posterior tibial, or peroneal angiosomes were identified on CT angiography. Soft-tissue ZOI (longitudinal extent of soft-tissue air) and fractu

Migration of Spinal Hardware Into the Popliteal Vein: A Case Report.

CASE: A 41-year-old woman with prior T12-L4 instrumentation for adolescent lumbar scoliosis presented with left posteromedial thigh pain initially diagnosed as an adductor strain. Two years later, she re-presented with knee stiffness, lower leg mottling, and paresthesia suspected to be reflex sympathetic dystrophy. Radiographs later revealed a 13.6-cm radiopaque object in the popliteal fossa and rod absence from the lumbar spine. The spinal rod was removed from the popliteal vein by orthopaedic surgery. CONCLUSION: Radiographs are an easily obtained, low-cost, and high-yield resource that are

[Research progress in diagnosis and treatment of multiple ligament knee injuries].

OBJECTIVE: To summarize the research progress in the diagnosis and treatment of multiple ligament knee injuries (MLKI). METHODS: Domestic and international literature related to MLKI in recent years was retrieved, and a systematic summary and analysis were conducted from the aspects of etiology, classification, diagnosis, treatment, and rehabilitation. RESULTS: High-energy trauma is the main cause of MLKI, while low-energy and ultra-low-speed injuries also require attention; ultra-low-speed injuries are more common in obese individuals with a higher risk of complications. The Role of Completion Angiography for Bypass to Tibial and Pedal Arteries.

INTRODUCTION: Femorotibial or pedal bypass (FTPB) for chronic limb threating ischemia (CLTI) is a technically demanding intervention. Despite the standardization of several technical and procedural details, the role of completion angiography (CA) is still a matter of debate. The present study aims to evaluate the role of CA in FTPB. METHODS: All consecutive patients treated with FTPB were prospectively collected from 2016 to 2024 into a dedicated database evaluating clinical, anatomical, surgical characteristics and medical therapies. Patients considered for the study were treated for CLTI wi

Genicular nerves: ultrasound anatomy, scanning techniques, and clinical implications.

The genicular nerves are sensory nerves that innervate the knee joint, playing a crucial role in pain transmission. Because of their small size, they are frequently underappreciated and overlooked during routine ultrasound examinations. With the advent of high-resolution ultrasound, these nerves can be visualized directly, facilitating targeted interventions. A detailed understanding of genicular nerve anatomy, ultrasound landmarks, and characteristic imaging appearances has significant clinical implications for managing knee pain. Given the high prevalence of chronic knee pain due to osteoart

Concurrent Intramuscular Thigh Hematoma and Femoropopliteal Venous Thrombosis During Supratherapeutic Warfarin Anticoagulation.

An anticoagulated patient with unilateral thigh swelling may have more than one process. A 79-year-old man receiving long-term warfarin for atrial fibrillation and a previous ischemic stroke presented with 3 days of atraumatic left-thigh pain, swelling, and inability to walk. His international normalized ratio was 6.41. Computed tomography (CT) showed an approximately 4 cm × 7 cm intramuscular hematoma in the left vastus intermedius. Duplex ultrasonography additionally demonstrated femoropopliteal venous thrombosis causing partial occlusion, with absent spectral flow in the sampled femoral vei

Ensemble machine learning-based scaling up of wetland carbon dynamics across the Southeastern U.S.

BACKGROUND: Although the Southeastern United States (SE US) has been identified as a major terrestrial carbon sink, the balance of CO2 and CH4 fluxes from its wetlands remains poorly constrained due to strong environmental heterogeneity and limited regional-scale assessments. In this study, we used observations from 24 wetland sites located in the SE US, including freshwater and salt marshes, to train machine learning models (Random Forest, XGBoost, Support Vector Machine, Artificial Neural Network, and Partial Least Squares Regression) to predict monthly mean gross primary productivity (GPP),

PBRM1 knockdown is associated with NRF2‑dependent ALDOA upregulation and metabolic reprogramming in HCCC‑9810 intrahepatic cholangiocarcinoma cells.

IMPORTANCE: The coronavirus disease 2019 (COVID-19) was the third leading cause of mortality in the United States for three years in a row. The genetic contributions to disease severity remain unclear and many previously identified single nucleotide polymorphisms (SNPs) have not been replicated nor linked with functional significance. OBJECTIVE: To identify SNPs associated with mortality among hospitalized COVID-19 patients supplemented by expression quantitative trait loci (eQTL) evidence to infer plausible functional mechanisms related to COVID-19 severity. DESIGN: A quality-controlled gen

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Epidemiology of Vascular Injury Within the U.S. Department of Defense Trauma Registry From 2007-2023.

Background: Since WWII, the incidence of vascular trauma in U.S. military casualties has steadily risen, with the highest rates having been reported from the recent conflicts in Iraq and Afghanistan. Within civilian populations, the incidence of vascular trauma is lower, but with disproportionately higher rates of mortality, morbidity, and resource use for these injuries. Analyses of trauma databases have informed technological advances that best help our U.S. Military Health System (MHS) manage vascular injuries. We sought to perform a retrospective cohort analysis in military casualties with

Risk Factors and Timing of Fasciotomy for Isolated Pediatric Lower Extremity Trauma.

BackgroundPrevious single-center reports have demonstrated a longer time from traumatic leg injury to fasciotomy in pediatric trauma patients (PTPs) with compartment syndrome. We hypothesized most fasciotomies in isolated pediatric lower extremity trauma (LET) are delayed (>6 hours from admission) and sought to investigate risk factors for pediatric fasciotomy.MethodsThe 2017-2020 TQIP database was queried for PTPs <17 years old with isolated injury to the leg. The primary outcome was fasciotomy. A multivariable logistic regression analysis was performed to identify associated risk factors for

Damage control strategy in peripheral vascular injury caused by firearm: A case report.

In peripheral vascular injury, the need for amputation following prolonged ischemia can be avoided through rapid revascularization. However, critically ill patients often cannot undergo single-stage surgery; thus, the use of vascular shunts temporarily alleviates ischemia, protecting the limb until definitive treatment. The present study describes the case of a 28-year-old male patient with a vascular injury in the right leg secondary to a firearm wound. Following admission, he developed grade IV hypovolemic shock. The patient underwent surgery, where a complete transection of the popliteal ar

Unilateral Acute Compartment Syndrome Following Bilateral Total Knee Arthroplasties: A Case Report.

CASE: A 55-year-old man developed unilateral acute compartment syndrome (ACS) within 3 hours after bilateral total knee arthroplasties (TKA). Timely fasciotomies, followed by primary closure (within 48 hours), resulted in full neurological recovery within 2 weeks. By 1 year postoperatively, the patient had excellent range of motion (0-120° bilaterally) and patient-reported outcomes (KSS 200 and WOMAC 88.3). We hypothesize that the ACS was both due to bleeding from a small branch from the popliteal artery or vein that goes into vasospasm and due to the tourniquet ischemia-reperfusion phenomenon

Popliteal Venous Aneurysms: A Systematic Review of Treatment Strategies and Outcomes.

Background: Popliteal venous aneurysms (PVA) are an uncommon but potentially severe condition due to their association with increased risk of recurrent pulmonary embolisms. Because of their rarity, their aetiology, natural history, and optimal treatment strategies have been poorly defined. The aim of this paper is to report a comprehensive systematic review on the treatment strategies and outcomes in PVA, summarizing current evidence. Methods: A systematic literature search was conducted in PubMed, Scopus, and Web of Science, covering studies published from database inception through February
