The miniarthrotomy technique of ankle arthrodesis: a cadaver study of operative vascular compromise and early clinical results.

We present a two-part study of the miniarthrotomy ankle fusion technique for minimally deformed joints: a laboratory investigation of ankle vascularity and a clinical review of the early results. In the laboratory portion, five pairs of cadaver legs were injected with radiographic dye, after which arthrodesis was performed via either the open technique (one leg of each pair) or the miniarthrotomy technique (the contralateral leg of each pair). Two legs in the open arthrodesis group had disruption of the peroneal arterial circulation, but no disruption was identified in the miniarthrotomy group

[Applications of nine types of free pedal flaps in hand surgery].

From 1987, nine types of free skin flap or composite free flap from the food were applied to treat 22 cases of hand injury. These flaps included 1. composite skin flap of pedal dorsum, first toe-web, toe-pulp, and toe dorsums with long extensor tendons of the toes, 2. composite skin flap of the second metatarsophalangeal joint; 3. "wrap around" flap; 4. the first web flap; 5. plantar skin flap of great toe with only the artery and nerve; 6. "wrap around" flap with dorsalis pedis flap; 7. composite dorsalis pedis flap with the second toe; 8. the dorsalis pedis flap with superficial peroneal ner

Successful revascularization of a partially avulsed foot in a 6-year-old child.

The foot of a 6-year-old child was revascularized successfully following an avulsion and partial amputation through the tibiotalar joint. Partial degloving had disrupted the anterior and posterior tibial arteries with resultant ischemia. We excised the entire damaged segment of the posterior tibial artery and performed a reverse saphenous vein graft, end-to-end reconstruction of the defect. Peroneal and tibial nerve function returned within 5 months. Four years later, the patient has regained full, painless range of motion and normal strength and sensibility with no evidence of premature growt

Free vascularized deep peroneal nerve grafts.

An ideal donor site for vascularized nerve grafts should have a constant anatomy, minimal functional loss after the nerve has been sacrificed, and a dependable blood supply parallel to the nerve over a relatively long distance. Creating a pedicle for a free vascularized deep peroneal nerve graft with the anterior tibial vessels seems to be a most suitable method for repairing long nerve gaps of over 20 cm and digital nerve defects with severe finger damage. Applications of this nerve graft to digital nerve losses with severely scarred beds created by avulsion injury, and two-stage reconstructi

Anatomical considerations of irreducible medial subtalar dislocation.

Irreducible medial subtalar dislocation has been associated with the talar head "buttonholing" through the extensor digitorum brevis (EDB) muscle, entrapment of the talar head in the extensor retinaculum, talonavicular impaction, interpositioning of the EDB muscle between the talus and navicular, and buttonholing of the talar head through the talonavicular ligament and joint capsule. Cadaver analysis of the medial subtalar dislocation in this study supported entrapment of the talar head in the extensor retinaculum and talonavicular impaction as well as impingement of the deep peroneal nerve an

Arteriovenous fistula after fibular osteotomy leading to recurrent haemarthroses in a total knee replacement.

We present a patient with an arteriovenous fistula of the peroneal artery acquired after a left dome tibial osteotomy with midshaft fibular osteotomy. He had subsequently had a total knee replacement on that side. The arteriovenous malformation was only diagnosed when he represented with symptoms and signs of venous hypertension with sterile recurrent haemarthroses in the left knee. Percutaneous obliteration of the fistula, by a combination of coil embolisation and balloon occlusion, cured the symptoms.

Orthopedic management of knee dislocations. Comparison of surgical reconstruction and immobilization.

Thirty-nine patients with 43 complete knee dislocations managed between 1973 and 1990 were reviewed retrospectively to compare the results of surgical reconstruction to nonreconstructive treatment of these injuries. The average patient age was 34 years and the average follow-up was 5 years (range: 1 to 18 years). Patients were evaluated by physical examination and the Lysholm knee scoring scale. Fourteen knees (33%) sustained popliteal vessel injury and five (9%) required amputation. Peroneal and tibial nerve injuries involved 13 knees (30%). Twenty-five of 39 patients (64%) sustained other as

Complete dislocation of the knee: spectrum of associated soft-tissue injuries depicted by MR imaging.

OBJECTIVE: Complete knee dislocation is a rare injury. The purpose of this study was to evaluate the spectrum of injuries that are depicted by MR imaging in patients with a dislocation of the knee and to determine if there is any predictive factor that might indicate those patients who may be at risk for popliteal nerve injury. MATERIALS AND METHODS: A retrospective search for patients sustaining traumatic knee dislocations who had radiographs and an MR imaging examination of the knee as part of their initial evaluation was done at three level I trauma centers for the period between 1989 and

Maintenance treatment of diabetic patients, associating arterial obstructive tibio-peroneal disease.

When a melito-diabetic patient presents trophic infected injury on the limb, it is essential an evaluation of the circulatory conditions for therapeutic procedures orientation. In some circumstances, although arterial pulsation is absent, there is no ischemia of tissues. In these cases, the maintenance treatment, with eventual resection of the necrosed and infected tissues may be adopted. Evolution of 70 diabetic patients with trophic injuries on extremities were submitted to a maintenance treatment. Age of patients varied from 28 to 88 years, with an average of 56.8. The most occurrence was v

[Management of blunt injury of the popliteal artery].

PURPOSE: this retrospective study was undertaken to analyse our results with blunt popliteal artery injury and to detect flaws in our approach. MATERIAL AND METHOD: between 1979 and 1993, 31 consecutive patients with a blunt injury to the popliteal artery were retrospectively reviewed. RESULTS: 4 patients were in shock on admission from a popliteal artery bleed. One patient died before treatment. Primary amputation of the leg was performed in one patient because of extensive tissue destruction. Arterial reconstruction was performed in the remaining 29 patients: 1 patient died of associated i

Revascularization of a seven-week-old infant lower extremity: case report.

A seven-week-old infant suffered a closed segmental fracture of the left tibia and fibula complicated by vascular disruption and a secondary compartment syndrome. Initial orthopedic assessment revealed a clinically mottled, cool, pulseless extremity 4 hr post-injury. Radiologic investigations showed multiple displaced fractures of both the tibia and fibula. The infant underwent four compartment fasciotomies and intramedullary nailing of the tibial fractures. The vascularity of the limb failed to improve significantly. Angiograms done 22 hr post-injury revealed disruption of arterial inflow jus

Open type IIIB and IIIC fractures treated by an orthopaedic microsurgical team.

An orthopaedic team with an extensive microsurgical background treated 46 patients with open fractures, 22 Type IIIB and 24 IIIC, of the upper and lower extremities during the past 10 years. All 24 patients with Type IIIC fractures and 12 with IIIB fractures had associated arterial injuries. In all patients from both categories, an effort was made to revascularize the limbs using microsurgical techniques and to stabilize the fractures as early as possible. Of the Type IIIC fractures, 16 were in the lower extremity and 8 in the upper extremity. Of the Type IIIB injuries, 8 were in the lower and

Effects of static lung inflation on sympathetic activity in human muscle nerves at rest and during asphyxia.

Muscle sympathetic activity is inhibited during the second half of phasic lung inflation associated with normal (negative pressure) breathing or artificial ventilation with intermittent positive-pressure, and this inspiratory inhibition appears unrelated to the associated changes in arterial pressure. In this present study we tested the hypothesis that a static inflation of the lungs would cause a sustained inhibition of muscle sympathetic activity. Microneurographic techniques were used to record muscle sympathetic activity from the peroneal nerve, and arterial pressure was monitored continuo

Osteochondroma: an unusual cause of vascular disease in young adults.

Vascular complications such as pseudoaneurysm, arterial thrombosis, luminal stenosis due to extrinsic compression, deep venous thrombosis, and arteriovenous fistula are known complications of osteochondroma. The authors describe three cases of vascular injury caused by osteochondroma: popliteal artery impingement, popliteal artery pseudoaneurysm formation, and superficial femoral artery pseudoaneurysm with peripheral occlusion of the tibial and peroneal arteries due to embolization. Fifty-six cases of vascular complications due to osteochondroma from the English literature are also reviewed. T

Delayed diagnosis of a peroneal artery false aneurysm at a concomitant tibial fracture. A case report.

A 49-year-old man had posttraumatic persistent calf swelling and a tibial and fibular fracture. Despite the intramedullary nailing of the fracture, the swelling did not improve, and at the 6th postoperative week it was misdiagnosed (using venogram) as deep vein thrombosis. Therefore, it was mistreated with anticoagulants, which led to great deterioration of the local signs. An arteriogram revealed an initially missed false peroneal artery aneurysm. Surgical treatment was performed immediately. The 6-week delay had led to some atrophy of the posterior compartment muscles, fortunately without an

Surgery of varicose veins on ambulatory basis. Early and late results.

Varicose veins resection on a day-case basis has the advantage of the early return of the patient to his usual activities, thus also reducing the cost and occupation of hospital beds. Between 1980-1992, 1710 patients (2325 extremities) were operated on as outpatients. The preoperative management included: clinical examination, Doppler ultrasounds, Duplex scanning, ambulatory venous pressure and phlebography. The surgical method was based on two axioms: resection of affected segments only, and functional operation according to the hemodynamic parameters. At the end of the first trimester, 1.572

Allograft reconstruction of the anterior and posterior cruciate ligaments after traumatic knee dislocation.

Seven patients (average age, 26.3 years) with traumatic knee dislocations were retrospectively evaluated more than 2 years (average, 51 months) after having fresh-frozen allograft anterior and posterior cruciate ligament reconstructions. All patients were treated consecutively at an average of 9.6 days after injury. Two patients had arterial injuries and three patients had or developed common peroneal nerve palsy. Five patients had 20 additional injuries. All patients were enlisted in an early, aggressive physical therapy regimen with early protected weightbearing. Four patients required a man

The Dislocated Knee.

Knee dislocation, although relatively rare, may be the result of high-or low-velocity injuries. Well established is the need for urgent diagnosis and treatment to avoid vascular complications and amputation. The initial evaluation should include objective assessment of arterial circulation by means of Doppler pressure measurements; the finding of any asymmetric pressure warrants an arteriogram. Late arterial occlusion may occur, which mandates careful serial reexamination in all patients, including those with initially symmetric pressure. Injury to the peroneal nerve is also common, and the re

The deep peroneal nerve in the foot and ankle: an anatomic study.

Injury to the deep peroneal nerve in the foot and ankle may result from trauma, repetitive mechanical irritation, or iatrogenic harm. The nerve is most susceptible to injury along its more distal anatomic course. Dissection of 17 cadaver specimens was undertaken to describe the course of the deep peroneal nerve and quantify its branch patterns. In the distal one third of the leg, the nerve was located superficial to the anterior tibial artery between the tibialis anterior and extensor hallucis longus muscles. Typically, the nerve crossed deep to the extensor hallucis longus tendon to enter the
