Venous thromboembolism prophylaxis in burn patients: A 12-year retrospective cohort study in the era of "trauma dosing" low molecular weight heparin.

Venous thromboembolism is a recognized complication of burn injury, yet the contemporary incidence in the setting of routine pharmacologic prophylaxis remains uncertain. We conducted a twelve-year retrospective cohort study of adult patients admitted to a single academic burn center between 2012 and 2023. The primary outcome was clinically significant venous thromboembolism, defined as pulmonary embolism or deep vein thrombosis above the popliteal vein confirmed by imaging. Univariable and multivariable logistic regressions were performed to identify factors associated with venous thromboembol

Surgical management and limb salvage outcomes in orthopedic trauma patients with concomitant popliteal artery injury: a single-center retrospective analysis.

BACKGROUND: Traumatic popliteal artery injury often occurs with complex orthopedic and soft-tissue damage and requires coordinated vascular, skeletal, decompressive, and wound management. We evaluated real-world treatment patterns and limb-salvage outcomes in a single-center cohort of patients with combined orthopedic trauma and popliteal artery injury. METHODS: We retrospectively reviewed 48 hospitalization records from 46 patients treated between 2002 and 2025. Repeated hospitalizations related to the same injury were reconciled into 46 independent patient/injury episodes. The primary outco

TCF4-driven cathepsin S promotes papillary thyroid carcinoma via PI3K/AKT/mTOR.

Papillary thyroid carcinoma (PTC) usually has a good prognosis, but a subset of patients develops lymph node metastasis, radioiodine resistance and recurrence, and lacks effective molecular biomarkers and therapeutic targets. Cathepsin S (CTSS), a lysosomal cysteine protease, has been implicated in several malignancies, yet its role and regulation in PTC remain unclear. We integrated GEO and TCGA-THCA datasets with eQTL-based genetic instrument analyses to screen PTC-related genes and evaluate their association with clinicopathological features and DFS. CTSS expression was examined in clinical

Simultaneous arterial and venous repair with damage-control fixation in ballistic femoral shaft fracture: A case report and literature review.

INTRODUCTION: Low-velocity ballistic thigh injuries can produce comminuted femoral shaft fractures with associated superficial femoral artery and vein lacerations, requiring rapid haemorrhage control and restoration of perfusion. Computed tomography angiography (CTA) provides high-resolution mapping of osseous and vascular damage, guiding the repair sequence. CASE PRESENTATION: A 23-year-old man presented 1 h after a mid-thigh gunshot wound with haemorrhagic shock, expanding haematoma, and absent distal pulses. CTA revealed a comminuted femoral shaft fracture, superficial femoral artery extra

Evidence based management of popliteal vessel injuries: A critical review, updates and controversies in the management of a difficult injury.

Popliteal artery injuries are rare even in urban Trauma Centers. The vast majority occur secondary to penetrating trauma. These injuries are uncommon; therefore, few Trauma Surgeons and Trauma Centers have developed significant experience with their management. Experiences reported in both military and civilian publications consistently report the highest complications and amputation rates of all vascular injuries due to popliteal vessel injuries. The popliteal artery is an end artery. Injuries cause significant ischemia that significantly threaten limb viability. The popliteal vein provides t

Vein Graft Characteristics and Clinical Outcomes in Blunt Popliteal Artery Injury: A Retrospective Case Series.

BACKGROUND: Blunt popliteal artery injury (PAI) often requires complex vascular reconstruction because of extensive arterial damage. However, the frequency of vein graft use, required graft length, and their association with clinical outcomes remain unclear. This study aimed to evaluate vein graft characteristics and clinical outcomes in blunt PAI. METHODS: We retrospectively reviewed patients with blunt PAI treated at a single trauma center. Patient demographics, mechanism of injury, reconstruction method, vein graft length, soft tissue reconstruction, and amputation were investigated. Sever

A Cadaveric Case Report of an Anomalous Sural Nerve Course Through the Gastrocnemius.

The sural nerve is a superficial sensory nerve of the posterior leg that innervates the lateral foot. Anatomical variations in its course are clinically relevant, as they may affect surgical procedures, nerve blocks, and the risk of iatrogenic injury. This case report describes a rare variant in which the sural nerve pierces between the two heads of the gastrocnemius muscle, a course infrequently reported in the literature and important for surgical awareness. During routine lower-extremity dissection by first-year medical students at William Carey College of Osteopathic Medicine, Hattiesburg,

A novel approach to the treatment of functional popliteal artery entrapment syndrome in college athletes.

BACKGROUND: Functional popliteal artery entrapment syndrome (FPAES) is a rare disorder that results from hypertrophy and overuse injury of calf muscles, which thereby compress neurovascular structures in the popliteal fossa. The majority of FPAES cases are found in young athletes who present with symptoms during exercise. Most commonly, FPAES is surgically treated with muscle debulking utilizing a posterior S-shaped incision, but wound healing and neuropathy remain problematic in this approach. Here, we present our case series of college athletes undergoing FPAES repair using a novel medial ca

Penetrating Ballistic Trauma to the Popliteal Fossa Without Neurovascular Injury Despite High-Risk Trajectory: An Unusual Case Report.

Gunshot injuries involving the popliteal fossa are typically considered limb-threatening due to the dense concentration of major vascular and neural structures within the confined anatomical space posterior to the knee joint. Penetrating trauma to this region commonly results in injury to the popliteal artery, popliteal vein, tibial nerve, or common peroneal nerve, often necessitating urgent vascular or reconstructive intervention. Preservation of neurovascular integrity following ballistic trauma to the popliteal fossa is exceptionally uncommon. We report a rare case of an isolated gunshot in

Secondary Limb-Salvage Fifty Years After Vascular Injury: A Case Report.

A 73-year-old veteran presented to vascular surgery with a nonhealing left lower leg wound. Fifty years before the presentation, he sustained a gunshot wound to the left leg during combat. Emergent superficial femoral artery to popliteal artery bypass using vein graft and complex bone and soft-tissue repair resulted in successful primary limb salvage. He was entered into the Vietnam Vascular Registry at the time of his initial injury. On current presentation, he was diagnosed with infected myositis ossificans and bypass occlusion. He was originally offered wound care alone with possible amputa

Migration of Spinal Hardware Into the Popliteal Vein: A Case Report.

CASE: A 41-year-old woman with prior T12-L4 instrumentation for adolescent lumbar scoliosis presented with left posteromedial thigh pain initially diagnosed as an adductor strain. Two years later, she re-presented with knee stiffness, lower leg mottling, and paresthesia suspected to be reflex sympathetic dystrophy. Radiographs later revealed a 13.6-cm radiopaque object in the popliteal fossa and rod absence from the lumbar spine. The spinal rod was removed from the popliteal vein by orthopaedic surgery. CONCLUSION: Radiographs are an easily obtained, low-cost, and high-yield resource that are

Epidemiology of Vascular Injury Within the U.S. Department of Defense Trauma Registry From 2007-2023.

Background: Since WWII, the incidence of vascular trauma in U.S. military casualties has steadily risen, with the highest rates having been reported from the recent conflicts in Iraq and Afghanistan. Within civilian populations, the incidence of vascular trauma is lower, but with disproportionately higher rates of mortality, morbidity, and resource use for these injuries. Analyses of trauma databases have informed technological advances that best help our U.S. Military Health System (MHS) manage vascular injuries. We sought to perform a retrospective cohort analysis in military casualties with

Damage control strategy in peripheral vascular injury caused by firearm: A case report.

In peripheral vascular injury, the need for amputation following prolonged ischemia can be avoided through rapid revascularization. However, critically ill patients often cannot undergo single-stage surgery; thus, the use of vascular shunts temporarily alleviates ischemia, protecting the limb until definitive treatment. The present study describes the case of a 28-year-old male patient with a vascular injury in the right leg secondary to a firearm wound. Following admission, he developed grade IV hypovolemic shock. The patient underwent surgery, where a complete transection of the popliteal ar
