Combined skeletal and vascular injuries of the lower extremities.

In order to determine the long-term results of surgical treatment in patients with significant combined skeletal and arterial injuries, the authors reviewed the records of those patients treated for this injury between 1970 and 1981, at their institutions. These cases were confined to fractures and/or dislocations of the femur, knee, and tibia which were associated with vascular injuries. Thirty-one patients with 32 injured extremities fit these criteria for our review. The distribution of the orthopedic injuries were as follows: femoral fractures, 16; tibial fractures, 20; and knee dislocatio

Popliteal arteriovenous fistula following meniscectomy.

A case of arteriovenous fistula between the popliteal artery and vein after injury during medial meniscectomy is presented. The diagnosis was made on the fifth postoperative day by angiography, and a successful reconstruction of the artery and vein was accomplished by primary anastomosis of the artery following resection of the damaged part; the vein was closed by lateral venorrhaphy.

[Popliteal vascular injuries. Prognostic classification].

Despite recent advances in the treatment of vascular injuries, the prognosis of injuries to the popliteal vessels is still poor with an incidence of 10-40 p. 100 of amputations recorded in the literature. Mortality rate is 4 p. 100. The authors have seen 13 cases and have studied seven factors influencing the prognosis - the duration of ischaemia, the general condition of the patient, the type of lesion, the state of the veins, the severity of the ischaemia, the level of the lesion and associated soft tissue injury and wounds. Each of the parameters has been assessed with a grade proportional

[Injuries of the major blood vessels in joint dislocations].

Experience with the treatment of 12 patients with injuries of major vessels resulting from dislocations of joints is generalized. In 5 of the patients an associated injury of nerve trunks was diagnosed, in 4 patients there were injuries of major veins. Reconstructive operations were made in 11 patients. In 8 patients full recovery was stated, three patients were subjected to amputation of the extremity. The main cause of the failure was late diagnosis of the closed trauma of the artery and progressing ischemia of the extremity.

[Problems with vascular end-to-end anastomoses].

Despite accurate preoperative investigation of the vessels of the lower part of the leg using angiography and plethysmography as a preparation for a free latissimus dorsi myocutaneous flap, problems can appear at the anastomosis during the operation. In the reported case using a free latissimus dorsi myocutaneous flap for coverage of a tissue defect at the lower leg, thrombosis appeared due to a difference between the calibers of the vein graft and the thoraco-dorsal artery. The existing difference of the calibers was therefore equalized by controlled narrowing of the vein graft. Thereby paten

Latissimus dorsi myocutaneous free flap vascularised by an extra-anatomic graft in the lower limb following traumatic disruption of the popliteal artery.

A case is presented in which the thoracodorsal artery of a latissimus dorsi free flap was anastomosed to an autogenous reverse saphenous bypass graft. The bypass graft was used to revascularise a leg with traumatic disruption of the popliteal artery and an open comminuted fracture of the distal tibia and fibula with extensive soft tissue loss. At 2-year follow up, isotope scans showed patency of the extra-anatomic graft and a good flow into the thoracodorsal artery.

[Extra-anatomical femoropopliteal shunt].

The construction of an extra-anatomical femoropopliteal shunt resulted in the saving of a lower limb following a gunshot wound to the upper third of the thigh involving vascular lesions and extensive tissue loss. By applying this technique it was possible to obtain revascularization of the limb without the septic risks of reconstruction in situ, with the attendant difficulty of initial skin coverage of the wound.

Improved limb salvage in popliteal artery injuries.

This study reviews the recent experience with popliteal artery injuries at the Massachusetts General Hospital. Twenty-two patients suffered 24 injuries. The overall limb salvage was 83%. Blunt trauma accounted for 19 of the cases and was associated with femur fractures, knee dislocations, and tibia-fibular and plateau fractures: four amputations (21%) resulted. There were five penetrating injuries from three gunshot wounds, one stab wound, and one laceration: no amputations occurred. The major factor in the amputated limbs was delay in diagnosis and therapy of the arterial injury associated wi

The continuing challenge of popliteal vascular injuries.

During a 6 year period, 35 patients with 56 popliteal vascular injuries were treated. Thirty-three arteries and 23 popliteal veins were affected. Fifty-four percent of the patients had both an arterial and a venous injury. Twenty injuries were due to penetrating trauma and 15 injuries to blunt force. An overall amputation rate of 16 percent followed attempts at vascular repair. Blunt injuries were associated with a 30 percent amputation rate, whereas penetrating injuries were associated with only a 5 percent amputation rate. When our results were reviewed and compared with those of others, sev

Arterial injuries below the knee: fifty-one patients with 82 injuries.

Clinical features of tibial and peroneal artery injuries are characterized in a review of 51 patients with 82 injured arteries. Injuries were penetrating in 34 (67%) patients and blunt in 17 (33%). Physical findings suggested arterial injuries in 42 (82%) patients, but nine had no signs of vascular trauma. Nineteen patients with no palpable distal pulses had average preoperative delays of nearly 5 hours, suggesting a lack of appreciation for the morbid potential of these injuries. Operations included ligations of single-vessel injuries and arterial reconstructions in the 21 (41%) patients with

Traumatic false aneurysm of the ankle. A case report.

A 27-year-old man incurred a false aneurysm of the peroneal artery secondary to an inversion injury to the ankle. A search of the literature disclosed no other cases of false aneurysms with ankle sprains. The false aneurysm of the peroneal artery appeared as a mass with a compressive neuropathy of the sural nerve. The diagnosis of false aneurysm of the peroneal artery following an inversion injury to the ankle should be suspected in cases of persistent localized swelling with inordinate pain that does not subside with elevation and immobilization and is associated with peroneal neuropathy.

The surgical treatment of blunt and penetrating injuries of the popliteal artery.

Thirty-six patients sustaining blunt and penetrating injuries to the popliteal artery underwent vascular reconstruction with an average ischemic interval of eight and one-half hours (range: four to 14 hours). There were 16 associated venous injuries, 15 nerve injuries, and 20 fractures with or without knee dislocations. Arterial repair was performed by saphenous vein interposition graft in 23 patients and by end-to-end anastomosis in 16 patients (three patients had more than one operation). Eight of the venous injuries were reconstructed and the remaining eight veins were ligated. Fasciotomy w

[Epiphyseal detachment of the proximal end of the tibia in a child with a vascular lesion. Apropos of a case].

The authors report a case of lesion of the popliteal artery associated with a fracture involving separation of the upper end of the tibia in a child following a serious accident. This lesion was treated by resection-vein graft after reduction of the fracture and immobilisation by two pins. In the light of this case, the authors undertake a review of the world literature, then assess the mechanism and the bone and vascular lesions, the various pathological classifications and the aetiologies. Finally, they emphasize the difficulty in diagnosing the bone lesion and, above all, the vascular lesio

Injury to the popliteal artery.

During a 32 year period, 164 patients with 165 popliteal artery injuries were treated. One hundred twenty-five injuries were due to penetrating trauma, and 40 to blunt force. During the first decade reviewed, with ligation the main method of management, the amputation rate was 74 percent. Almost routine attempts at vascular repair over the ensuing 10 years reduced the amputation rate to 28 percent. During the final 12 years, six amputations were required for 81 injuries, thereby producing an amputation rate of only 6 percent. From this experience, the following principles of management have ev

Principles and indications for primary venous repair.

The management of injured veins continues to represent a formidable and stimulating challenge. While there has been past resistance to repair injured veins, and the controversy persists for some, an increasing number of reports emphasize that a more aggressive approach to the repair of injured veins is warranted, particularly in the lower extremities. The recent experience in Southeast Asia, as outlined in reports from the Vietnam Vascular Registry, has demonstrated that some of the previous fears of an increased incidence of thrombophlebitis or pulmonary embolism with attempted venous repairs

Vascular injuries near the knee: an updated series and overview of the problem.

Injury of the popliteal artery results in amputation more often than any other arterial injury. Adjacent injuries, small vessel thrombosis, and muscle necrosis are the major deterrents to limb salvage. This report updates a previously published series, expanded to include 110 injuries treated during a 14-year period. Emphasis is placed on the management of adjacent injuries, errors promoting amputation, and principles of improving limb survival. The hallmark of success is the rapid and complete restoration of arterial and venous flow.

Injury to popliteal vessels.

Between 1969 and 1981, 61 patients with 85 popliteal vascular injuries were seen. In the nine patients who required amputation, common risk factors included (1) extensive time delays (greater than 36 hours) between injury and repair, (2) associated bone and soft-tissue injuries leading to postoperative wound infection. amd (3) early occlusion of popliteal artery repair or delay in performance of fasciotomy. The low amputation rate in patients without operative delay or associated bone and soft-tissue injuries (2/54 = 3.7%) was attributed to the careful application of standard vascular surgery
