We conducted a review of 450 single lower-limb amputations performed in our hospital in Bangladesh between July 1982 and June 1987. The incidence of amputation in the specific area of 1000000 inhabitants covered by the hospital was 0.75/10(3) per year. The indications for amputation were: limb ischemia in 366 patients (81%), traumatic crush injury in 45 (10%), diabetes-associated complications in 20 (5%), severe limb infection in 10 (2%), and neoplasm growth in 10 (2%). The ratio of above-knee (AK) to below-knee (BK) amputation was 1:65, and 36 patients (8%) required reamputation, 22 of whom h

A detailed examination was undertaken of hospitalized car occupants who sustained a lower limb injury in a frontal crash. The assessment included an analysis of the type, severity and causes of these injuries and mechanisms involved in lower limb fractures. The findings showed that fractures and dislocations occurred in 88% of lower limb injury cases, that more than half were from crashes < 48 km hour-1 and that the number of fractures was directly proportional with delta V. Ankle dislocations and foot fractures from the floor and toe pand were the most common injury-source combination overall

Blunt trauma to the knee of sufficient force to result in knee dislocation or fracture is commonly associated with popliteal artery injury. The challenging problem of bilateral popliteal artery injury has been rarely reported. We describe a case of bilateral popliteal artery injury after bumper crush injury between two automobiles that illustrates a successful method of management. Expeditious revascularization with minimum ischemia time was obtained by using the posterior approach, rather than the conventional medial approach, allowing two surgical teams to work simultaneously.

Patients suffering blunt leg trauma resulting in below-knee fracture, tibial artery injury, and soft-tissue damage are at major risk for amputation. In an attempt to identify the factors which may forecast limb loss despite vascular surgical repair, all patients with tibial fractures admitted between 1980-1988 were reviewed. Forty-four of 366 (12%) patients presented with clinical evidence of tibial artery injury. Twenty-seven of these 44 patients had angiographic evidence of at least one patent tibial vessel providing adequate distal flow. The remaining 17 patients required operative repair o

A case of lower limb crush injury is reported. A 16 year old female sustained severe soft tissue, muscle, skeletal and vascular lesions during a road traffic accident. She rapidly presented with severe haemorrhagic shock due to a popliteal arterial injury. At the scene, massive vascular loading by emergency physicians using both colloids and group O negative packed red cells was not enough to improve the haemodynamic state, even though there was adequate control of the bleeding artery. The pneumatic antishock trousers were therefore used despite the severe injuries to the legs, with rapid impr

The clinical presentation and management of 28 popliteal artery injuries following penetrating and blunt trauma during a 6 year period is reviewed. Clinical and Doppler evaluation identified an arterial injury in 24 (85 per cent) patients. In eight (29 per cent) patients with arteriovenous fistulae or false aneurysms, pedal pulses were palpable during initial assessment. Three (10.7 per cent) patients had limb amputation attributable to delayed vascular repair, crush injury and sepsis. End-to-end arterial repair following blunt trauma resulted in early failure with thrombosis in two patients.

One of the most easily misdiagnosed injuries, which can result in severe consequences in young patients, is a fracture involving the epiphyseal growth plate. The Salter and Harris type V fracture is a relatively uncommon injury resulting from a severe crushing force applied through the epiphysis to an area of the growth plate. It is most likely to occur in the region of the knee or ankle. The following is a case report of a comminuted compression fracture of the epiphysis of the fifth metatarsal head, with complete transverse midshaft fracture of the proximal phalanx of the fifth digit.

Apart from preliminary notices of present work, previous reports of experimental and clinical trials of the effects of a high-peak pulsed electromagnetic field (PEMF) on degeneration and regeneration of peripheral nerves lacked statistical analysis. Therefore, we designed experiments with standardised operative, histological, cytological and morphometric techniques to assess the effect of PEMF on lesions of the common peroneal nerves in paired male rats matched for age, environmental conditions and level and type of lesion. One of two types of lesion was induced in the left common peroneal ner

Forty-two fractures involving the distal femoral epiphyseal plate in 41 patients were reviewed. The average age at injury was 11.2 years. The follow up period ranged from one year to 20.3 years, with an average of 6.3 years. The Salter-Harris classification of epiphyseal plate injures proved to be a good indicator of the mechanism of injury and prognosis. Using stringent criteria to classify the end results, 2 out of 3 of the total group and 3 out of 4 of 29 Salter-Harris Type I and Type II injuries had good or excellent results. Type V crushing injuries of the growth plate, open injuries, fra

It has been hypothesized that a longitudinal compression force on a growing long bone may cause partial or complete closure of a physis by crushing the germinal cell layer. In the absence of a fracture of the physis, the roentgenograms at the time of injury would be normal. Thus, this diagnosis can be made only in retrospect. In order to prove such a hypothesis, the following criteria would have to be met: (a) normal findings on roentgenograms taken in at least two planes at the time of injury, (b) no treatment, and (c) subsequent positive roentgenographic findings showing partial or complete

A case of crush injury to the lower extremities of an ironworker involved in the demolition of a bridge is presented. Extrication of the entrapped limbs was complicated by weakening of the bridge support due to acetylene torch cutting. After 3 hours the decision was made to complete the traumatic disarticulation of the right knee on site. Essential elements in the successful completion of such field intervention are presented and the unusual features of the case are emphasized.

Supracondylar fractures of the femor or humerus and fractures of the radius, ulna, tibia or fibula, plus crush injuries of the limbs, most likely should not be manipulated in the field because of the potential for traumatizing adjacent nerves, veins and arteries. An emergency splinting system that allows immobilization without repositioning has been developed. Furthermore, fractures in or about the elbow, wrist, knee, or ankle should not be subjected to reduction in the field, unless there are compelling reasons to justify the risk of producing significant additional trauma. Conservative immob
