CASE: A 55-year-old man developed unilateral acute compartment syndrome (ACS) within 3 hours after bilateral total knee arthroplasties (TKA). Timely fasciotomies, followed by primary closure (within 48 hours), resulted in full neurological recovery within 2 weeks. By 1 year postoperatively, the patient had excellent range of motion (0-120° bilaterally) and patient-reported outcomes (KSS 200 and WOMAC 88.3). We hypothesize that the ACS was both due to bleeding from a small branch from the popliteal artery or vein that goes into vasospasm and due to the tourniquet ischemia-reperfusion phenomenon

Background: Popliteal venous aneurysms (PVA) are an uncommon but potentially severe condition due to their association with increased risk of recurrent pulmonary embolisms. Because of their rarity, their aetiology, natural history, and optimal treatment strategies have been poorly defined. The aim of this paper is to report a comprehensive systematic review on the treatment strategies and outcomes in PVA, summarizing current evidence. Methods: A systematic literature search was conducted in PubMed, Scopus, and Web of Science, covering studies published from database inception through February

Background: Treating varicosities originating from a popliteal fossa perforating vein (PFPV) is challenging due to their proximity to nerves and complex morphology. Data on endovenous laser ablation (EVLA) for PFPV varicosis are limited. Methods: This retrospective, single-center study reviewed all primary varicose vein surgeries from May 2021 to December 2024. Only primary PFPV varicosis cases with CEAP stage C2s or higher were included. Patients with recurrent disease or primary truncal insufficiency due to reflux from the saphenopopliteal junction were excluded. EVLA was performed using 147

INTRODUCTION: Popliteal artery pseudoaneurysms are rare but potentially life-threatening complications following orthopedic procedures, particularly osteotomies. These pseudoaneurysms, characterized by a hematoma caused by disruption of the arterial wall and contained by surrounding tissues rather than all arterial layers, can cause severe ischemic damage if not promptly diagnosed and treated. Early detection and timely surgical intervention are critical to prevent catastrophic outcomes such as thrombosis, embolization, or limb loss. This case highlights the challenges in diagnosing and managi

ABSTRACT: Popliteal artery injuries are rare even in busy urban trauma centers. The vast majority result from penetrating mechanisms of injury. These injuries are uncommon; therefore, few trauma surgeons and trauma centers have developed significant experience with their management. Experiences from both military and urban arenas of warfare consistently report the highest complications and amputation rates of all vascular injuries secondary to popliteal artery injuries. The popliteal artery is an end artery. Injuries cause significant ischemia, which threaten limb viability. From a surgical st

OBJECTIVE: Isolated infrapopliteal occlusive disease poses significant clinical challenges due to limited durable treatment options and poor limb salvage and survival rates. Although endovascular therapy (ET) has gained prominence for its minimally invasive approach, popliteal-distal bypass (PDB) provides an effective open approach that minimizes dissection and length of bypass required. This study aims to compare real-world outcomes of PDB vs infrapopliteal ET for chronic limb-threatening ischemia (CLTI) secondary to isolated infrapopliteal occlusive disease.

Popliteal vein (PV) injuries, which are less common than popliteal artery (PA) injuries, present with significant diagnostic and therapeutic challenges. These injuries are associated with a wide range of injuries. Early recognition and interventions provide better outcomes. Over time, the management of PV injuries has been shifted from ligation of the vessel to different forms of venous repair. We present a case of a 40-year-old female presenting with a supracondylar femur fracture with concomitant PA and PV injury. PA was repaired with a synthetic graft. However, the graft thrombosed due to v

Autologous vein grafting is a common method for revascularization in cases of traumatic limb arterial injuries. However, no clear guidelines have been established regarding the use of tourniquets following vascular reconstruction. We report a case of graft thrombosis that occurred during a planned orthopedic surgery one week after the initial revascularization. The patient was a 56-year-old man with a complex ligament injury of the left knee accompanied by a popliteal artery injury. After admission to our hospital, he underwent emergency popliteal artery reconstruction using an autologous vein

AIM: Deep vein thrombosis (DVT) is acknowledged as the primary complication associated with tibial plateau fractures (TPFs), necessitating the implementation of preventive measures such as physical exercise. While ankle pump exercise (APE) is an effective preventive measure, challenges such as insufficient monitoring and a shortage of nursing staff have been identified. Therefore, our study aimed to evaluate the efficacy of the self-developed telemonitored APE compared to conventional APE.
METHODS: The study was conducted in a randomized parallel-group design (Consolidated Standards of Report

This is a case of successful limb salvage using Viabahn stent grafts in a 47-year-old male with bilateral type III open femur fractures and traumatic disruption of the superficial femoral and popliteal arteries following a crush injury. Both femoral arteries were treated with endovascular repair using a total of four Viabahn endoprostheses (two per side), which restored lower extremity perfusion and avoided the need for open bypass. Open bypass was not feasible due to extensive venous trauma rendering the veins unsuitable for conduit harvest; Viabahn stents provided the only viable option for

The paper aims to investigate the difference between the effect of enoxaparin alone and enoxaparin + lower limb circulation (LLC) in the prevention of deep vein thrombosis (DVT) after intertrochanteric fracture in older people and to provide a basis for clinical anticoagulant therapy. A total of 121 patients undergoing hip fracture (HF) surgery were divided into the enoxaparin group (71 patients) and the enoxaparin + LLC group (50 patients) to compare the baseline features, changes in D-Dimer Index (DDI), and the incidence and distribution of DVT between the two groups. There was no significan

Deep vein arterialization provides an alternative approach to avoid limb amputations in patients with no other revascularization options. An athletic 28-year-old had an anterior cruciate ligament tear in an on-the-job injury. After an anterior cruciate ligament repair, he was found to have an occluded popliteal artery. An above-knee popliteal artery to posterior tibial vein bypass was performed because of a lack of bypass options. This led to the resolution of his rest pain. This salvage maneuver gave the patient at least 1 more year without amputation, showing that deep vein arterialization c

Popliteal artery pseudoaneurysms are uncommon and can present as delayed complications of trauma, with clinical symptoms that may not appear until many years after the initial injury. This report describes a remarkable case of a giant popliteal artery pseudoaneurysm diagnosed 24 years following a gunshot wound. The patient, a 71-year-old male, presented with a palpable swelling and restricted movement in the left popliteal region. Notably, he had sustained a gunshot injury to the same limb more than two decades earlier, during which no immediate vascular complications were identified. Diagnost

BACKGROUND: To evaluate the short-term clinical performance of Symvess (acellular tissue engineered vessel-tyod), formerly known as human acellular vessel, or HAV, compared with autologous vein for the treatment of extremity arterial injury.
METHODS: Subjects treated with acellular tissue engineered vessel (ATEV) from two clinical trials were propensity score-matched on a 1:2 basis to PROspective Observational Vascular Injury Trial (PROOVIT) registry subjects with the same injured artery and treated with autologous vein. Matching characteristics included age, sex, trauma type (penetrating vs

There have been many variations of knee arthroscopy posterolateral portal establishment techniques, yet few reports of associated complications. The objective of this study was to describe a standardized inside-out posterolateral portal development technique and to report data on its safety. We conducted a retrospective case series of adult patients who underwent knee posterior arthroscopy using a modified inside-out posterolateral portal development from January 1, 2019, to December 1, 2024. Neurovascular complications were defined as injury to either the popliteal artery, or popliteal vein,

BACKGROUND: Pediatric meniscal injury rates and their arthroscopic repair are on the rise. With the growing popularity of all-inside devices used in this potentially higher-risk population, studies examining the proximity of these devices to neurovascular structures are needed.
PURPOSE/HYPOTHESIS: The purpose of this study was to provide distances to neurovascular structures from all-inside devices placed arthroscopically at both high-risk approach angles and zones of the lateral menisci of pediatric cadaveric specimens. It was hypothesized that the neurovascular structures would be situated

Arthroscopic surgery, especially of the knee, is a minimally invasive procedure with low rates of complications. Rarely, this procedure can be associated with traumatic arteriovenous fistula (AVF) formation secondary to popliteal arterial injury. Here, we present the case report of a 33-year-old woman with symptomatic iatrogenic popliteal arteriovenous fistula formation following right knee arthroscopy. The patient has a 10-year history of left knee pain secondary to an unspecified knee injury. Magnetic resonance imaging (MRI) revealed loose bodies within the knee joint and a medial meniscal t

A 49-year-old man presented with repeated episodes of posterior knee effusions 11 months following right knee medial meniscus debridement. He was referred to Physical Medicine & Rehabilitation (PM&R) by his orthopaedic surgeon for further non-operative management. He had previously undergone ipsilateral vein stripping procedures with vascular surgery five months following his meniscal procedure. Magnetic resonance imaging (MRI) of the knee with contrast along with a subsequent ultrasound evaluation with no evident power Doppler flow confirmed the presence of a right posterior-medial gastrocnem

ObjectivesInfection of peripheral interposition grafts is a rare but devastating complication following aneurysm repair. Typically, graft infection necessitates explantation and, if possible, revascularization of the limb. However, treatment complexity varies substantially depending on the location and extent of infection. This case describes the management of a popliteal artery interposition graft infection.MethodsWe describe an 84 year old male with a history of left popliteal artery aneurysm repair with PTFE interposition graft (found on workup a year prior for a gangrenous great toe) who p

INTRODUCTION: There is no consensus yet whether delayed limb salvage procedures with an "ischemic time" of more than 6 hours are worthwhile, as these repairs are often complicated by reperfusion injury. Our study aims to determine the outcomes of delayed revascularization to validate our current treatment approach and assess areas for future improvement.
MATERIAL AND METHODS: We performed a retrospective cohort study on a highly selected group of patients who underwent delayed revascularization surgery for lower extremity traumatic vascular injuries in our institution from January 2008 to Jun
