Tenodesis reconstruction for chronic ankle instability: graft considerations and structures at risk with tunnel placement.

UNLABELLED: Lateral ankle ligament surgical reconstruction can take many forms, including anatomic and nonanatomic reconstructions. The nonanatomic reconstructions require the use of autograft or allograft tendons to recreate the vectors of the injured anterior talofibular ligament (ATFL) and the calcaneofibular ligament (CFL). The purpose of this study was to determine the minimum, maximum, and average graft length requirements for a modified Chrisman-Snook procedure. The structures at risk during drilling of the bone tunnels were documented to guide recommendations for tunnel placement. Modi

Pseudoaneurysm of the anterior tibial artery: A rare complication of proximal tibial steinman pin insertion.

An anterior tibial artery pseudoaneurysm is a rare and unexpected complication of Steinmann pin insertion. We describe the case of an 18-year-old boy, who sustained such an injury to the anterior tibial artery during this procedure. Diagnosis was confirmed on a magnetic resonance (MR) angiogram. Aneurysmal sac excision with lateral repair of the vessel wall was performed. Postoperatively, a good flow was documented on a follow-up MR angiogram. This case highlights a major and unexpected complication of a so-called minor procedure. Too posterior a pin placement in the proximal tibia should be a

The avascular plane of the Achilles tendon: a quantitative anatomic and angiographic approach and a base for a possible new treatment option after rupture.

INTRODUCTION: Achilles tendon ruptures, especially ruptures caused by pathologic conditions and also by achillotendinitis are often attributed to the alleged hypovascularisation of the Achilles tendon. Anatomic studies often mention an avascular plane. The purpose of this study was to re-investigate the arterial supply of the Achilles tendon. MATERIAL AND METHODS: Lower legs of 28 anatomic specimen were injected with a radiologic contrast agent and subsequently an arterial angiography was performed. Afterwards the legs were embalmed and later anatomically dissected. The origin of arteries ent

Variations in anatomy of the popliteal artery and its side branches.

Knowledge of anatomic variations of the popliteal artery is essential for the management of peripheral vascular disease and in orthopedic surgery. The aim of this study was to perform an overview of the literature describing variations of the popliteal artery. To identify relevant literature, we performed a systematic search on MEDLINE, EMBASE, and the Cochrane Database of Systematic Reviews. We included 4 studies of anatomic dissections and 11 radiologic retrospective series, comprising 7671 limbs, and a variation in popliteal branching was seen in almost 10%. The 3 most frequent variations i

Evaluation of popliteal artery injury risk with locked lateral plating of the tibial plateau.

OBJECTIVES: This study was undertaken to determine if there is increased likelihood of popliteal artery injury as one places a fixed-angle lateral proximal tibia locking plate with posterior plate lift off and or anterior plate translation from the ideal position. METHODS: A Synthes (Synthes USA, West Chester, PA) 3.5-mm and 4.5-mm lateral proximal tibia locking plate was placed consecutively on each of six specimens in the straight lateral (SL) position. Screw position with respect to the medial cortex was recorded as well as the distance of the posterior most screw tip to the popliteal arte

Ultra-low-velocity knee dislocations.

BACKGROUND: Reports of low-velocity knee dislocations have focused primarily on dislocations occurring during athletic competition. The authors identified a subset of patients with low-velocity knee dislocations that occurred during activities of daily living, such as stepping off a curb, stepping off a stair, or simply falling while walking (ultra-low-velocity dislocations). HYPOTHESIS: Ultra-low-velocity knee dislocations are common in obese individuals and are associated with more complications than high-velocity knee dislocations. STUDY DESIGN: Case series; Level of evidence, 4. METHODS

[Application of free anterior serratus musculo-fascial flap in bridge style for the soft tissue defect at leg].

OBJECTIVE: To investigate the application of free anterior serratus musculo-fascial flap in bridge style for the soft tissue defect at leg. METHODS: From Sept. 2006 to Jan. 2009, the free anterior serratus musculo-fascial flaps were used in bridge style in 7 cases with soft tissue defects at legs. The anterior serratus musculo-fascial flaps were elevated with subscapular and circumflex scapular vessels forming a T-shaped vascular pedicles. The T-shaped pedicle was end-to-end anastomosed with the two ends of the posterior tibial artery at the healthy leg. The musculo-fascial flap and its pedic

Arteriovenous fistula formation after a closed proximal tibial fracture in a child.

Arteriovenous fistula formation after a closed extremity fracture is rare. We present the case of an 11-year-old boy who developed an arteriovenous fistula between the anterior tibial artery and popliteal vein after closed fractures of the proximal tibia and fibula. The fractures were treated by closed reduction and casting. A fistula was diagnosed 12 weeks after the injury. It was treated by embolisation with coils. Subsequent angiography and ultrasonography confirmed patency of the popliteal vein and anterior and posterior tibial and peroneal arteries, with no residual shunting through the f

Lateral calcaneal artery as a recipient pedicle for microsurgical foot reconstruction.

In the microsurgical reconstruction of the foot, anterior tibial artery-dorsalis pedis artery and posterior tibial artery-plantar artery are mainly used as recipient arterial pedicles. These arteries are the main sources for foot circulation and the preservation of these arterial circulations is very important. Although the end-to-side technique or the flow-through technique is selected for the microsurgical anastomosis, the possibility of injury to the circulation of these main arteries exists. We showed the availability of this artery with an angiographic injection study in five fresh cadave

Vascular safety during high tibial osteotomy: a cadaveric angiographic study.

BACKGROUND: High tibial osteotomy is technically demanding. Risks include injury to the popliteal neurovascular bundle. The present goal was to further define this risk. HYPOTHESIS: The distance from the posterior tibia to the popliteal artery increases with increasing knee flexion. A saw angle perpendicular to the coronal plane can injure the popliteal artery. STUDY DESIGN: Descriptive laboratory study. METHODS: Seven fresh-frozen cadaveric lower extremities were used. Lateral radiographs at knee flexion angles of 90 degrees , 60 degrees , 45 degrees , 30 degrees , and 0 degrees were taken

[Lower-limb arterial thrombosis in a relapse of nephrotic syndrome].

INTRODUCTION: Venous thromboembolic complications are well-known in patients with nephrotic syndrome. Arterial thrombosis has rarely been reported and is mainly observed in adults. CASE REPORT: A 9-year-old patient with a history of steroid-dependent nephrotic syndrome was admitted after foot trauma. He presented with severe pain in his right foot, which was cold and mottled, with reduced sensation and motor activity. No pulse was palpable in his right foot. Doppler's ultrasonography confirmed the thrombosis of anterior and posterior tibial arteries in their distal portions. The patient recei

Three-step approach to the harvest of the fibula osteoseptocutaneous flap.

INTRODUCTION: The ability to reliably include a skin paddle with the fibula osteoseptocutaneous (OSC) flap is crucial both from the perspective soft tissue reconstruction and flap monitoring. In this study, we describe a three-step approach to the harvest of the fibula OSC flap that is reliable and versatile. METHODS AND MATERIALS: Step 1 starts by exploring the posterior crural septum from the anterior incision of the skin island with the aim being to identify the septocutaneous vessels that will supply the skin. Step 2 proceeds from the posterior aspect of the skin island. The septocutaneou

Neurovascular safety and clinical outcome of outside-in repair of tears of the posterior horn of the medial meniscus.

PURPOSE: The purpose of this study was to evaluate the safety and clinical outcome of a new posterior approach for the known outside-in technique for repair of tears of the posterior horn of the medial meniscus (PHMM). METHODS: First, a cadaveric study was performed on 6 cadaveric knees to assess the safety of a point just lateral to the semitendinosus tendon as an entry point for outside-in repair. Dissection was done to measure the clearance of this point to the nearby popliteal bundle and saphenous nerve. A prospective case series study was then performed to assess the clinical outcome of

Arterial anatomy of the talus: a cadaver and gadolinium-enhanced MRI study.

BACKGROUND: Avascular necrosis following a fracture of the talar neck may be secondary to the injury itself, or may result from the surgical approach and exposure during treatment. We sought to define the arterial anatomy of the talus using gadolinium-enhanced magnetic resonance imaging (MRI) and through gross dissection following latex injection of cadaver limbs. The use of gadolinium-enhanced MRI for the evaluation of the arterial supply of the talus has not been previously reported. METHODS AND MATERIALS: We utilized 12 fresh frozen cadaver limbs to study the arterial anatomy of the talus.

Acute ischemia of the lower limb after injury by gunshot: case report and review of literature.

INTRODUCTION: Civilian and military vascular trauma has evolved dramatically. It is estimated that 80% of all cases of vascular injuries are due to peripheral vascular trauma, most involving the lower extremities. Arterial below-knee injuries by gunshot still remain a challenge, with an associated rate of amputation of 20-54%. MATERIAL AND METHODS: This case reports a 48 year-old male patient who sustained a gunshot below his left knee. At hospital admission, loss of substance and hemorrhage at the medial and lateral faces of the upper third of his left leg were present with signs of distal i

Pseudoaneurysm of the anterior tibial artery detected by emergency medicine physician performing bedside ultrasound.

We present the case of an anterior tibial artery pseudoaneurysm secondary to lacerating trauma caused by a straight-edge trimming blade. A 56-year-old man presented to emergency department (ED) with 1-month history of left calf pain. Thirty days before presentation, the patient lacerated his left lateral pretibial area with a cutting blade--similar to a machete--while cutting grass. He was seen at the state hospital, and primary suture was performed. Ten days later, the suture was removed. A week later, the patient revisited his physician, complaining of swelling and tenderness in the left cal

Vascular injury after occult knee dislocation presenting as compartment syndrome.

BACKGROUND: Popliteal artery injury can occur in up to one-third of patients with knee dislocation. Delay in the diagnosis of popliteal artery injury is the leading cause of amputation in this limb-threatening injury. OBJECTIVES: To remind emergency physicians to consider popliteal artery injury in any pulseless lower extremity, even in cases of spontaneous reduction of the knee dislocation before evaluation by medical personnel. CASE REPORT: We present a case of popliteal artery injury and occult knee dislocation in which the diagnoses were delayed for hours by incorrectly attributing the a
