Nucleocytoplasmic transport in gynaecological cancer: From mechanisms to therapeutic potential (Review).

BACKGROUND: In recent times due to the advancement of haemodialysis techniques, there is increase in the life span of the patients with end-stage renal disease, which tends to increase the risk of complications of bacteraemia due to skin penetration for blood access and leading to propensity for more skeletal and spinal complications. Hence, early diagnosis of spine infection and early initiation of appropriate therapy are important in ensuring successful treatment and preventing further morbidity. MATERIALS AND METHODS: The study included 22 males and 12 females who were known case of end-st

Pharmacokinetics of Dexamethasone in Tuberculous Meningitis.

BACKGROUND: Dexamethasone is recommended as adjunctive therapy for tuberculous meningitis (TBM). Co-administration with rifampicin is expected to reduce dexamethasone exposure in TBM, an effect that may be more pronounced with the higher rifampicin doses currently being evaluated in clinical trials. METHODS: This pharmacokinetic study was nested in a randomized controlled trial comparing the safety of high-dose rifampicin (oral, 35 mg/kg; intravenous, 20 mg/kg) plus linezolid, with or without aspirin, versus standard-dose rifampicin (10 mg/kg) for adults with HIV-associated TBM. Possible tuberculous meningitis presenting with predominant voiding dysfunction in an elderly patient: A case report.

Tuberculous meningitis (TBM) typically presents with neurological symptoms. Voiding dysfunction in TBM is usually attributed to spinal cord involvement. We report a rare case of possible TBM co-existing with possible prostatic tuberculosis, presenting primarily with urinary symptoms in the absence of spinal pathology. A 69-year-old male presented with a two-year history of progressive urinary retention, urgency, and frequency. Initial management for benign prostatic hyperplasia was ineffective. He subsequently developed fever, headache, and dizziness. Cerebrospinal fluid (CSF) analysis reveale

Patterns and Factors Associated With Cerebral Infarction on MRI in Tuberculous Meningitis: Secondary Analysis of the ACT-TBM Trial.

BACKGROUND: Cerebral infarction is a frequent and serious complication of tuberculous meningitis (TBM), contributing substantially to morbidity and mortality. Moreover, studies on infarct patterns and associated factors/predictors remain limited in TBM. ACT-TBM trial (Aspirin or Clopidogrel Therapy in the Treatment of Tuberculous Meningitis) evaluated the efficacy and safety of adjunctive antiplatelet therapy (aspirin or clopidogrel) to standard antitubercular therapy in TBM for the occurrence of stroke or cerebral infarction. Here, we conducted a secondary analysis of the ACT-TBM trial to cha

Cerebrospinal Fluid Hypo-inflammation Drives Mortality in HIV-Associated Tuberculous Meningitis.

BACKGROUND: Outcomes in tuberculous meningitis (TBM) are closely linked to host inflammation. Anti-inflammatory corticosteroid therapy improves survival in HIV-negative TBM, but not in people with HIV, among whom TBM is fatal in 40-50% of cases. Therefore, we investigated how mortality is associated with the local immune response in people with HIV-associated TBM. METHODS: We measured baseline concentrations of immune signaling mediators in cerebrospinal fluid (CSF) of 149 adults with HIV in Uganda, who presented with definite or probable TBM. Participants received both antimycobacterial and

Development and evaluation of a nomogram model for predicting prolonged hospitalization after spinal tuberculosis focus decompression, fusion, and internal fixation surgery.

BACKGROUND: Spinal tuberculosis remains a significant clinical challenge in high-prevalence regions. Despite advances in medical treatment, surgical interventions, such as debridement, decompression, and fusion, are often required, but their complexity increases perioperative morbidity and prolongs hospitalization. Identifying preoperative predictors of extended hospital stay may improve patient management and resource allocation. METHODS: In this retrospective study, 256 patients with confirmed spinal tuberculosis who were treated between January 2021 and December 2024 were included. Prolong

Unveiling the microbiological experience in exploring spinal TB cases from a tertiary care center.

PURPOSE: Spinal tuberculosis, a severe form of extrapulmonary tuberculosis, poses significant diagnostic and therapeutic challenges, often leading to neurological complications and deformity. Microbiological confirmation is crucial for diagnosis and effective treatment. We aim to evaluate the microbiological profile and diagnostic yield of various methods in spinal TB cases at a tertiary care centre and to correlate with clinical features and treatment outcomes. METHODS: A prospective study was conducted on samples from patients with suspected spinal TB (n = 58) from July 2023 to September 20

Two needles, one haystack: Lessons from spinal tuberculosis mimicking lumbar epidural complication in a post-partum patient.

While both considered rare in contemporary Australian anaesthetic practice, the symptoms of epidural complication can mimic those of spinal tuberculosis, as we outline here in this unusual case. A 33-year-old woman presented to hospital with new neurological deficits days after receiving a lumbar epidural for labour analgesia. A complication of epidural was initially suspected; however, spinal tuberculosis was found be the cause. This case demonstrates many of the challenges and unique requirements involved in providing neuraxial anaesthesia to the obstetric cohort, namely: consenting linguist

Unmasking the cognitive burden of adult tuberculous meningitis: A systematic review and meta analysis.

INTRODUCTION: Tuberculous meningitis (TBM) is a devastating form of extrapulmonary tuberculosis with high mortality and substantial long-term neurological sequelae. Cognitive impairment is a particularly debilitating outcome yet remains under-recognised and understudied. However, heterogeneous methodologies and tools remain a major barrier to pooling data and forming clinical guidelines. AIMS AND OBJECTIVES: To evaluate the prevalence, patterns, and determinants of cognitive impairment among adult TBM patients. METHODOLOGY: A total of 761 records were identified, and six studies met the inclus

Utility of nucleic acid amplification tests in the diagnosis of osteoarticular tuberculosis.

Osteoarticular TB (OATB) is a manifestation of extrapulmonary TB (EPTB), wherein spinal TB constitutes ∼50% of OATB individuals. Diagnosis of OATB is quite difficult owing to deep inaccessible lesions within tissues, scanty bacterial load and imitation of disease with other osteoarticular conditions. OATB diagnosis mostly relies on clinical features and imaging in endemic countries, however, it is difficult to distinguish between OATB and inflammatory arthritis, i.e. rheumatoid arthritis, gout, ankylosing spondylitis, etc. using these modalities. Other laboratory tests include bacteriological

Understanding ischemia in children with tuberculous meningitis (iThemba): a protocol paper.

BACKGROUND: One million children develop tuberculosis (TB) each year and a quarter of these die. TB meningitis (TBM) is the most severe form of TB disease and even if diagnosed and treated, 20% die and over 50% of survivors are left with permanent neurological disability. Much of the morbidity and mortality associated with TBM is due to infarction yet, despite this, our understanding of the pathogenesis of infarction in TBM remains limited, especially in children. METHODS: The iThemba study (Understanding ischemia in children with tuberculous meningitis), aims to recruit 100 children with prob

Identification of an autophagy-related novel signature for spinal tuberculosis: a multi-cohort machine learning study and Mendelian randomization analysis.

BACKGROUND: Spinal tuberculosis (STB) is an infectious disease caused by Mtb with unclear diagnosis and molecular mechanisms. Autophagy is reported to be associated with the pathology of spinal tuberculosis. The present study intends to elucidate the role of autophagy-related miRNAs and genes in STB. METHODS: Core miRNAs were identified through WGCNA and differential analysis. A total of 113 machine learning algorithms were used to develop a diagnostic model. Target genes were predicted and overlapped by TargetScan, miRDB, and miRTarBase. The two-sample Mendelian randomization analysis was ut

Tuberculous abscess of the left gluteal region and iliac fossa following transvaginal ultrasound-guided oocyte retrieval: a case report.

Osteoarticular and soft-tissue tuberculosis is relatively uncommon in clinical practice and is often characterized by insidious onset and non-specific symptoms. Its manifestations may mimic those of degenerative disorders, frequently leading to delayed or incorrect diagnosis. These challenges are further amplified when the infection occurs at rare, procedure-related sites, which are easily overlooked. We report the case of a 31-year-old woman who developed persistent pain in the left gluteal and lower abdominal regions after ultrasound-guided transvaginal ovarian needle drilling (UTND) perform
