SPECT/CT in differentiation of pseudarthrosis from other causes of back pain in lumbar spinal fusion: report on 10 consecutive cases.

PURPOSE: SPECT fused with computed tomography (CT) provides a new approach for more accurate diagnosis of pseudathrosis after spinal fusion procedures. The aim of this study was to compare the findings of SPECT fused with CT (SPECT/CT) with those of CT alone for the diagnosis of pseudarthrosis. MATERIALS AND METHODS: SPECT and CT of 10 consecutive patients with recurrence of back and/or leg pain and with suspicion of pseudarthrosis on conventional radiologic imaging were analyzed retrospectively. All had previously undergone anterior and/or posterior lumbar fusion techniques. Presence of scre

Motor nerve injuries following the minimally invasive lateral transpsoas approach.

OBJECT: The aim of this study was to determine the incidence of motor nerve injuries during the minimally invasive lateral interbody fusion procedure at a single academic medical center. METHODS: A retrospective chart review of 118 patients who had undergone lateral interbody fusion was performed. Both inpatient and outpatient records were examined to identify any new postoperative motor weakness in the lower extremities and abdominal wall musculature that was attributable to the operative procedure. RESULTS: In the period from 2007 to 2011 the lateral interbody fusion procedure was attempte

Free vascularised fibular graft for neglected femoral neck fractures in young adults.

Neglected femoral neck fractures in young adults pose a great challenge to orthopedic surgeons because of the high risks of nonunion and osteonecrosis. We attempted to determine whether free vascularised fibular grafting through an anterior approach could enhance bone union, improve hip function and reduce complications in the treatment of such fractures. Thirteen patients with neglected femoral neck fractures were treated with a free vascularised fibular graft through an anterior approach between 2004 and 2008. The mean age was 30.9 years (range, 17-47 years). The average delay between injury

Comparison of kinematics of ACL-deficient and healthy knees during passive flexion and isometric leg press.

BACKGROUND: Studying the kinematics of the ACL deficient (ACLD) knees, during different physiological activities and muscle contraction patterns, can improve our understanding of the joint's altered biomechanics due to ACL deficiency as well as the efficacy and safety of the rehabilitations exercises. METHODS: Twenty-five male volunteers, including 11 normal and 14 unilateral ACLD subjects, participated in this study. The kinematics of the injured knees of the ACLD subjects was compared with their intact knees and the healthy group during passive flexion and isometric leg press with the knees

Chronic ankle instability and corticomotor excitability of the fibularis longus muscle.

CONTEXT: Neuromuscular deficits are common in people with chronic ankle instability (CAI). Corticomotor pathways are very influential in the production of voluntary muscle function, yet these pathways have not been evaluated in people with CAI. OBJECTIVE: To determine if corticomotor excitability of the fibularis longus (FL) differs between individuals with unilateral CAI and matched control participants without CAI. DESIGN: Case-control study. SETTING: Laboratory. Patients or Other Participants: Ten people with CAI (4 men, 6 women; age = 21.2 ± 1.23 years, height = 175.13 ± 9.7 cm, mass =

Pseudoacetabulum a subtle radiological feature of a chronic dislocated total hip replacement.

We report the presence of an unusual radiological feature of formation of pseudoacetabulum in a chronically dislocated and asymptomatic total hip arthroplasty. An elderly demented patient with a history of recurrent dislocations presented to us after an unwitnessed fall and leg length discrepancy. Radiographs showed a dislocated hip arthroplasty with a well-defined, concentric, radio-opaque shadow around the dislocated femoral head suggestive of a pseudoacetabulum. Previous radiographs revealed formation of heterotopic ossification in the soft tissues surrounding the hip. The dislocated joint

Necrotizing fasciitis of the lower limb following rat bites.

Necrotizing fasciitis is a rare life-threatening condition characterized by rapidly spreading infection in the subcutaneous tissues. Most of the time, the infection is precipitated by an event that produces a break in the epidermis. The authors report the case of a 55-year-old farmer who developed a necrotizing fasciitis of the left lower limb following a rat bite. The patient had serial debridement of his wound and broad-spectrum antibiotics and subsequent partial thickness skin grafting. His total duration of admission was 93 days. Early diagnosis, an aggressive wound debridement, and broad-

Reconstruction and anticipatory Langenskiöld procedure in traumatic defect of tibial medial malleolus with type 6 physeal fracture.

Traumatic osteoarticular or ligament defect of the tibial medial malleolus is a rare entity in children. Associated lesions may include soft tissue and joint defect, subsequent instability of the ankle, and growth arrest. We report here, the case of an 11-year-old boy, a victim of a severe trauma to the ankle, managed by an original technique. It combined a reconstruction by a composite iliac crest and gluteal fascia graft, an anticipatory Langenskiold procedure, and a serratus anterior muscle flap. This original technique proved to be a suitable alternative in this type of trauma.

Hip dislocation during lengthening of congenital short femur.

Hip dislocation is a serious but rarely reported complication of femoral lengthening in congenital short femora. We report a retrospective series of three hip dislocations that occurred during lengthening, analyze the factors contributing to the dislocation, and discuss the treatment of this difficult problem. The average lengthening achieved was 9 cm. We found progressive acetabular dysplasia and decreasing center edge angle. Closed reduction failed. Treatment included soft tissue release, open reduction with femoral shortening, and acetabular procedures. At a mean follow-up of 4.4 years, two

Does swaddling influence developmental dysplasia of the hip?: An experimental study of the traditional straight-leg swaddling model in neonatal rats.

BACKGROUND: The overall effect of swaddling has been controversial for centuries. Its positive effect on the psychological development of the infant has popularized it in European and North American countries, but its negative effect on the development of the hip is of great concern. In our experiment, the influence of straight-leg swaddling in an animal model was observed radiographically and histologically. METHODS: One hundred and twelve neonatal rats were divided into a control group and three experimental groups that were swaddled with use of surgical tape in a manner simulating the huma

The techniques of soft tissue release and true socket reconstruction in total hip arthroplasty for patients with severe developmental dysplasia of the hip.

PURPOSE: Total hip arthroplasty (THA) is an effective procedure for developmental dysplasia of the hip (DDH); however, it is sometimes difficult to complete for severe cases because of femoral head dislocation, dysplasia of the acetabulum and the femur, disparity in limb length, soft tissue contraction, and muscular atrophy. We aimed at exploring the efficiency of the techniques of release and balance of soft tissues and reconstruction of true socket THA for patients with severe DDH. METHODS: From January 2000 to January 2009, 46 adult patients with severe DDH (50 hips) were included in this

Cementless total hip replacement for severe developmental dysplasia of the hip: our experience in Crowe's group IV.

Total hip replacement in developmental dysplasia of the hip grade IV of Crowe's classification presents some difficulties. In this study, we present our results of the treatment for this pathology, also describing the surgical techniques used and the complication we had. In this paper, 18 total hip replacements in developmental dysplasia of the hip Crowe IV were studied clinically and radiologically before and after surgery, with a mean follow-up of 4.2 years (min: 1 year). The average Harris Hip Score improved from 52 to 89. The average leg lengthening was 36 mm. When a subtrochanteric shorte

Dislocation of primary total hip arthroplasty and the risk of redislocation.

6554 primary total hip arthroplasties were reviewed. Risk factors for dislocation were analysed to assess which were important in terms of predicting recurrent instability. The patients risk of having a second dislocation was independently associated with the surgical approach adopted (p = 0.03) and the time to first dislocation from the primary hip replacement (p = 0.002). Early dislocators whose surgery was performed through an anterolateral approach had less recurrence than late dislocators through a posterior or transtrochanteric approach. None of the other risk factors including head size

Factors influencing 2-year health care costs in patients undergoing revision lumbar fusion procedures.

OBJECT: Revision lumbar fusion procedures are technically challenging and can be associated with tremendous health care resource utilization and cost. There is a paucity of data regarding specific factors that significantly contribute to increased cost of care. In light of this, the authors set out to identify independent risk factors predictive of increasing 2-year direct health care costs after revision lumbar fusion. METHODS: One hundred fifty patients undergoing revision instrument-assisted fusion for adjacent-segment disease (50 cases), pseudarthrosis (47 cases), or same-level stenosis (

The natural history of asymptomatic lumbar canal stenosis in patients undergoing surgery for cervical myelopathy.

We retrospectively examined the prevalence and natural history of asymptomatic lumbar canal stenosis in patients treated surgically for cervical compressive myelopathy in order to assess the influence of latent lumbar canal stenosis on the recovery after surgery. Of 214 patients who had undergone cervical laminoplasty for cervical myelopathy, we identified 69 (32%) with myelographically documented lumbar canal stenosis. Of these, 28 (13%) patients with symptomatic lumbar canal stenosis underwent simultaneous cervical and lumbar decompression. Of the remaining 41 (19%) patients with asymptomati
