Forensic physician's dilemma: Attempted suicide or homicide? A case report of an atypical cut-throat injury.

Cut-throat injuries are among the most serious forms of sharp force trauma encountered in medico-legal practice and frequently present complex interpretative challenge as they may suggest suicide, homicide or rarely, be accidental. particularly when classical morphological features are absent. In this case a 48-year-old woman with a history of depressive disorder presented with a deep transverse incised wound to the anterior neck, requiring emergency tracheostomy and surgical repair. Operative findings revealed two full-thickness incisions involving the thyroid cartilage and thyrohyoid membran

Temporal changes and associated factors for elevated serum gentamicin levels in continuous local antibiotic perfusion (CLAP) therapy: A multicenter analysis.

INTRODUCTION: Fracture-related, periprosthetic joint, and surgical site infections are difficult to eradicate once biofilm develops. Continuous local antibiotic perfusion (CLAP) delivers sustained high local gentamicin levels with low systemic exposure, but unexpected serum gentamicin elevation has been reported. This study aimed to describe temporal changes in serum gentamicin during CLAP and identify factors associated with levels ≥2 μg/mL. METHODS: We conducted a multicenter study within the Trauma Research Group of Nagoya. Among 205 patients treated with CLAP, 131 met the inclusion criter

Estrogen and Lysyl Oxidase: How Crosslink Changes Increase Biological Female Athlete ACL Injury Risk.

Across sports that involve cutting, pivoting, and single-leg jump landings, biological female athletes sustain anterior cruciate ligament injuries at 2- to 9-fold greater rates than males. The mechanical resilience of the anterior cruciate ligament depends on extracellular matrix stabilization. Crosslinking, primarily mediated by lysyl oxidase, is essential to extracellular matrix maturation, to resisting the tensile and shear forces encountered during athletic activity, and to long-term anterior cruciate ligament health. Estrogen-mediated lysyl oxidase modulation may weaken the extracellular

Burking as a cause of homicidal asphyxia: A forensic case report.

Burking a rare and distinct form of homicidal asphyxia is when the perpetrator(s) obstruct all the external respiratory orifices while simultaneously applying thoracic compression. It is historically associated with the 19th-century serial killers Burke and Hare. It is a complex form of 'complicated asphyxia' which can present significant diagnostic challenges due to its potential for subtle or overlapping postmortem findings. This report describes the autopsy and forensic investigation of a 35-year-old male victim of a suspected homicide. External examination revealed periorbital ecchymosis,

Continuous integral debridement: implementation in the Asia-Pacific region.

The health-economic burden of hard-to-heal wounds is rising across the Asia-Pacific (APAC) region, yet regional data remain sparse, particularly for venous leg ulcers and pressure injuries. Effective wound bed preparation, including management of bioburden and biofilm, is central to healing outcomes, but debridement practice across APAC is inconsistent. In May 2026, the Journal of Wound Care published the continuous integral debridement (CID) concept, built around a four-step cleanse, debride, cleanse, dress (CDCD) cycle designed to standardise debridement in everyday clinical practice. This a

Fatal hyperthermia and heatstroke in children and adults: a ten-year retrospective autopsy-based analysis.

Hyperthermia is a preventable cause of death that lacks a specific postmortem finding, and its diagnosis relies on scene and environmental information and on the exclusion of alternative causes. We characterized the demographic, environmental, and pathological features of hyperthermia deaths over a ten-year period at a national forensic reference board and compared pediatric and adult patterns descriptively. Twenty-two cases certified as hyperthermia or heatstroke between 2015 and 2024 were reviewed retrospectively; investigation files, scene and meteorological records, clinical documents, aut

Impact of virtual monoenergetic images on the assessability of lower extremity arteries in (Poly-) trauma photon-counting detector CT.

BACKGROUND: Computed tomography (CT) is state of the art imaging modality in patients after severe trauma. A double-bolus contrast media protocol can be used to combine both arterial and parenchymal contrast in one scan. In case of lower extremity trauma, the CT scans can be extended to include the legs in order to rule out vascular pathologies. Virtual monoenergetic (VME) images derived from a Photon-Counting detector CT (PCDCT) proved to be helpful in vascular diagnostics and could therefore also add information in these extended trauma scans. Aim of this study was to determine the VME recon

Complete laryngotracheal separation following blunt neck trauma: A forensic reconstruction of an unusual fatal injury.

INTRODUCTION: Complete laryngotracheal separation (CLTS) is an exceptionally rare and life-threatening consequence of blunt neck trauma. Because external cervical injuries may be minimal or nonspecific, diagnosis can be delayed in both clinical and forensic settings. CASE PRESENTATION: A 35-year-old man was found unconscious at the roadside and died two hours after hospital admission despite emergency management. Autopsy revealed extensive cervical subcutaneous emphysema, complete laryngotracheal separation, bilateral thyroid cartilage fractures, cricoid cartilage disruption, destruction of t

Fatal gastrointestinal complications of pica: Medico-legal findings from two forensic autopsy investigations.

BACKGROUND: Pica is a behavioral disorder characterized by persistent ingestion of non-nutritive substances. Although primarily described in psychiatric contexts, it may lead to severe gastrointestinal complications and death. In forensic practice, such cases raise important medico-legal issues when the cause of death is unclear. OBJECTIVES: To describe fatal gastrointestinal complications of pica, identify morphological indicators of chronic ingestion at autopsy, and demonstrate how systematic forensic examination contributes to accurate cause-of-death determination. METHODS: Two fatal case

What Factors Increase the Risk of Dislocation After Primary Total Hip Arthroplasty Using the Direct Anterior Approach?

BACKGROUND: While the direct anterior approach (DAA) for total hip arthroplasty (THA) is associated with a lower risk of dislocation, this complication has not been eliminated and can be acutely disabling when it occurs. This study aimed to identify patient, surgical, and radiographic factors that contribute to dislocation risk for the DAA based on primary THAs performed over a 15-year period. METHODS: Using an institutional database, we identified 8,658 DAA THAs performed by four surgeons from March 2009 through May 2024. A Cox proportional hazards regression was performed to determine how p

Fatal arrhythmia associated with myocardial contusion caused by a horse kick: A forensic autopsy case.

BACKGROUND: Horse-related injuries often involve high-energy blunt trauma that can be fatal. In cases of chest trauma associated with horse kicks, death is typically attributed to structural cardiac injuries such as cardiac rupture or tamponade. Fatal arrhythmia associated with myocardial contusion following a horse kick has rarely been reported in the forensic literature. CASE PRESENTATION: A 48-year-old man was kicked in the left lower chest by the right hind limb of a Thoroughbred horse while handling the animal. The patient collapsed immediately after the impact. Fatal firearm injury during peaceful assembly: A forensic case report with human rights implications.

Nepal's government banned social media in early September 2025 as it was exposing nepotism and the excessive and luxurious lifestyles of families of influential political leaders. The ban enraged Nepali youth particularly those aged 14-29 known as Generation Z (Gen Z) who accordingly gathered to protest peacefully and lawfully as allowed by Article 17, Nepal Constitution 2015. However, the police opened fire on them using live ammunition causing 77 deaths. We present a case of a 22-year-old man who had been protesting and died from gunshot injuries inflicted by the police during these demonstr

Development of a novel universal primer set with high sensitivity and specificity for detection of diatom.

The diatom test has long been used as an ancillary examination in the diagnosis of drowning. The conventional diatom test requires the handling of strong acids and depends on the investigator's skill and expertise. Several issues are also associated with the diatom test, such as sample contamination and diatom loss during testing. We have developed a novel primer set for detecting diatoms using real-time PCR. The primers were designed based on the rbcL gene as a marker, and the amplicon length was 68 bp. A high detection sensitivity (0.1 pg DNA) was achieved for all diatom species tested, and

Terminal ballistics of the stray bullet in soft tissue simulant -proof-of-concept.

A stray bullet is a projectile that, after being fired, strikes an unintended object, such as a bystander. Accident of this kind can occur due to several reasons, such as crossfire, celebratory gunfire and sports shooting related incidents. Even though the forensic importance of such events is great, they may also present diagnostic challenges to forensic practitioners. In case of an incident involving a potential stray bullet, distinguishing between homicide and accident is important. Estimating the shooting distance can be an important part of the process as stray bullets may be lethal even

Cellular basis of accelerated whole-tooth regeneration.

Teeth are ectodermal organs that have, throughout their long evolutionary history, retained the capacity for full regeneration and replacement, even in adult stages. Yet, because most mammals (e.g., humans, mice) lack lifelong dental replacement, we do not fully understand its tempo and mode, and we do not have a clear picture of the cell populations and signals that contribute to the process. Here, we used cichlid fishes from Lake Malawi, species that differ in tooth formula (tooth shape and number) but share one-for-one tooth replacement, to (i) explore the tempo of dental replacement after

Non-accidental burns in a neurodevelopmentally vulnerable child: A case of institutional child abuse.

Paediatric burns may result from accidental or non-accidental mechanisms, with diagnostic challenges arising when histories suggest dubious accidental causes. We report an 8-year-old boy, placed in a local temple due to longstanding behavioural difficulties, who sustained burns reportedly from hot water spillage. Examination revealed deep burns affecting the left face and neck, lower back, buttocks, perineum and posterior thighs. The distribution and severity were inconsistent with the reported explanation, raising suspicion of non-accidental injury. Multiple facial angiofibromas prompted mult

Protest-related injuries during the Capitol Hill Autonomous Zone protest in Seattle, Washington, USA in 2020.

INTRODUCTION: George Floyd's death in 2020 galvanised large protests around the country, including the emergence of the Capitol Hill Autonomous Zone (CHAZ) in Seattle, Washington, a non-policed, organised protest region that may have differing injury risks than other regions. We sought to quantitatively describe characteristics of injuries related to protests documented at visits to two nearby major emergency departments, including the only Level 1 trauma centre in the state. METHODS: Using the International Classification of Diseases, 10th Revision code inclusion criteria, we identified 1938
