Infectious facade of autoimmunity: when SLE masquerades as tuberculosis.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder characterised by widespread inflammation affecting multiple organ systems. Its varied clinical manifestations often complicate diagnosis, as it can closely mimic other conditions, particularly infectious diseases such as tuberculosis. This case report describes a woman in her early 20s initially suspected of having disseminated tuberculosis and treated accordingly due to overlapping symptoms, including fever, night sweats, lymphadenopathy and bilateral exudative pleural effusion. Subsequent evaluation, however, confirmed a dia

A retrospective study on unfavorable 28-day neurological outcomes of stage II TB meningitis.

Tuberculous meningitis (TBM) is often associated with adverse neurological outcomes, however, there is a lack of relevant research. The aim of this study is to explore the risk factors that affect the poor prognosis of 28-day neurological function in patients with stage II TBM. A retrospective analysis was conducted on the clinical data of patients with stage II TBM who visited our hospital from January 1st, 2018 to August 1st, 2019 based on the staging criteria of the Medical Research Council. The neurological function prognosis after 28 days of follow-up was determined to establish independe

Imaging in Pott's spine: A review of typical and atypical imaging features and diagnostic challenges.

Pott's spine is associated with high morbidity, long-term disabling sequelae and even mortality. The classical paradiscal form, where there is destruction of end plates of adjacent 2-3 vertebral bodies along with intervening disc involvement and associated paravertebral abscess is well recognized. However, due to its varied clinical and radiological presentations, it remains a diagnostic challenge often resulting in delay in diagnosis. This is especially true of countries where tuberculosis has been almost eliminated but now are witnessing increase in incidence both due to immigration and HIV

Concurrent brain infection of herpes simplex virus and tuberculous meningitis complicated by tuberculous abscess: a case report.

BACKGROUND: Herpes simplex encephalitis is the most common form of encephalitis, while tuberculous meningitis is a rare condition accounting for less than 1% of all tuberculosis cases. Since the symptoms are similar in both diseases, differentiating between them is a clinical challenge, even after performing cerebrospinal fluid analysis, as the final diagnosis remains based on the specific investigations that were unusually positive for both diseases in our patient, which have not been reported previously, making this case the first in medical literature on this subject. CASE PRESENTATION: A

A Clinico-Immunological Perspective of Paradoxical Reaction in HIV-ve Tuberculous Meningitis with Therapeutic Possibilities.

Tuberculous meningitis (TBM) is a critical form of tuberculosis with high morbidity and mortality. Paradoxical reactions (PR) are frequently observed in neurotuberculosis, with diverse manifestations. A selective immune dys/upregulation leads to cytokine surge. The exact immune pathogenesis is not known. This review explores the established literature-based knowledge in PR in HIV-ve TBM to visualize the gray zone in neurotuberculosis and possible immune therapeutic adventures. We undertook this systematic review as per the preferred reporting items for systematic reviews and meta-analyses guid

Accidental needle inoculation of bacillus Calmette-Guerin vaccine in medical worker - TBC arthritis.

Despite the increase of mycobacterial infection, the incidence of tuberculous hand infection remains extremely low. We report a case of a 51-old pediatric nurse, who inadvertently inoculated her index finger via needlestick, while performing BCG immunization of the baby. Two days after the event, painful swelling and erythema of the distal interphalangeal joint region of the index finger appeared. Further diagnostic confirmed rare case of mycobacterial finger joint infection. It is important that hospital stuff should be informed about the possibility of BCG accident transmission to reduce the

Comparison Between the Efficacy of Oblique Lumbar Debridement Using an Expandable Channel Combined With Posterior Percutaneous Internal Fixation and Traditional Anterior-Posterior Surgery for Single-Segment Lumbar Tuberculosis.

AIM: Lumbar tuberculosis can cause spinal instability and neurological deficits, often requiring surgery. Traditional anterior-posterior surgery is effective but highly invasive, leading to greater trauma and longer recovery. Minimally invasive techniques, such as oblique lumbar debridement with posterior percutaneous fixation, may reduce surgical damage and improve recovery. However, their efficacy remains unclear. This study compares this minimally invasive approach with conventional surgery to assess its feasibility as an alternative treatment. METHODS: A retrospective analysis was conduct

Circulating MicroRNAs as diagnostic indicators in tuberculous Meningitis-A case -control study in North Indian population.

PURPOSE: Tuberculous meningitis (TBM), a severe and often fatal form of tuberculosis, showing high mortality and long-term neurological sequelae. Recent evidence suggests that microRNAs play a crucial role in TBM pathogenesis and may serve as potential biomarkers for diagnosis and disease progression. METHODS: Eight TBM patients and three healthy controls were recruited. Whole blood samples RNA was extracted and processed. Sequencing was performed on the Illumina platform with a 50-bp single-end read configuration. Data preprocessing included quality control (FastQC), adapter trimming (Cutada

Inhibition of miR-199b-5p Suppresses the Tuberculosis-Induced Inflammation in Spinal Tuberculosis via Targeting Gcnt2.

Spinal tuberculosis (STB) is more common in children and young adults and can lead to neurological deficits and spinal deformity in severe cases. Therefore, it is important to find a biomarker for studying the pathogenesis of STB. RT-qPCR was utilized to measure the expression of miR-199b-5p in STB. Moreover, the GPL33147 database was utilized to validate the expression of miR-199b-5p in both STB patients and healthy individuals. Pearson's correlation analysis was employed to explore the correlation between miR-199a-5p and Glucosaminyl (N-acetyl) transferase 2 (Gcnt2) in peripheral blood monon

Tuberculous monoarthritis in a rituximab-treated patient with rheumatoid arthritis: diagnostic and therapeutic challenges in a tuberculosis-endemic region.

A woman in her 60s with seropositive rheumatoid arthritis developed tuberculous arthritis of the wrist following rituximab therapy, despite negative latent tuberculosis (TB) screening. Although rituximab is considered safer than anti-TNF agents in terms of TB risk, this case highlights a rare yet serious complication. Diagnosis was confirmed via synovial fluid analysis after persistent monarthritis unresponsive to standard RA therapy. Management was complicated by drug-induced liver injury and ototoxicity, leading to multiple modifications of anti-tubercular therapy. The therapeutic dilemma ce
