Contusive trauma of the popliteal artery.

The writers have reviewed thirteen cases of contusive trauma of the popliteal artery which came under their observation. The incidence of such lesions in arterial trauma is 22 per cent. The limb was saved in 76 per cent; but the result was only functionally good in only 46 per cent. The fundamental factors for successful revascularization are early diagnosis and precise technique. The operation of choice is a by-pass using the saphenous vein from the contralateral limb; this "jumps" right over the focus of arterial contusion.

Management of vascular injuries to the leg.

Our experience with 69 vascular injuries in 56 patients led us to modify the management of vascular injuries to the leg. We believe that prompt and complete angiography whenever the general condition of the patient allows it, early fasciotomy when indicated before vascular repair, thrombectomy of the injured artery and vein and local instillation of heparinized saline solution, vascular repair before orthopedic stabilization of fractures in selected cases, external fixation of the fracture when there is significant soft tissue injury, and early skin grafting resulted in an improved level of ca

Management of traumatic peripheral vein injuries. Primary repair or vein ligation.

The management and outcome of 83 patients who had 86 venous injuries were retrospectively reviewed to identify optimal management techniques in patients with peripheral vein injuries. Venous injuries of the arms were associated with no long-term sequelae, and management with vein ligation appears safe. In patients with venous injuries of the legs, primary repair by lateral suture or primary end-to-end reanastomosis is recommended when technically easy. In patients who are unstable or in whom primary repair cannot be performed, vein ligation is recommended. Autogenous vein interposition graftin

Use of polytetrafluoroethylene grafts in elderly and high-risk patients.

Despite well established methods of vascular surgery, the elderly patient has not been considered a candidate for operation because of a high operative risk and a limited life expectancy. Simplified surgical procedures and the use of new graft material now permit vascular reconstruction with minimal injury to the tissues, little blood loss, and a short operative time. The immediate postoperative mortality after vascular surgery in 68 patients 70 to 88 years of age threatened with immediate amputation was 3%. Seventy-five percent of the limbs were intact 12 months after operation and 55% after

Nonoperative management of a traumatic peroneal artery false aneurysm.

The standard approach to management of traumatic false aneurysms is operative. This report describes successful treatment of a traumatic peroneal artery aneurysm following a gunshot wound in a 29-year-old man by an intraluminal steel coil occlusion technique. The coil was guided through the popliteal artery and aneurysm into the distal peroneal artery via a percutaneously placed catheter: following disengagement of the coil the outflow tract of the aneurysm was occluded, and another passed and released into the proximal peroneal artery occluded the inflow tract.

Peripheral nervous system complications of coronary artery bypass graft surgery.

Among 421 patients undergoing coronary artery bypass graft surgery, 55 (13%) developed 63 new peripheral nervous system (PNS) complications postoperatively. Most common was a brachial radiculoplexopathy, which occurred in 23 patients. Of these, 21 involved lower trunk or medial cord fibers. In 17 there was a correlation between the site of jugular vein cannulation and the side affected, suggesting that needle trauma played a role. Stretching from chest wall retraction may have caused some cases. Other deficits included 13 saphenous, 8 common peroneal, and 5 ulnar mononeuropathies. Six patients

Tibial artery injuries.

Twenty-eight consecutive patients with tibial or peroneal artery injury are reported. Of these, eight underwent direct arterial repair, eight received vein graft reconstruction, and the involved artery was ligated in the remainder, with amputation required for various reasons in four. Tibial or peroneal artery injury is a relatively common problem. Successful management requires an aggressive approach to early diagnosis, which in turn requires a high index of suspicion and arteriography. Early appropriate management will prevent the serious complications of ischemia, false aneurysm, arterioven

A decade of missile-induced vascular trauma.

During 10 years of serious strife the Royal Victoria Hospital, Belfast, has dealt with trauma inflicted by a wide variety of missiles. Injuries involving 316 vessels were sustained by 192 patients. A group of "knee-cappings' accounted for a large proportion of popliteal vessel injuries. A majority of patients also suffered damage to other regions of the body. Hypotension was a notable feature and several patients arrived in extremis, particularly those with injuries to a major vessel trunks and vital organs. The benefits of rapid evacuation in relation to ischaemia time are discussed. The oper

Accidents in the surgical treatment of varicose veins.

Serious accidents in the surgical treatment of varices are quite exceptional and it is impossible to say with any accuracy how often they occur. Generally they result from major errors of surgical technique, and arise mainly from: haemorrhaging after the tearing of the femoral vein or because of the loosening of the stump of the saphenofemoral junction, traumatism of the femoral artery, damage of the nervus peronaeus in crossectomy of the saphena parva. Minor accidents are more frequent but here too it is impossible to state a precise percentage. There are four fairly common types of accident:

Popliteal artery injuries associated with knee dislocation. (Three cases treated with successful outcome).

Knee dislocation is an uncommon lesion. It is, however, frequently associated with injury to surrounding structures. The possible coexistence of trauma of the popliteal artery is a serious threat to the limb's integrity. Its incidence is high (about one third of cases) and, unless speedily discovered and treated, it leads to limb loss. Three cases of knee dislocation with combined injury to the popliteal artery successfully underwent surgery. On the basis of such experience, the authors survey the questions of vascular reconstructive surgery that still remain open: in particular, the delay in

Arterial injuries in children.

Twenty-nine children sustained major arterial injuries secondary to gunshot wounds (nine), blunt injuries (11), penetrating injuries by sharp objects (five), and arteriographic injury (four). The femoral artery was most commonly injured, followed by the brachial, the carotid, subclavian, popliteal, aorta, innominate, and vertebral arteries. We repaired the majority of the arterial injuries by resecting the damaged vessel, with primary anastomosis or interposition grafting when necessary to avoid tension. Postoperative complications included clotted grafts in two patients, which were rendered p

Complete dislocations of the knee with popliteal vascular injury.

Complete dislocation of the knee is rare, but is frequently associated with popliteal artery disruption. Prompt recognition and early revascularization are paramount for a successful and functional result. Arteriography is recommended in all cases of complete dislocation of the knee. Excision of the damaged arterial segment with saphenous vein bypass grafting is the treatment of choice. Associated venous and nerve injuries should be recognized and repaired early. Liberal use of compartment decompression procedures give better long-term results.

Extra-anatomical vascular bypass and external skeletal fixation in limb replantation.

The scientific background for limb replantation began 75 yr ago with the work of Hopfner and of Carrel and Guthrie. Thirty-five years ago, Hall indicated that the techniques were available, but it was not until 17 yr ago that Malt and McKhann performed the first successful replantation of a limb in man. Since then, the techniques have become more sophisticated and guidelines have been formulated. Many successful replantations have been reported. We present two children with partial amputation of a lower limb. Both had severe soft tissue and bone damage sufficient to contraindicate replantation

Retrograde venous bullet embolism.

Bullets may embolize against normal blood flow into distal veins because of gravity or the Valsalva maneuver. A case is described in which a bullet was fluoroscopically repositioned from the popliteal vein into the area of a lacerated iliac vein and trapped with a balloon catheter obviating popliteal exploration.

Injury to the popliteal artery.

Popliteal artery injury is a common clinical problem which has been associated with a relatively high rate of limb loss. Proper management of popliteal artery injury, which includes attention to minimizing delay, repair of venous injury, careful technique and appropriate fasciotomy can result in substantial improvement in the rate of limb salvage following popliteal artery injury.

[Post-traumatic compartment syndrome of the leg (author's transl)].

Fifteen compartment syndromes seen in the first few hours have been collected. Fourteen occurred after tibial fracture and one after a simple contusion. In typical cases it is emphasised that ischaemia affects the contents of the compartment in an insular fashion so that distal ischaemia is not present. The main arteries are sound and demonstrated by distal pulses, the vascularity of the foot, Doppler studies and arteriography. The diagnosis is difficult but must be made at the earliest opportunity by repeated clinical examination. Measurement of the compartment pressure may be useful because

Free transfer of a large sensory flap from the first web space and dorsum of the foot including the second toe for reconstruction of a mutilated hand.

A 19-year-old man sustained a mutilating crush injury in August, 1978, in an 80-ton punch press. The injury resulted in transmetacarpal amputations of the index through little fingers. The thumb was revascularized by microvascular anastomoses. The replanted index to the little finger position failed. A free transfer of the patient's second toe to the fifth metacarpal was done to provide a post to oppose the thumb. The first web space of the toe was used in expanding the contracted single cleft of the hand. The innervated dorsal foot flap provided coverage for the dorsal surface of the hand. An
