[Uterine rupture superficial externum--a case report].

The article presents a case of external superficial rupture of the uterus--along the left lateral margin--with ruptured posterior lamina of the broad uterine ligament. The complication developed in a 29-year-old patient--following delivery from breech fetal presentation with footling, in 41 hbd, 4th delivery. The patient had no history of any uterine operations, reported no injury to the uterus or inflammations of her sex organs. The symptoms of bleeding into the abdominal cavity and hypovolemic shock developed during early postpartum period. The operation was performed on an emergency basis:

Tibial displacement and rotation during seated knee extension and wall squatting: a comparative study of tibiofemoral kinematics between chronic unilateral anterior cruciate ligament deficient and healthy knees.

BACKGROUND: Following anterior cruciate ligament (ACL) rupture, the knee becomes unstable with alterations in joint kinematics including anterior tibial displacement (ATD), and internal tibial rotation. Therapeutic exercises that promote faulty kinematics should be discouraged, especially early post-reconstruction, to avoid graft stretching and possibly longer-term osteoarthritis. Our study aimed to compare ATD and tibial rotation during two commonly prescribed exercises, namely: open kinetic chain (OKC) seated extension and closed kinetic chain (CKC) single leg wall squatting in ACL-deficient

Computed tomography of coxofemoral injury in five mute swans (Cygnus olor).

Five mute swans (Cygnus olor) were presented with inability to stand or with abnormal positioning of a leg. Clinical examinations indicated the possibility of femoral fractures or coxofemoral luxations. The suspected diagnosis was proven by means of computed tomography (CT), while superimposition of gastrointestinal contents or other artefacts limited radiographic diagnosis in three birds. A typical CT sign for lesions of the coxofemoral joint apart from femoral displacement was haemorrhage within the pelvic bones (especially around the acetabulum), found in four of the five birds. Small femor

Treatment of early-onset Gorham syndrome with 8-year follow-up.

A case of Gorham disease with several years of follow-up is reported. At birth he had a mass in the thigh which was had pathology demonstrating a lymphangioma. By age 3 years, he had lymphedema of the ipsilateral foot and discrepant leg lengths. Radiography revealed heterogenous dystrophy of the bones and osteolysis of the hallux. At age 6, a spontaneous fracture of the right tibia was treated with surgery. Histopathology of a bone sample demonstrated bone remodelling, fibrous tissue, and large vascular lacunas within bone tissue, bordered by cells expressing the lymphaticmarker D2-40. At 8 ye

Treatment of long bone nonunions: factors affecting healing.

PURPOSE: Nonunions of the upper and lower extremity have been associated with pain and functional deficits. Recent studies have demonstrated that healing of these nonunions is associated with pain relief and both subjective and objective functional improvement. The purpose of this study was to determine which patient and surgical factors correlated with successful healing of a nonunion following surgical intervention. METHODS: Between September 2004 and February 2008, all patients with a "long bone nonunion" presenting to our academic trauma service were enrolled in a prospective data base. B

Focal traumatic brain stem injury is a rare type of head injury resulting from assault: a forensic neuropathological study.

INTRODUCTION: Brainstem haemorrhage is common in cases of head injury when it is associated with space-occupying lesion and increases in the intracranial pressure (duret haemorrhage), in cases of diffuse axonal injury (in dorso-lateral quadrant) and diffuses vascular injury (in the periventricular tissue). However focal traumatic brainstem injury is rare. MATERIAL AND METHOD: We identified 12 cases of focal traumatic brainstem injury from review of 319 case of head injury. The head trauma had been caused by different mechanisms of complex fall from height and assault. 10/12 are associated wit

Physical therapy services utilization and length of stay for obese patients following traumatic leg fracture.

BACKGROUND: Obesity is an increasing epidemic that can complicate the treatment of simple injuries and can increase health care costs. The purpose of the present study was to determine whether obesity is a factor in the utilization of inpatient physical therapy services and length of stay following a traumatic lower leg fracture. METHODS: A retrospective study of patients admitted to the hospital in 2005 and 2006 with a primary discharge diagnosis of lower leg or ankle fracture was conducted. Inclusion criteria were age > or = 18 years, only 1 involved lower extremity, and nonweight-bearing o

Genome-wide association of an integrated osteoporosis-related phenotype: is there evidence for pleiotropic genes?

Multiple musculoskeletal traits assessed by various methods at different skeletal sites serve as surrogates for osteoporosis risk. However, it is a challenge to select the most relevant phenotypes for genetic study of fractures. Principal component analyses (PCA) were conducted in participants of the Framingham Osteoporosis Study on 17 measures including bond mineral density (BMD) (hip and spine), heel ultrasound, leg lean mass (LLM), and hip geometric indices, adjusting for covariates (age, height, body mass index [BMI]), in a combined sample of 1180 men and 1758 women, as well as in each sex

Previous fractures at multiple sites increase the risk for subsequent fractures: the Global Longitudinal Study of Osteoporosis in Women.

Previous fractures of the hip, spine, or wrist are well-recognized predictors of future fracture, but the role of other fracture sites is less clear. We sought to assess the relationship between prior fracture at 10 skeletal locations and incident fracture. The Global Longitudinal Study of Osteoporosis in Women (GLOW) is an observational cohort study being conducted in 17 physician practices in 10 countries. Women aged ≥55 years answered questionnaires at baseline and at 1 and/or 2 years (fractures in previous year). Of 60,393 women enrolled, follow-up data were available for 51,762. Of these,

Utility of single photon emission computed tomography/computed tomography imaging in evaluation of chronic low back pain.

INTRODUCTION: Abnormal morphologic findings in imaging were thought to explain the etiology of low back pain (LBP). However, it is now known that variety of morphologic abnormalities is noted even in asymptomatic individuals. Single photon emission computed tomography/computed tomography (SPECT/CT) could be used to differentiate incidental findings from clinically significant findings. OBJECTIVE: This study was performed to define the SPECT/CT patterns in patients with LBP and to correlate these with clinical and magnetic resonance imaging (MRI) findings. MATERIALS AND METHODS: Thirty adult

Staged protocol for the treatment of chronic tibial shaft osteomyelitis with Ilizarov's technique followed by the application of intramedullary locked nail.

Open tibial shaft fractures are the most common open fractures, and many complications can occur. During the treatment period, infection leading to osteomyelitis was the most common complication. However, no consensus exists regarding the ideal management for such cases in the literature. The purposes of this retrospective study were to review the treatment of patients with chronic tibial shaft osteomyelitis over the past 14 years who were referred to the authors' institution and to provide a staged protocol for spontaneous wound healing. The staged protocol included: (1) radical debridement fo

[Treatment of foot drop in orthopaedic practice].

Foot drop is a symptom of many diseases and leads to an unaesthetic limping gait, falls and injuries. The most common cause of foot drop is injury to the common peroneal nerve, which innervates ankle joint and foot extensors. Other causes of foot drop include: sciatic nerve injury, lumbar plexopathies, L4/L5 radiculopathy, central nervous system related neuropathies (anterior horn cell disease, brain tumor). Also compartment syndromes of the lower leg can lead to foot drop deformity. Ankle-foot orthoses (AFO) are helpful in the treatment of paralyzed extensor muscles of the foot. The surgical

Bizarre allegation of negligence in fabricated pin migration--a case report.

Kirschner's wires (popularly known as 'K-wires' or 'pins') are used commonly in various orthopaedic surgical fixations. Around the shoulder joint such pins are used for fixation of acromioclavicular joint, sternoclavicular joint, proximal humerus and clavicle. In such fixations, spontaneous loosening and extraction of pins would not cause as much anxiety and worry to the surgeon as a pin which breaks and migrates within the body. Such alarm is not just due to the potential damage that the migrating pin can cause, but also due to fear of misinterpretation of the event in legal suits as negligen

Lower limb deformity following proximal tibia physeal injury: long-term follow-up.

BACKGROUND: Proximal tibial physeal injuries are quite rare, but their complications can be of great importance. The purpose of this study was to evaluate the effect of this injury on the axis and length of a child's limb. MATERIALS AND METHODS: This study focused on 12 patients with proximal physeal injury of the tibia (8 boys and 4 girls; mean age at the time of injury: 8.9 years). Injuries were classified according to the Salter-Harris scheme into 5 types (type II--7 patients, type III--3 patients, type IV--1 patient, type V--1 patient). In 5 cases, a coexisting fracture of the injured lim

Failure of knee arthroplasty secondary to inadequate technique in cephalomedullary nailing of a subtrochanteric hip fracture.

A case of total knee replacement loosening caused by a failed cephalomedullary nail is presented. The nail was inserted for the treatment of an unstable subtrochanteric femoral fracture. The distal locking screw of the nail subsequently fractured allowing the leg to shorten and causing the nail to displace the femoral component of the knee replacement leading to a valgus alignment and deformity of the knee. In order to revise the knee the long γ nail had to be replaced by a shorter nail to allow a femoral component with a long stem to be used. This case illustrates the importance of using shor

Posttraumatic chronic patellar dislocation treated by distal femoral osteotomy and medial patellofemoral ligament reconstruction.

Chronic patellar dislocation is a rare condition where the patella remains dislocated throughout knee range of motion during flexion and extension. In adults, the delayed presentation of this condition is often due to symptoms caused by the onset of severe secondary osteoarthritis. To the authors' knowledge, all of the cases reported in the literature have been treated by patellofemoral or total knee replacements depending on patient age and the extent of the arthritis. This article describes a rare case of a 22-year-old woman who sustained a traumatic chronic patellar dislocation for 5 months

Ipsilateral femoral neck and shaft fracture in children: a report of two cases and a literature review.

Concomitant ipsilateral fractures of the neck and shaft of the femur in children are rare. The most recent report in this context found a total of only nine reported cases (<12 years of age) following a search of the indexed English literature. These injuries occur in children due to high-velocity trauma, and there is no generally accepted method of treatment. We report three additional cases from the literature and two cases of our own. In our cases, one had a residual 10° varus deformity at the subtrochanteric level in the femur, but this did not affect hip function. Another patient exhibite

[Intrapelvic dislocation of lag screw in gamma nailing].

Within the last decade, intramedullary nailing systems have gained enormous popularity in the treatment of fractures of the trochanteric region. However, these implants are not free of any complications. Commonly reported treatment failures include stress-derived femur fracture, rotational displacement, varus deformity of the femoral neck and head, and implant cutout through the femoral neck. So far, there are only five cases known in the literature describing dislocation of the medial leg screw into the pelvis. The patient history, operative intervention, and course are reported. The incidenc

[Open segmental fractures of the tibia treated by external fixation].

INTRODUCTION: Open segmental fractures of the tibia are rare but severe injuries. In these fractures the wide zone of injury (damage of all structures of the lower leg) creates very unsuitable biological conditions for healing of the fracture. OBJECTIVE: The aim of our work was to present the results of treatment of patients with segmental open fractures of the tibia treated by external fixation. METHODS: We analyzed treatment results of 21 patients with open segmental tibial fractures who were treated using the method of external fixation at the Clinical Center Nis from January 1, 1995 to J

Stress fracture of the first metatarsal after total knee arthroplasty: two case reports using gait analysis.

Stress fractures after total knee arthroplasty (TKA) occur mainly in patients with considerable deformity of the knee. In addition, the majority of these fractures after TKA involve the hip joint. We present two cases of stress fractures of the first metatarsal after TKA in patients with severe varus deformity. Correction of leg alignment and pain reduction obtained by TKA lead to stress fracture of the bone. Gait analysis was carried out for both cases in order to clarify the gait characteristics of the stress fracture. As a result, side-to-side differences of the distance between first metat
