Background and Objectives: Despite its advantages, lateral close-wedge high tibial osteotomy (LCWHTO) requires proximal tibiofibular joint detachment (PTFJD) or fibular shaft osteotomy for gap closing. These fibula untethering procedures are technically demanding and not free from the risk of neurovascular injuries. Our novel fibula untethering technique, tibial-sided osteotomy (TSO) near the proximal tibiofibular joint (PTFJ), aims to reduce technical demands and the risk of injury to the peroneal nerve and popliteal neurovascular structures. The purposes of this study were to introduce the T

BACKGROUND: Peroneal artery injury following internal fixation of a calcaneal fracture is rare. The occurrence of a delayed pseudoaneurysm with an arteriovenous fistula after a peroneal artery injury is even rarer and has not been reported previously.
CASE SUMMARY: Herein, we report the case of a 65-year-old female patient with a calcaneal fracture (Sanders Type ⅢAB) who underwent an open reduction and internal fixation surgery through the tarsal sinus approach. Two months postoperatively, she experienced left foot pain. Physical examination revealed pulsation at the surgical site and a posit

BACKGROUND: All-inside techniques for meniscal repairs offer comparable outcomes and healing rates with reduced operative time and fewer incisions; however, iatrogenic neurovascular injuries during arthroscopic meniscal repairs are a significant concern.
PURPOSE: To identify the zones of risk and incidence of injury concerning the common peroneal nerve (CPN) and popliteal artery in relation to the popliteal tendon (PT) from the anterolateral (AL) and anteromedial (AM) portals during a simulated all-inside technique in the pediatric population.
STUDY DESIGN: Descriptive laboratory study.

As blood flow to the reverse sural artery flap (RSAF) occurs via the peroneal artery, the health of the peroneal artery is generally considered important. In severe limb injuries, the main trunk of the peroneal artery is often disrupted at the fracture site, which brings uncertainty in the use of RSAF. We hypothesized that RSAF could be used even in cases where the main trunk of the peroneal artery is interrupted if retrograde blood flow from the plantar side forms a communication network with the superficial sural artery. Therefore, we performed RSAF in cases where the blood flow of the commu

INTRODUCTION: The medial midline portal and anterocentral both are not standard portals for ankle arthroscopy, but they provide a wide field of vision.
AIMS: The aim of this study was to compare anatomical safety between medial midline and anterocentral portal in arthroscopy in respect to the least injury to adjacent structures.
MATERIALS AND METHODS: 20 cadaveric ankles were dissected and related anatomical structures were measured from anterocentral and medial midline portals.
RESULTS: The dorsalis pedis artery (DPA) was at a mean distance of 10.08 mm, the deep peroneal nerve was at 16.20

Reconstructing nerve gaps greater than 13-15 centimeters often requires vascularized nerve grafts. However, using the sural nerve presents a technical challenge due to the lack of a consistent adjacent artery. This study introduces various free vascularized sural nerve flap designs to overcome this limitation. From 2018 to 2020, six patients at Chang Gung Memorial Hospital underwent eight free vascularized sural nerve grafts reconstruction, including brachial plexus, median, ulnar, posterior tibial, and sciatic nerve reconstructions. Three types of flap designs were performed: shunt-restricted

BACKGROUND: The objective of this study is to detail the terminal distribution of the peroneal artery, which may be at risk in a number of surgical procedures. Five per cent of the population presents with a dominant peroneal artery (arteria peronea magna), which is responsible for the entire foot vascularization.
METHODS: Anatomic dissection of 54 specimens from 44 cadaveric donors was performed. Twenty belonged to women and 24 to men, whereas 10 lacked traceability data. Mean age of the donors was 75.01 ± 16.14 (range 38-101) years. Ten donors had bilateral extremities.
RESULTS: Six types

BACKGROUND: Anterior knee pain, with varus deformity and excessive external tibial torsion, can markedly impair function and quality of life. Although various surgical techniques address these deformities, dome-shaped osteotomy offers a promising approach for correcting malalignment while preserving joint integrity. This study evaluated clinical and radiological outcomes of dome-shaped osteotomy in patients with these combined deformities.
METHODS: Twenty-eight knees from 21 patients with anterior knee pain, varus deformity (more than 5°), and excessive external tibial torsion (more than 30°)

BACKGROUND: Pediatric meniscal injury rates and their arthroscopic repair are on the rise. With the growing popularity of all-inside devices used in this potentially higher-risk population, studies examining the proximity of these devices to neurovascular structures are needed.
PURPOSE/HYPOTHESIS: The purpose of this study was to provide distances to neurovascular structures from all-inside devices placed arthroscopically at both high-risk approach angles and zones of the lateral menisci of pediatric cadaveric specimens. It was hypothesized that the neurovascular structures would be situated

In this clinical vignette, we report the case of a 22-yr-old division I collegiate wrestler who presented with chronic left foot and ankle weakness, tightness, and a burning sensation extending from the midlateral calf to the dorsum of the foot. Initial magnetic resonance imaging completed of the left knee 1 yr before presentation was unremarkable. He was given a diagnosis of exertional compartment syndrome and he was medically cleared to compete. However, after sustaining an unrelated left knee injury during a match 4 wks later, a repeat magnetic resonance imaging revealed a multiloculated cy

The addition of surgical angiogenesis to reconstructed peripheral nerves is argued to improve nerve regeneration. Surgical angiogenesis can be performed by wrapping a vascularized flap around the nerve graft, which is believed to enhance nerve regeneration following reconstruction. The protocol aimed to outline the surgical procedure and report long-term viability and patency for the superficial inferior epigastric artery fascia (SIEF) flap in a rabbit peroneal nerve defect model to add vascularization and enhance nerve regeneration. In this article, step-by-step instructions on the harvest an

BACKGROUND: Neurovascular complications after tibial plateau fractures are rarely reported, despite their clinical relevance. While Schatzker type IV fractures have traditionally been considered the most at risk, supporting evidence is limited. This is the first large-scale study systematically evaluating neurovascular injuries in tibial plateau fractures. The aim was to determine the incidence of arterial and neurological injuries and to identify fracture patterns most frequently associated with these complications.
METHODS: We conducted a retrospective review of patients who underwent open

Background The talus is the second largest tarsal bone and makes the osseous link between the leg and foot region. The branches of the dorsalis pedis, posterior tibial, and peroneal arteries enter vascular foramina (VF) on the various surfaces of the talus and provide intraosseous blood supply. Understanding the morphology and morphometry of VF might be helpful in reducing the risk of vascular injury associated with surgical interventions to the talus. Aim and objectives The purpose of this study is to contribute reference data for the morphology of VF of talus in a sample from Türkiye. Mater

PURPOSE: Drilling the tibial tunnel in tibiofibular-based posterolateral corner (PLC) reconstructions is usually performed with a starting point from Gerdy's tubercle. Drilling the tunnel starting from the anteromedial tibial cortex could reduce the risk of popliteal neurovascular (PNV) injury. The purpose of this study is to assess the distance on Magnetic Resonance Imaging Scans (MRI) of the PNV bundle and peroneal nerve (PN) to these two different tunnel trajectories.
METHODS: A total of 87 MRI scans were assessed. The ideal exit point of the tibial tunnel in PLC reconstructions was marked

BACKGROUND: Anterior tarsal tunnel (ATT) syndrome is caused by the compression of the deep fibular nerve (DFN) within the ATT beneath the inferior extensor retinaculum, bounded by the tendons of the extensor hallucis longus (EHL) and extensor digitorum longus (EDL). Compression may result from direct trauma, repetitive mechanical irritation, and thrombosis of the dorsalis pedis artery. Injury to the contents of ATT could occur during ankle arthroscopy. Therefore, this study was undertaken to provide a detailed description of the anatomy of the ATT and its clinical implications.
