Using 320-slice computed tomography to preoperatively investigate the leg perforator arterial system and design a perforator flap for patients with a soft-tissue defect in the leg.

PURPOSE: To investigate the leg perforator arterial system, identify the perforator flap's pedicle artery and its projected cutaneous point using a 320-slice computed tomography (CT 320) scanner. METHODS: A total of 24 patients with leg soft-tissue defects unilaterally underwent 320-slice CT angiography scanning (CTA 320) with 47 legs. The used method enabled investigation of the perforator arteries originating from the tibial, peroneal arteries, perforator flap's pedicle artery and its projected cutaneous point. These data were used to preoperatively design an improved flap. Then, the CT-con

A Hybrid Approach That May Facilitate the Repair of Traumatic Popliteal Artery Pseudoaneurysms.

PURPOSE: Traumatic popliteal pseudoaneurysms may occur in accidents or as an iatrogenic complication of a total knee arthroplasty. Complications often arise in open repair because of distorted anatomy. Up to 22% of the patients may suffer above-knee amputation. Endovascular treatment has proven to be an effective solution. However, the long-term performance of stents at the hinge point of the popliteal artery is questionable. We present a hybrid technique that was used successfully in 2 cases. TECHNIQUE: Our approach takes advantage of both open and endovascular techniques. At first, we apply

Effect of Anatomic Variations in the Anterior Tibial Artery on Risk of Injury During Orthopaedic Knee Surgeries.

BACKGROUND: Injury to the posterior vasculature is a potential complication in orthopaedic knee surgery that may be associated with variations in its anatomy, such as the type II-A2 variant, which places the anterior tibial artery (ATA) in closer proximity to the tibia. However, how close surgical instrumentation comes to injuring the ATA is not well described. PURPOSE: To determine how the type II-A2 variant of the popliteal vasculature affects proximity of the ATA to instrumentation for orthopaedic knee procedures. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A tota

Vascular ultrasound diagnosis and clinical implications of aberrant anterior tibial artery: case report.

The lower limb is vascularized by the femoral artery, which continues as the popliteal artery. After the distal margin of the popliteus muscle, the popliteal artery divides into the anterior and posterior tibial arteries. Anatomical variations in the bifurcation of the popliteal artery are frequent. One such variation is the aberrant anterior tibial artery, in which the artery runs anterior to the popliteus muscle instead of following the normal dorsal course. We report a case in which this anatomical variation was diagnosed with vascular ultrasound in a patient complaining of intermittent cla

"Pure Fat Flap"-Perforator-based Adiposal Layer Only Flap for Lateral Ankle Reconstruction.

Lateral ankle soft tissue defects pose challenges, especially in cases due to chronic pressure from cross-legged sitting, which usually present with a large dead space, small skin opening that often accompanies an open joint. Traditional reconstruction methods using fasciocutaneous flaps may result in donor site morbidity such as delayed wound healing or nerve injury. In this article, we present a case of diabetes-related lateral ankle defect successfully treated using adiposal layer only flap, also known as pure fat flap. The anatomy and the surgical technique of adiposal layer only flap were

Chimeric Free Flaps Based on End-to-Side Anastomosis for Complex Lower Extremity Reconstruction.

BACKGROUND: Complex lower extremity defects are difficult to cover and often require multiple free tissue transfers. Chimeric anterolateral thigh free flaps (ALTF) and peroneal artery perforator free flaps (PAPF) have been designed specifically as an alternative for reconstruction with arterial end-to-side (ETS) anastomosis. We aimed to assess our institutional experience with this technique and to define its role in complex lower extremity reconstruction. METHODS: From July 2012 to March 2023, 27 patients with complex lower extremity defects underwent reconstruction with chimeric ALTF or PAP

Staged treatment with arterial reconstruction and flap transplantation for ischemic mangled extremity: An observational study.

Patients with traumatic ischemic mangled extremities first undergo arterial reconstruction using autogenous vein grafts, followed by flap transplantation as a staged treatment for soft tissue reconstruction. This study aimed to report the outcomes of such a staged treatment. Thirteen patients underwent arterial reconstruction between February 2015 and April 2017 due to damage to the major blood vessels by the traumatic mangled extremities. Of them, 6 patients (5 males and 1 female with a mean age of 51 years, age range: 36-60 years) who underwent soft tissue reconstruction due to necrosis were

Lateral Meniscal Tears Can be Safely Repaired "All-Inside" Without Peroneal Nerve Injury: Case Series and Literature Review.

INTRODUCTION: The menisci are a pair of fibrocartilaginous structures important for the normal biomechanical function of the knees. Tears are common, and multiple approaches have been used to repair meniscal tears. Of the approaches, the all-inside approach has historically been avoided for tears to the posterior aspect of the lateral meniscus (LM) due to the risk of popliteal and common peroneal neurovascular injury. CASE REPORT: We reviewed the electronic medical records of all patients who underwent all-inside LM repair by our senior author from August 2022 through August 2023. Demographic

Ultrasound imaging of the posterior lateral corner of the knee: a pictorial review of anatomy and pathologies.

Assessment of the posterior lateral knee pain poses diagnostic challenges, requiring accurate evaluation of various structures in light of the medical history and physical examination. Despite substantial progress in the ultrasonographic diagnosis of musculoskeletal disorders, the current protocol (EURO-MUSCULUS/USPRM. Basic scanning protocols for knee) fails to conduct a comprehensive investigation into the intricate, tendons, and ligaments of the posterior lateral knee. This pictorial review aims to bridge this gap by offering a systematic approach to utilize ultrasound examination of the le

Popliteal deep vein thrombosis in a pediatric patient with Streptococcus viridans osteomyelitis: a case report.

An 11-year-old male presented with severe pain in the left knee after a minor twisting injury. Magnetic resonance imaging (MRI) revealed a large fluid collection posterior to the distal femur. CT and intraoperative findings revealed left popliteal vein thrombosis. Irrigation and debridement were performed, and intraoperative cultures confirmed osteomyelitis due to pan-sensitive Streptococcus viridans (S. viridans). Treatment included antibiotic and anticoagulation therapy. At 6 months, the patient was asymptomatic and cleared for full activity. This case demonstrates that deep vein thrombosis

Anatomical Structures at Risk in Percutaneous Preparation Talonavicular Fusion.

BACKGROUND: The stability of the hindfoot greatly relies on the integrity of the talonavicular joint. Pathologies affecting this joint often necessitate fusion. Our study explores the risks posed to neurovascular and tendon structures during simulated percutaneous talonavicular joint preparation for fusion. METHODS: In 9 fresh cadaver specimens, the talonavicular joint was accessed through two portals. A 2-mm Shannon burr was employed for joint surface preparation with distraction provided by a pin-based distractor. Dissections were performed to assess potential damage to critical structures,

Navigating Postpartum Venous Thromboembolism: A Case of Thrombophilia, Bleeding Complications, and Chronic Inferior Vena Cava Syndrome.

Postpartum venous thromboembolism (VTE), encompassing deep vein thrombosis (DVT) and pulmonary embolism (PE), is a critical complication occurring in the postpartum period. The pathogenesis involves a hypercoagulable state induced by pregnancy-related physiological changes, venous stasis from reduced mobility and pelvic compression during delivery, and endothelial injury. Postpartum VTE is a leading cause of maternal morbidity and mortality, necessitating heightened clinical vigilance. Understanding the risk factors, implementing prophylactic measures, and ensuring timely intervention are para

Extensive Deep Vein Thrombosis and Pulmonary Embolism in a Young Patient With Heterozygous Factor V Leiden Mutation and Antiphospholipid Syndrome.

Although thrombotic events are uncommon in young individuals, patients with genetic mutations in coagulation factors may develop extensive multisite thrombosis. We present the case of a 26-year-old patient, a smoker for nine years, who was admitted to the hospital complaining of right thigh pain with swelling, right flank abdominal pain, dyspnea, and hemoptysis. A medical history provided by the patient indicated that one month prior to presentation, an accidental fall had resulted in multiple rib fractures, bilateral hemopneumothorax, and pneumomediastinum. These injuries were treated with bi
