[Variations of venous lesions of lower extremities associated with varicose veins].

374 varicose extremities in 238 patients were examined. In 344 (92%) out of 374 extremities valvular incompetence and dilatation of the basal trunk of the great saphenous vein over its length were found. In 212 (56.7%) extremities dilatation of the first median anastomosis between the great and small saphenous veins was found. The upper medial inflow in the leg gets injuried in 36.1% of cases, and the superficial tibial vein in 28%. Lateral varicosity occurred in 0.5% of cases. Injuries to the lateral branches without dilatation of basal trunk constituted 5.6%. There were no cases of dilatatio

Popliteal artery injury following fracture or dislocation at the knee. Diagnosis and management.

Fracture dislocations or dislocations of the knee can produce an arterial injury that may be difficult to evaluate clinically. Aggressive diagnosis and treatment can produce satisfactory results in both vascular and orthopedic situations if performed early after the injury. Five cases of popliteal arterial injury were seen from April 1974 to August 1976. Three cases were following posterior knee dislocation, one was a femoral fracture displacement, and one was a rotary tibiofibular displacement. Four of the patients had complete popliteal artery transection and one had intramural hematoma and

Selective intra-arterial tolazoline infusion in peripheral arterial trauma.

Intra-arterial tolazoline has been used as an adjunct for angiographers and has been investigated for application in certain peripheral vascular disorders secondary to its potent activity as a vasodilator. We have had experience with three cases in which severe peripheral arterial trauma had resulted in nonviable extremities in spite of both orthopedic manipulation and vascular reconstruction. Continuous infusions of intra-arterial tolazoline were begun with marked improvement in blood flow in all extremities and resultant viability in all three within 48 hours. All three of these extremities,

[Revascularization syndrome].

The author presents two cases of serious trauma of the extremities complicated with lesions of the arteries. Both cases were treated in the cardiovascular surgery clinic in Ljublgana. In the first case besides on osteosynthesis of a fractured femur, a termoterminal anastomosis was performed on the totally severed Superficial Femoral Artery. In spite of the the Thrombectomin that was later performed on the Posterior Tibial Artery, gangrene of the foot set in, and exitus lethalis, due to a cerebral process. In the second case, besides a fractured femur, the patient had an arterial stupor of the

[Injuries of the thigh bone with a destruction of the neuro-vascular bundle].

From 1969 to 1971 the authors treated 1360 injured patients, eight of which suffered serious injuries of the thigh; fractured femur complicated by a severed Femoral Popliteal Artery, and lesions of the Sciatic Nerve. Six patients were injured at work, one in an automobile accident, and one was a gunshot victim. In approximately 1000 fractures one is complicated by an injured neurovascular bundle. The clinical picture of such a case is characteristic. The arteriograph is used only to locate the lesion. Reconstruction of the artery must be accomplished some eight to twelve hours following the in

Popliteal vessel injury.

Popliteal vessel injury still carries a high amputation rate despite the progresses which have been made in vascular surgery. Three patients with such injuries treated with vascular repair are reported and a review of the recent literature is given. Prognostic factors as early diagnosis, delay in treatment, associated injuries as fracture/dislocation and venous injury, soft tissue injuries, adequate vascular repair, and postoperative complications are discussed.

Major venous injuries and bullet wounds.

A review of the reported incidence, management and results of treatment of major vein injuries is presented and a brief account given of the mechanism of vascular damage by high-velocity bullet wounds. This paper reports 3 cases of isolated major venous injury caused by high-velocity bullets. Repair was performed in each case. Two patients survived with a good short-term clinical result. The third patient died during operation. The advantages of repairing injured veins are discussed.

The effect of acute popliteal venous interruption.

Popliteal vascular trauma continues to be associated with a relatively high morbidity rate when compared to other major vascular injuries in extremities. There is continuing controversy regarding the management of popliteal venous injuries. The advocates of ligation of injured veins have postulated that there is an increased incidence in thrombophlebitis and pulmonary embolism associated with attempted venous repair. There is a paucity of valid statistics supporting either side of this controversy. Clinical experience documented in the Vietnam Vascular Registry and experimental work at Walter

Venography in penetrating injuries of the extremities.

Of 30 consecutive cases of penetrating injuries to the extremities studied both by arteriography and venography, five were found to have significant venous injuries. Three of these cases are presented to show the value of venography in detecting inadequate venous collateral circulation, intraluminal thrombus, and venous obstruction. It is suggested that preoperative venography will compliment arteriography, aid in the diagnosis of venous injuries, demonstrate variable venous anatomy, and outline other venous pathology outside of the area of trauma.

Major peripheral veins injuries.

The injury was severe in wounded limb patients of this series who suffered from an associated major peripheral vein trauma. The presence of such an injury weighed heavily on the prognosis. Thirty eight patients with major peripheral veins injuries are reviewed. The injury had resulted from war wounds, work or road accidents. The superficial femoral vein was the most frequently injured vein. Associated injuries were frequently noted: soft tissues injuries in 35 patients, fractures in 33, arterial injuries in 32 and peripheral nerve injuries in 22 patients. Shock was more often present and more

Knee dislocation. An illusive cause of critical arterial occlusion.

Popliteal artery injury is a hazardous and often subtle complication of traumatic knee dislocation. Review of the literature in which series of knee dislocations were reported has revealed a 28% incidence of thrombosis or rupture of this critical vessel. Fifty-seven percent of these patients required leg amputation. The most frequent cause of limb loss has been absence or delay of direct surgical repair of the associated popliteal artery injury. This report describes a management plan that is intended to minimize the likleihood of future disabling leg amputations following knee dislocation.

Pediatric vascular trauma. Manifestations, management, and sequelae of extremity arterial injury in patients undergoing surgical treatment.

Twenty pediatric patients with a mean age of 9.2 years exhibited 21 injuries to arteries of an extremity and underwent operation. Included were 12 iatrogenic injuries and nine non-iatrogenic penetrating injuries. Femoral, brachial, popliteal, radial, axillary, iliac, and tibial arteries were sites of vascular trauma. Long-term follow-up, averaging 36 months, was possible in 14 patients. Postoperative scanograms, digital impedance plethysmographic studies, and bone age roentgenograms were routinely obtained. Limb length disparities greater than 5 mm, extremity blood pressure differentials great

Pediatric arterial injuries.

Forty-three children were treated for peripheral arterial injuries. Single injuries of the radial, ulnar, anterior or posterior tibial arteries with good collateral supply were best treated by ligation. Injuries about the elbow or knee, associated with fractures, carried the highest morbidity. Early arteriography, prompt exploration in doubtful cases, and a high level of expertise are essential for good results in these two areas.

Arterial injuries of the extremities.

Fifty-eight patients with arterial injuries of the extremities were treated during the past 8 years. Fifty-one had acute injuries and seven had nonacute injuries. Blunt trauma or shotgun wounds caused 74% of the injuries, and 55% were associated with skeletal trauma. All the acute injuries endangered the limb; the average ischemic time was 8.5 hours. Brachial, popliteal, and superficial femoral arterial injuries were seen most frequently. Repair was accomplished with autogenous saphenous vein grafts in 47% and end-to-end anastomosis in 41%. Six patients died, four due to injuries of other orga

Alterations in pulmonary function, coagulation and fat metabolism in patients with fractures of the lower limbs.

If the underlying pathology of the syndrome of fat embolism is the presence of circulating emboli of neutral fat with adherent platelets and red blood cells, then one could expect certain changes in normal vascular cellular elements and fat metabolism, with resultant alterations in pulmonary function. In our series of 118 patients with fractures of the lower limbs we found that more than half of them exhibited hypoxemia as well as a decrease in the hematocrit and platelet counts with a concomitant increase in platelet adhesiveness. In addition, those patients with hypoxemia showed increased fi
