Stability and motion after traumatic dislocation of the knee.

In a series of 10 patients with traumatic dislocation of the knee joint, closed reduction could be accomplished in nine. Conservative treatment was employed in four and operative repair of the ligaments and capsule in six patients. In two of three patients with complicating artery injury, vascular repair was successful. Above-knee amputation was performed in one patient because of delay in the diagnosis of vascular injury and in another patient because of arteriosclerotic gangrene. At follow-up examination, on average 6 years after the accident, the stability and motion of the knee were evalua

Peroneal flap for reconstruction in the extremity: preliminary report.

The peroneal flap has many advantages, such as thin subcutaneous tissue, availability of a large-diameter artery and vein for anastomosis, a long pedicle, and freely designed size and shape. This technique is introduced because it has many outstanding features that allow it to be used to meet a variety of specific needs, such as a free peroneal flap, a peroneal island flap, a free vascularized fibular graft with skin, and a monitoring flap in free vascularized fibular grafts. We have found the peroneal flap to be especially useful.

[Pseudoaneurysm of the peroneal artery caused by a non-penetrating injury].

The rareness of false arterial aneurysm, a consequence of a non-penetrating injury of a limb, induced the Authors, on the basis of the most recent literature, to review its physiopathologic, clinical and therapeutical aspects, with the purpose to hasten its diagnosis and prevent its complications through the adoption of the most suitable surgical treatment.

Arterial injuries below the knee: fifty-one patients with 82 injuries.

Clinical features of tibial and peroneal artery injuries are characterized in a review of 51 patients with 82 injured arteries. Injuries were penetrating in 34 (67%) patients and blunt in 17 (33%). Physical findings suggested arterial injuries in 42 (82%) patients, but nine had no signs of vascular trauma. Nineteen patients with no palpable distal pulses had average preoperative delays of nearly 5 hours, suggesting a lack of appreciation for the morbid potential of these injuries. Operations included ligations of single-vessel injuries and arterial reconstructions in the 21 (41%) patients with

Traumatic false aneurysm of the ankle. A case report.

A 27-year-old man incurred a false aneurysm of the peroneal artery secondary to an inversion injury to the ankle. A search of the literature disclosed no other cases of false aneurysms with ankle sprains. The false aneurysm of the peroneal artery appeared as a mass with a compressive neuropathy of the sural nerve. The diagnosis of false aneurysm of the peroneal artery following an inversion injury to the ankle should be suspected in cases of persistent localized swelling with inordinate pain that does not subside with elevation and immobilization and is associated with peroneal neuropathy.

The surgical treatment of blunt and penetrating injuries of the popliteal artery.

Thirty-six patients sustaining blunt and penetrating injuries to the popliteal artery underwent vascular reconstruction with an average ischemic interval of eight and one-half hours (range: four to 14 hours). There were 16 associated venous injuries, 15 nerve injuries, and 20 fractures with or without knee dislocations. Arterial repair was performed by saphenous vein interposition graft in 23 patients and by end-to-end anastomosis in 16 patients (three patients had more than one operation). Eight of the venous injuries were reconstructed and the remaining eight veins were ligated. Fasciotomy w

Nonoperative management of a traumatic peroneal artery false aneurysm.

The standard approach to management of traumatic false aneurysms is operative. This report describes successful treatment of a traumatic peroneal artery aneurysm following a gunshot wound in a 29-year-old man by an intraluminal steel coil occlusion technique. The coil was guided through the popliteal artery and aneurysm into the distal peroneal artery via a percutaneously placed catheter: following disengagement of the coil the outflow tract of the aneurysm was occluded, and another passed and released into the proximal peroneal artery occluded the inflow tract.

Peripheral nervous system complications of coronary artery bypass graft surgery.

Among 421 patients undergoing coronary artery bypass graft surgery, 55 (13%) developed 63 new peripheral nervous system (PNS) complications postoperatively. Most common was a brachial radiculoplexopathy, which occurred in 23 patients. Of these, 21 involved lower trunk or medial cord fibers. In 17 there was a correlation between the site of jugular vein cannulation and the side affected, suggesting that needle trauma played a role. Stretching from chest wall retraction may have caused some cases. Other deficits included 13 saphenous, 8 common peroneal, and 5 ulnar mononeuropathies. Six patients

Tibial artery injuries.

Twenty-eight consecutive patients with tibial or peroneal artery injury are reported. Of these, eight underwent direct arterial repair, eight received vein graft reconstruction, and the involved artery was ligated in the remainder, with amputation required for various reasons in four. Tibial or peroneal artery injury is a relatively common problem. Successful management requires an aggressive approach to early diagnosis, which in turn requires a high index of suspicion and arteriography. Early appropriate management will prevent the serious complications of ischemia, false aneurysm, arterioven

Free transfer of a large sensory flap from the first web space and dorsum of the foot including the second toe for reconstruction of a mutilated hand.

A 19-year-old man sustained a mutilating crush injury in August, 1978, in an 80-ton punch press. The injury resulted in transmetacarpal amputations of the index through little fingers. The thumb was revascularized by microvascular anastomoses. The replanted index to the little finger position failed. A free transfer of the patient's second toe to the fifth metacarpal was done to provide a post to oppose the thumb. The first web space of the toe was used in expanding the contracted single cleft of the hand. The innervated dorsal foot flap provided coverage for the dorsal surface of the hand. An

Tourniquet-induced nerve ischemia: an experimental investigation.

This experimental investigation has demonstrated that an inflated pneumatic tourniquet causes slowing in conduction velocity of the sicatic nerve. The amount of slowing and recovery time vary according to inflation pressure and to the duration of applied pressure. The conduction velocity always returned to normal provided the tourniquet was inflated for 2 hours or less, and pressures were below 500 mm Hg. The nerve directly under the tourniquet was most susceptible to injury. Inflation pressure should be as low as possible and be determined according to systolic blood pressure. An Esmarch band

Fractures of the tibia through the proximal tibial epiphyseal cartilage.

Thirty-nine fractures of the tibia involving the proximal tibial epiphyseal cartilage were treated at the Campbell Clinic over a twenty-five-year period. Stress roentgenograms were essential in making the diagnosis in three patients. Two patients had disruption of the popliteal artery, and both had posterior displacement of the tibial shaft. The other immediate complications that we encountered were anterior compartment syndrome, peroneal-nerve palsy, and associated ligamentous and meniscal injuries. Of twenty-eight fractures (twenty seven patients) with an average follow-up of 7.1 years, ther

[Replacement of skin defects on the foot with dorsal foot artery pedicle island flap (author's transl)].

On the basis of four known cases, authors describe the art. dors. pedis pedicle island flap method (McCraw, Furlow) for replacement of skin defects on the foot. The pedicle of the flap is composed of the art dors. pedis, the vena comitans and the peroneal nerve. The flap is suitable for replacement of skin defects around the external or internal malleolus, on the margin of the sole and heal, over the tendon Achilles as well as on the anterior surface of the lower third leg. Vascular anatomy of the island is described and details of the technics for the formation of island are given. Beside the

Ischemic contracture in the lower limb.

Twenty cases of ischemic contracture in the lower limb demonstrate that the age distribution may be higher than that of patients with similar lesions in the upper limbs. Non-traumatic and traumatic causes are encountered equally frequently. Pain is one of the most important symptoms associated with long standing lesions. The ischemic lesion develops most frequently and severely in the anterior compartment of the leg. The functional prognosis is poor when both nerves are affected and when the deformity is severe.

Popliteal artery injury following fracture or dislocation at the knee. Diagnosis and management.

Fracture dislocations or dislocations of the knee can produce an arterial injury that may be difficult to evaluate clinically. Aggressive diagnosis and treatment can produce satisfactory results in both vascular and orthopedic situations if performed early after the injury. Five cases of popliteal arterial injury were seen from April 1974 to August 1976. Three cases were following posterior knee dislocation, one was a femoral fracture displacement, and one was a rotary tibiofibular displacement. Four of the patients had complete popliteal artery transection and one had intramural hematoma and

Selective intra-arterial tolazoline infusion in peripheral arterial trauma.

Intra-arterial tolazoline has been used as an adjunct for angiographers and has been investigated for application in certain peripheral vascular disorders secondary to its potent activity as a vasodilator. We have had experience with three cases in which severe peripheral arterial trauma had resulted in nonviable extremities in spite of both orthopedic manipulation and vascular reconstruction. Continuous infusions of intra-arterial tolazoline were begun with marked improvement in blood flow in all extremities and resultant viability in all three within 48 hours. All three of these extremities,
